Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to:propulsion and power supply systems;The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Vessel installed a new engine 2 years ago without prior approval and is reduced to operate as a 6 pack until installation can be verified to satisfaction of OCMI.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jackets were stored placed on top of adult life jackets.

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present. Engine space was found to have four open through hull fittings. 1 Fwd Stbd, 1 Stbd Qtr, 2 Transom

“Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessels posted instructions were found to be limited and non descriptive.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. No placard was posted on board.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Kit was no in compliance with requirements of 46 CFR 199.175(b)(10).

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Vessels anchor chain was found to be corroded to unsafe operable conditions.

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Lights for bilge pumps were inoperable at the operating station.

When using DSC techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the Commission or its designees. Radio was not programmed with assigned MMSI.

Fuel shutoffs were not properly labeled.

Vessel did not have applicable charts on board.

A PRE-ENGINEERED FIXED GAS FIRE EXTINGUISHING SYSTEM MUST: (3) AUTOMATICALLY SHUT DOWN ALL POWER VENTILATION SYSTEMS AND ALL ENGINES THAT DRAW INTAKE ARE FROM WITHIN THE PROTECTED SPACE. FOUND THAT WHEN THE OPERATOR DISCONNECTED THE WIRE TO SIMULATE ENGINE SHUTDOWN, IT DID NOT SHUT THE ENGINES DOWN. PROVIDE PROOF TO THE ATTENDING MARINE INSPECTOR THAT THE SYSTEM WORKS PROPERLY.

(A)AN ADULT LIFE JACKET MUST BE PROVIDED FOR EACH PERSON CARRIED ON BOARD A VESSEL. (B)IN ADDITION, A NUMBER OF CHILD SIZE LIFE JACKETS EQUAL TO AT LEAST 10% OF THE NUMBER OF PERSONS PERMITTED ON BOARD MUST BE PROVIDED. FOUND 12 ADULT AND 2 CHILD LIFE JACKETS WITH TEARS AND PUNCTURE MARKS THAT WERE TO LIFESAVING CAPABILITIES. PROVIDE PROOF OF OPERATIONAL LIFE JACKETS TO THE ATTENDING MARINE INSPECTOR.

Vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel owner did not have vessel ready for inspection 3 months before or after anniversary date. Vessel owner must contact OCMI office to schedule inspection. (46 CFR 176.500(b)(1))

Pre-engineered fixed fire extinguishing system visual and audible alarm and override function were found inoperable. A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) a light to indicated discharge, (2) an audible alarm that sounds upon discharge; and (3) a means to reset devices used to automatically shutdown ventilation systems and engines as required. 46 CFR 181.420(b)

Found high water level audible and visual alarm in forward compartment inoperable at the operating station. Replace/repair to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 182.530

Found engine room ventilation covers inoperable. Repair/replace ventilation covers to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 182.465(h)

Portable fire extinguisher bracket not IAW manufacturer design. Replace with manufacturer specified mounting bracket. 46 CFR 162.028-3(c)

Light list was found out of date. Update light list to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 184.420(a)(3)

Floating waterlight attached to ring life buoy had an expired battery.
