46 CFR 177.13-1. Each vessel must meet the applicable requirements in Subchapter S of this chapter. Bulkhead penetration in generator room, forward bulkhead centerline.

46 CFR 120.432(a). Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew spaces located below the main deck. Generator room emergency lighting inoperable.

46 CFR 176.25-50(b). At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Generator room return fuel valve is leaking, centerline aft near overhead.

46 CFR 183.10-20(k). Junction boxes, connection boxes, and outlet boxes, for use in damp or wet locations shall be of watertight construction. Bilge alarm wiring in steering compartment is not in a watertight enclosure.

46 CFR 185.30-5(c). All life preservers and ring life buoys shall be marked with the vessel's name. Vessel name missing on several life rings.

46 CFR 176.25-50(a). At each initial and subsequent inspection for certification, reinspection and at . every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Chlorine tank is severely corroded and prone to leaks/structural failure. Must be renewed.

46 CFR 182.20-5(b) Gauges to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. RPM Gauge for port main engine is inoperable.

46 CFR 181.15-10(c). L. Fire hose shall be 1% inches in diameter and 50 feet in length. Incorrect fire hose size, aft main deck

46 CFR 119.455 (b)(3). Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the tank compartment, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the compartment and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Must install a remote valve actuator for fuel tank emergency shutoff valve.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] circuits added for new oven installation with out oversite. Make repairs or alterations in compliance with 46 CFR 115.700 and any other applicable laws and or regulations. ovens were installed in a passenger seating area withou oversight and must be removed prior to carrying passengers.

Vessel found to be transporting +2000 gal of diesel fuel in unapproved plastic containers on an open deck. Remove diesel prior to movement.

Penetrations in boundaries for electrical cables, pipes, trunks, ducts etc. must be constructed to prevent passage of flame and smoke. Bulkhead penetrations in space forward of engine room is not sufficiently closed off. Vessel must ensure passage of smoke and flame through the bulkhead is prevented to the satisfaction of the local OCMI. 46 CFR 116.415

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. A second fuel supply line to engine room was found without a remote operation function to the valve. Vessel shall close valve and not use fuel supply line until a remote valve that meets regulation is installed. Vessel shall install valve NLT February 2025 to the satisfaction of the local OCMI. 46 CFR 119.455(b)(3)

System design and installation must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system by the Commandant. Vessel shall install and test the approved fixed fire suppression system IAW the manufacturers instructions/NFPA 2001 to the satisfaction of the attending Marine Inspector and the attending Certified Fire Inspection Company that is conducting the test prior to movement. 46 CFR 118.410(a)(3)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Once the Fixed Fire Suppression system is installed and tested to the satisfaction of the OCMI, the vessel shall conduct training and a fire drill with the crew to ensure crew is familiar with their duties prior to movement. 46 CFR 122.524(a)

A fixed gas fire extinguishing system aboard a vessel must be approved by the Commandant, and be custom engineered to meet the requirements of this section. Vessel has approved plans from MSC Dated September 14th 2022. However, plans do not reflect current piping arrangement/diagram, location of suppression system, or current flow calculations. Vessel shall obtain approved drawings/plans for current installed Fixed Fire Suppression system to reflect approved system location, piping diagram, flow capacity, and all other additional requirements IAW ASTM/NFPA 2001, 46 CFR, and Fireboy System Manufacturers Instructions.

Vessels engineered fixed fire suppression system is undergoing modifications to meet regulations. Vessel owner shall submit plans to MSC for approval prior to completing modifications to the pipping system and moving the suppression systems cylinder to the adjacent space (Void #6). The pipping of the system shall be in accordance with the approved drawings at the time of inspection. Due August 25th 2023.

The Commanding Officer, USCG Marine Safety Center, approves the design and construction of devices to be certified for installation and operation on board inspected vessels on the basis of tests and reports of inspection under the applicable marine engineering requirements in subchapter F of Title 46, Code of Federal Regulations, and under the applicable electrical engineering requirements in subchapter J of Title 46 Code of Federal Regulations.

