The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to...other hazards. The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely impacted its fitness and capability to safely maneuver the Houston Ship Channel. 1530 - Radar- All ships of 3,000 gross tons and upwards shall have a 9 GHz radar, or other means, to determine and display the range and bearing of radars transponder and in addition a 3 GHz radar. Vessel requested a deviation to enter the port for inoperable radar - SOLAS 74a V/19

0830- Pipes - Each transfer pipe system must not leak under static liquid pressure at least 1.5 times the maximum allowable working pressure. Vessel did not conduct a static liquid pressure test in accordance with US regulations - 33 CFR 156.170.

Both port and starboard lifeboats failed to be launched from the stowed position. Vessel must prove proper operation of both the port and starboard lifeboat launching arrangements to the satisfaction of class. The class report must detail the cause of the malfunction. Vessel must also file a ISM non-conformity to the company, prior to departure and the satisfaction of class.
