A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry (1) Six hand red flare distress signals, and (2) Six hand orange smoke distress signals. Vessel had 06 red flares, but only 03 orange flares. Obtain 3 additional orange flares to match vessel route requirements.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First-aid kit currently onboard had all expired medicine. Replace with non-expired items.

A vessel must carry either a first-aid kit approved under 160.041 or a kit with equivalent contents and instructions. The first-aid kits onboard the vessel were past the expiration dates. O/O is to replace expired kits.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel's #2 generator air filter had broken off and is no longer attached to engine. O/O is to renew #2 SSDG's prime mover's air filter.

All electrical equipment, including switches, fuses, lampholders, etc., must be suitable for the voltage and current utilized. Vessel's primary generator was found to have a cartridge style fuse missing from its' holder in the vacinity of the windings housing. O/O is to provide electrical system schematics and if the fuse required; replace IAW approved schematics. 46CFR183.220(b)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel's primary generator's electrical windings were found to be excessively dirty, causing a potential fire hazard. O/O is to have the primary generator's windings cleaned and re-inspected to the satisfactory of attending marine inspector. 46 CFR 183.200(b)

Wire connectors utilized in conjunction with screw type terminal blocks must be of the captive type such as the ring or the flanged spade type. Vessel's operating station equipment terminal block had unapproved spade type connections which were not of the captive type. O/O is to provide a repair proposal to the OCMI for review and approval, detailing corrective actions to bring the operating station's terminal block connections into compliance. 46 CFR 183.340(k)

Controls and valves for operation of fixed gas fire extinguishing system must be: (i) Located outside the space protected by the system; and (ii) Not located in a space that might be inaccessible in the event of fire in the space protected by the system. The fixed suppression system controls (consisting of two 108 cubic inch nitrogen charge bottles, one 30 second time delay bottle, and pressure sensors) are inside the space (engine room) they are meant to protect. These controls must be moved outside of the space. 46 CFR 181.410(b)

Electrical equipment on a vessel must be installed and maintained to Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Two florescent lights found with no covers in main deck galley. If light bulbs smashed, shards will enter ice machine or possibly start fire from nearby liquor bottles.

All vessels must have navigation lights. Crack in port navigation light housing found.

Each disposable hydrostatic release unit (HRU) must be marked with an expiration date of two years after the date on which the unit is installed. HRU on IBA found expired.

EAP education program must display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Hot-line sticker not found onboard.

Each vessel certificated on LBS in cold water must be provided with life floats of an aggregate capacity of 50% of the total number of persons permitted onboard. IBA not onboard due to servicing. Restricted to 1NM from shore. 46 CFR 180.207(b)

Each vessel that operates on the high seas must have on board an accepted Cat. 1, 406 MHz EPIRB, installed to automatically float free and activate. Battery and HRU were expired. Restricted to 3NM from shore. 46 CFR 180.64

Materials used in vital piping systems must meet the requirements in 46 CFR 182.710. The bilge line from stateroom 2 has a temporary flexible hose near the bilge manifold in the engine room. Repair prior to drydock credit. 46 CFR 182.710

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. The emergency light in stateroom 1 was not operational.

Each overnight accommodation space on a vessel with overnight accommodations for passengers must be fitted with an independent modular smoke detection and alarm unit that is UL217 approved. Port and stbd aft most overnight accommodations spaces do not have UL217 smoke detectors installed. Install smoke detectors as required. 46CFR181.405(c)

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by §185.510 of this part and, when applicable, the duties listed in the station bill required by §185.514 of this part. Training conducted on a sister vessel may be considered equivalent to the initial and quarterly training requirements contained in paragraph (a) of this section. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. 46CFR185.420

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Exposed electrical wire found in forward most accommodations space behind modular smoke detector. Remove exposed cable or properly terminate in a electrical enclosure. 46CFR183.340(g)

Vessels on a Limited Coastwise route must carry six hand red flares and six hand orange smoke distress signals. The vessel is missing 02 hand red flares. Vessel shall only operate on a Lakes, bays and sounds route until this deficiency is corrected.

Fire extinguishers shall be subjected to maintenance at intervals of not more than 01 year as per NFPA 10. The 2-A extinguishers in state rooms #1, #2, 3 and #4 have not been annually serviced since 2017.

A vessel must carry either a first aid kit approved under 140.041 or equivalent. The vessel's first aid kit is inadequate and does not have all supplies listed in 46CFR160.041.

A portable electric cord or cable must be in compliance with 46CFR111.60-13 for a flexible electric cord or cable. There are extension cords in the Saloon that are not approved under 46CFR111.60-13.

Emergency instructions shall be mounted at the operating station. The emergency instructions were not mounted at the operating station.

Each vessel certificated to operate on LBS in cold water must be provided with life floats of an aggregated capacity that will accommodate at least 50% of the total number of persons permitted onboard. IBA off vessel getting serviced. 46CFR180.207(b) Restricted to 1NM from shore

The master of the vessel shall ensure that the EPIRB's battery is replaced before the expiration date. Each disposable hydrostatic release must be marked with an expiration date of 2 yrs. Each life ring buoy must be marked clearly legible in block capital letters with the vessel's name. Topside lifebuoy does not have vessel's name marked on it. 46CFR185.604(b) 15NOV2018

A vessel must carry adequate and up to date charts of large enough scale to make safe navigation, Tide tables and current tables. Missing current tide tables, current tables and charts. 46CFR184.420(1)(4)15NOV2018

The master of the vessel shall ensure that EPIRB be tested monthly and shall be logged. No documentation shown for testing and logging monthly EPIRB tests. 46CFR728(a)(c) 01DEC2018