Except as otherwise provided in this paragraph all unfired pressure vessels shall be Coast Guard approved and installed to the satisfaction of the Officer in Charge, Marine Inspection. The requirements for design, construction and original testing of such approved unfired pressure vessels are in applicable regulations in Subchapter F (Marine Engineering) of this chapter. Information concerning the applicable regulations may be obtained from the Officer in Charge, Marine Inspector. 46 CFR 182.10.5(d), 46 CFR 54.15-10 two pressure vessels not subject to subchapter F are onboard servicable and do not have safety relief valves.

An inspection for renewal of a certificate of inspection shall include an inspection of the structure. 46 CFR 176.05-109a) Repair hole in deck in forward part of galley/concession area.

Lakes, bays, and sounds, and rivers routes. A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel had six non approved road flares onboard.

(a) An adult life jacket must be provided for each person carried on board a vessel. (b) In addition, a number of child-size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided to meet this section, except that: (1) Child-size life jackets are not required if the vessel's Certificate of Inspection is endorsed for the carriage of adults only, or (2) When all “extended size” life preservers (those with a lower size limit for persons of 1,195 millimeters (47 inches) in height or weighing 20.4 kilograms (45 pounds)) are carried on board, a minimum of only 5% additional child size devices need be carried. (c) Except as allowed by paragraph (d) of this section, each life jacket must be approved in accordance with either §§ 160.002, 160.005, or 160.055 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.155 or 160.176. Vessel is restricted to carrying 201 adult passengers and 03 child passengers.

Fixed Fire Suppression System onboard is an engineered system (USCG approval # 162.161) and not a pre-engineered system. The storage cylinder is located inside the space protected (machinery) and is not capable of automatic operation by a heat actuator within the space. Not meeting 46 CFR 118.410(c).

Piping, including valves, pipe fittings and flanges, conveying vapor, gas or liquid, the temperature of which exceeds 150F, must be insulated where necessary to prevent injuries. The port and starboard generator set exhaust piping in the #2 void space is not suitably insulated and exceeds 150F. Install exhaust lagging and prove proper temperature readings during operation to the attending CG Marine Inspector.

If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. The #1 void bilge high level alarm is wired using twist-on type connectors not housed w/in an enclosure.

Fire extinguishing equipment installed must be designed, constructed, installed, and maintained in a manner acceptable to the Commandant. The Port Side heat detector in the #2 void space above Port Side Generator was inoperable. Prove proper operation to attending CG Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers and crew from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The generator switchboard and main distribution panel had removed circuit breakers from the panels. Reinstall circuit breaker blanks or spares in all electrical distribution panels to the satisfaction of the attending CG Marine Inspector.

Reinstall PA system and conduct an operational test in the prescense of a Coast Guard Marine Inspector.

Weld aft deck railing to prevent passenger entrance to paddle wheel section the the satisfaction of the attending Coast Guard Marine Inspector.

Install flame screen on fuel vent piping.

Provide operable 2nd VHF radio on navigation bridge.

Install markings for maximum passengers permitted on the 3rd (100) and 2nd (200 combined).

Vessel has undergone modifications to the engine room (repower) and added 2 keel coolers. In addition, the vessel has changed from a wet exhaust to a dry exhaust system. The vessel also added approx. 800 lbs of fendering system on the bow. No credit DD/ISE will be given until the Marine Safety Center has approved the plans.

Shipyard removed approx. 5-7 tons of fixed ballast without USCG being present. Vessel is to undergo a new incline test in the presence of the attending USCG marine inspecto with approved MSC incline procedures.

Based on the above modifications, the CID of Sector NOLA now requires the engine room to meet subchapter "K" with regards to a fixed fire extinguishing system to protect the engine room. Vessel is to provide a plan to be submitted to Sector New Orleans to install a new fixed fire fighting system to protect the engineering space.

Based on the above modifications, the CID of Sector NOLA now requires the engine room's strucutral fire protection (insulation) to be renewed. The owner/operator must submit a plan with a proposed timeline to replace all insulation throughout the engine room to meet 46 CFR 116.415 and NVIC 9-97 Change 1.

During the drydocking period, MIs observed all lifejackets being stowed on top of the heating/cooling ducts. Owner/Operator must replace and restow lifejackets in a suitable location and replace all lifejackets as directed by the attending MI.
