Nothing in this subpart shall be construed as limiting the inspector from making such test or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. Port and stbd after legs 10 year not completed IAW CG-543 Policy Letter 07-02. 10 year shall be completed prior to operations.

After any survey of the vessel made under 42.09-25 to 42.09-40, inclusive, as applicable, has been completed and deficiencies corrected as provided, no change shall be made in the vessel's structure, equipment, arrangement, material, or scantlings as covered by such survey, without the prior specific approval of the assigning and issuing authority. Vessel failed to notify class of repairs and hull replacement prior to commencing work. Each portable fire extinguisher approved under subpart 162.028 of this chapter and each semi-portable fire extinguisher approved under subpart 162.039 of this chapter must be installed in compliance with Table 132.220 of this section. The following spaces are missing proper portable fire extinguishers (PFEs): Galley does not have a 40-B:C PFE, and the Paint Locker does not have a 40-B PFE.

Each vessel of 100 or more gross tons, or 19.8 meters (65 feet) or more in length, must have navigational lighting in compliance with the applicable navigation rules and with § 111.75-17(d) of this chapter. Port and starboard navigation lights are deteriorated, not in proper working order and must be certified by an independent laboratory to the requirements of ANSI/UL 1104 or EN 14744 (incorporated by reference; see § 110.10-1 of this subchapter) or an equivalent standard under § 110.20-1 of this subchapter and must be labeled as such.

Each lighting fixture in an electrical system operating at more than 50 volts must comply with UL 595, “Marine Type Electric Lighting Fixtures.” A lighting fixture in an accommodation space, radio room, galley, or similar interior space may comply with UL 57, “Electric Lighting Fixtures,” UL 1570, “Fluorescent Lighting Fixtures,” UL 1571, “Incandescent Lighting Fixtures,” UL 1572, “High Intensity Discharge Lighting Fixtures,” UL 1573, “Stage and Studio Lighting Units,” or UL 1574, “Track Lighting Systems,” as long as the general marine requirements of UL 595 are satisfied. Light fixtures in state rooms #8 & #9 are missing the required shields.

Each vessel must undergo dry dock examinations as follows: vessels that operate in salt water must be examined IAW Table 91.40-3(a) of this section. A single hull ship must be examined at a 2.5 year examination interval, which means it must undergo two examinations within any five years period. No more than three years may elapse between any two examinations. Marine Inspectors confirmed with vessel operator that their dry dock date was set for 07JAN2021, but the vessel did not go into dry dock until 22JAN2021.

Noted bilge discharge pipe in the stbd engine room had a soft patch temp repair. Make proper repairs to holed bilge discharge pipe located in the stbd engine room.

Eliminate potential fire hazard. Clean all excessive cooking grease in the galley grill's hood smoke extract fan and ducking.

Reposition emergency light fixture to provide illumination to the escape route from the engine room.

Conduct man overboard drills and prove rescue boat can be deployed and operated in a timely manner. Outboard engine could not be operated during drill.

Replace Pressure Relief Valve on Air Recievers IAW NVIC 8-91 (46 CFR 54.03)

Vessel will carry 36 persons due to 3 lifefloats at 12 persons IAW NVIC 8-91 [46 CFR 133.105(c)]

Under sized weak link installed.

Tachometer not operational for the Center MDE

Remote shut down for center MDE does not work.

Loose wire for deck light switch located in the galley.

Plastic seam was coming off.

Permanently secure battery box to deck located on flying bridge.

Properly configure life floats to sea painter and weak link.

Properly secure shore connection opening to switchboard.

Properly affix NOAA registration sticker onto EPIRB.

Vessel is to immediately stop all operations and proceed directly to Galveston for a RIN

Prove proper operation of general alarm bell and light in port E/R.

Reposition crankcase breather on port MDE to vent into the engine air intake and not onto the exhaust/lagging on port MDE.

Renew oiled exhaust lagging on port MDE.

Renew /repair ground indicator lights on main switchboard for both generators and prove proper operation.

Secure loose hanging wire for the overspeed device on the starboard generator.

Provide 2 light guards for lights in tool room and replace broken covers in engine rooms.

Replace line on starboard fwd ring bouy with black polypropylene line.

Move deep fat fryer emergency disconnect from behind fryer to an accessable location away from fryer.

Repair fuel leak on port MDE at fuel pump.

Prove proper operation of low lube oil pressure gauge on starboard generator.

Provide approved automation proceedures onboard vessel. Proceedures were previously approved but not located onboard.

Install emergency lighting for port and starboard engineroom emergency escapes and provide additional lighting for port and starboard embarkation areas.

Make repairs to hull/structural damage in way of #1 centerline tank.

Make repairs to fracture inway of #1 starboard tank by foward leg tower brace.

Make checkvales for preload dump valves operational.

Provide NDT and inspection report for leg to pad connections and pad inspection.

Provide crane servicing report and certification for main crain upon completion of repairs.

Submit machinery automation procedures in accordance with NVIC 1-78 to cognizant USCG Marine Safety Office for approval.

PROVE AUTOMATION PROCEDURES ONCE REVIEWED BY COGNIZANT MARINE SAFETY OFFICEAND PROVE SAME TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

SUBMIT UPDATED FIRE AND SAFETY PLAN TO COGNIZANT COAST GUARD MARINE SAFETY OFFICE FOR VERIFICATION AND APPROVAL. PLAN SHOULD BE IN ACCORDANCE WITH IMO RESOLUTION AND NVIC-8-84.

SUBMIT MACHINERY AUTOMATION PROCEDURES IN ACCORDANCE WITH NVIC 1-78 TO COGNIZANT MARINE SAFETY OFFICE FOR REVIEW.

PROPERLY INSTALL EMBARKATION LADDER FOR RESCUE BOAT TO THE SATISFACTION OF THE ATTENDING CG MARINE INSPECTOR.

PROVE EMBARKATION LIGHTS ILLUMINATE AREA IN WAY OF LIFERAFTS.

PROVIDE COMPLETE APPROVED OPERATIONS MANUAL INCLUDING APPENDICES, OR SUBMITUPDATED MANUAL TO COAST GUARD MARINE SAFETY CENTER FOR APPROVAL.

PROPERLY INSTALL MAIN ENGINE OVERSPEED SHUTDOWNS AND PROVE TO THE SATISFACTION OF THE ATTENDING CG MARINE INSPECTOR.

INSTALL INSERT OR VENT CLOSURE DEVICE FOR AIR COMPRESSOR ENGINE IN WAY OF CENTER ENGINE ROOM STARB SIDE.

INSTALL CURRENT 406 EPIRB

INSTALL 110V OUTLET FOR PORTABLE AIR BLOWER IN WAY OF CENTER ENGINE ROOM FOR AIR COMPRESSOR ENGINE.

Provide proof of weight test for small crane. Crane last tested 16JUN97.

Provide proof of annual inspection on all portable fire extinguishers. Lastinspection was performed on 15JUL98.

Provide proof of annual inspection for both life rafts. Last inspection was 24JUL98.

Remove excessive oil from engine room bilge.

Make vessel available for inspection of various metal inserts in the hull and main deck. (see MINS for details)

OPSMAN outdated; must be submitted for approval in accordance with NVIC 8- 91 and 46 CFR 109.

Vessel must submit previously approved plans & new plans to MSC for review and approval.

Provide on board - fuel oil transfer piping diagram.
