Where life preservers are stowed so that they are not readily visible to passengers, the containers in which they are stowed shall be marked "Life Preservers" with the number contained therein, in at least 1-inch letters and figures. This legend shall indicate the separate stowage of children's life preservers. Life preserver markings did not accurately reflect the number of life preservers stowed in each container and did not indicate a difference between stowage locations of adult preservers and children's preservers. Post required markings, reflecting accurate count of adult and children life preservers present in all containers used for life preserver stowage, in a conspicuous place, to the satisfaction of the attending CG marine inspector.46 CFR 185.30-10(a) "Old T"

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Present crew members did not have marine radio operator permit or higher class license present for inspection. Provide the marine radio operator permit or higher class license of a licensed crew member of the vessel to the satisfaction of the attending CG marine inspector. 47 CFR 80.159(e)(1)

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Records of chemical tests were not available for inspection. Provide records of chemical testing for all crew members to the satisfaction of the attending CG marine inspector. 46 CFR 16.260(a)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crew member assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Display and distribution of informational material and hot-line telephone number were not present on vessel. Display information and hot-line telephone number for crew member assistance to the satisfaction of the attending CG marine inspector. 46 CFR 16.401(a)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Man overboard drill not conducted. Conduct satisfactory man overboard drill in the presence of the attending marine inspector. 46 CFR 176.808(g)

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Fire drill not conducted. Conduct satisfactory fire drill in the presence of the attending marine inspector. 46 CFR 176.810(d)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel does not have an anchor light independent of the navigation lights. Provide anchor meeting the requirements of 33 CFR 83.30 to the satisfaction of the attending marine inspector. 46 CFR 183.420

A power-driven vessel of less than 12 meters in length may, in lieu of the lights prescribed in paragraph (a) of this Rule, exhibit an all-round white light and sidelights. Vessel exhibits an all-round white light and a white stern light, which is not permitted under Navigation Rule 23. Arrange vessels navigation lights to meet prescribed lights under this rule to the satisfaction of the attending Marine Inspector. 33 CFR 83.23(d)

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Mark vessel name and hailing port on stern of vessel to the satisfaction of the attending marine inspector. 46 CFR 67.123(a)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Neither the captain or the crew member aboard had a marine radio operators permit. Provide a marine operators permit to the satisfaction of the attending marine inspector. 46 CFR 184.502

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. This subpart summarizes the responsibilities of credentialed mariners, marine employers, MRO, SAP and other chemical testing service providers in 49 CFR part 40. The regulations in 49 CFR part 40 should be consulted to determine the specific procedures which must be established and utilized. DER is in random selection pool. Provide 1) Documentation the DER removed themselves from the random selection pool; 2) Documentation formally appointing a second DER to conduct the random selection process through the consortium for the DER in the random selection pool; or 3) Documentation of a disinterested third party to conduct the random selection process. EAP training program: An EAP training program must be conducted for the employer's crew members and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crew members and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. No proof of crew members and supervisory personnel training program. Provide documentation of company employees completing the training program described above. Also, provide proof of supervisory personnel completion of required additional 60 minutes of training for reasonable suspicion training to the satisfaction of the attending marine inspector. 46 CFR 16.401(b)

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A means to reset devices used to automatically shut down ventilation systems and engines. Pre-engineered fixed gas fire system's override switch inoperable. Repair and demonstrate that engines can be restarted to the satisfaction of a marine inspector. 46 CFR 181.420(b)(3)

Each ring life buoy on a vessel must: (1) Be approved in accordance with § 160.050 in subchapter Q of this chapter; Retro-tape degraded and not functional. Replace retro tape in kind. 46 CFR 180.70(b)(1)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: space with a through-hull fitting below the deepest load waterline. The space below the operating station contained multiple through hull fittings below the waterline, with no high water alarms present. Install high water alarm in space to the satisfaction of marine inspector. 46 CFR 182.530(a)(3)

A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 65 feet in length carrying not more than 49 passengers, other than a ferry, provided that: The pump is listed by an independent laboratory as meeting the requirements in UL 1113. All submersible electric bilge pumps are not UL 1113 compliant. Install UL 1113 compliant submersible electric bilge pumps. 46 CFR 182.520(e)

Conductors must be protected in accordance with their current carrying capacities. If the allowable current carrying capacity does not correspond to a standard device size, the next larger overcurrent device may be used provided it does not exceed 150 percent of the conductor current carrying capacity. Bilge pump manual/auto rocker switch fuses 20amp verse 15amp. Install 15amp cartridge fuses. 46 CFR 183.380(d)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Bilge pumps and high level alarms have blade type fuses. Install cartridge type fuses. 46 CFR 183.380(l)

Electrical equipment on a vessel must be installed and maintained to: minimize accidental personnel contact with energized parts. Both engine compartments main power positive terminal studs missing protective cover. Provide cover. 46 CFR 183.200(c)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First aid kit missing ammonia inhalants. Provide. 46 CFR 184.710

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Missing heavy weather emergency instructions. Provide. 46 CFR 185.510(a)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Missing both required portable lights. Provide. 46 CFR 183.430

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Missing placard for Kapok life jacket donning instructions. Provide. 46 CFR 185.516

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel does not have any means to test for presence of alcohol. Provide. 46 CFR 185.212/46 CFR 4.06-15(a)

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers or crews. Starboard bow railing crack in multiple locations. Repair to the satisfaction of the attended MI. 46 CFR 177.35-1(a)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Relocate pull cables so they no longer obstruct the supply from completely closing. 46 CFR 182.465(h)

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Vessel has eight red hand flare distress signal and three orange smoke distress signals. Vessel restricted to a Lakes/Bays/Sounds route or 30 min runs. 46CFR180.68(a)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Labeling of life jacket stowage is not separate or marked directly on the location of the child size life jackets. Provide proper labeling and stowage of life jackets. 46CFR180.78(a)(5)

Placards containing instructions for the donning and use of the life jackets abroad the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel has no donning instructions posted. Post life jacket donning instructions where clearly visible to passengers. 46CFR185.516(a)

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. All medications in the vessel's first aid kit are expired. Replace all expired contents of first aid kit. 46CFR184.710

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Both main engines have exposed rotating belts. Install belt guards on both the port and starboard main engines. 46CFR177.960

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40; Chemical drug testing program for vessel is not in accordance with procedures. Vessel is not apart of a consortium. Register vessel and employees into a consortium.

An adult life jacket must be provided for each person carried on board; 17 adult life jackets were provided on board though the vessel is certificated to carry 19 people. Provide 2 additional adult life jackets to meet the certificated amount. ** A total of 17 people are allowed on vessel until deficiency is rectified.**

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Coast Guard Light List, Tide Tables, and U.S. Coast Pilot; Vessel's publications are out-to-date. Provide up to date publications for Coast Guard List, Tide Tables, and U.S. Coast Pilot.

A vessel not required to have a power driven fire pump must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed as easily available during an emergency; Fire buckets are missing required lanyard. Attach a 3-6 foot lanyard to each fire bucket.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation; Remote fuel shutoff stations are not marked. Mark remote fuel shutoff stations.

On passenger decks of a ferry or of a vessel on an excursion trip the following must be installed: bulwarks, chain link fencing or wire mesh that has openings of not more than 4 inches in diameter or bars, slats, rail courses or an equivalent spaced at intervals of not more than 4 inches; vessel's stern has rail courses that has spacing more than 4 inches. Provide bulwarks, bars, slats, etc. to meet requirements.

A vessel must be provided with bilge pumps in accordance with Table 182.520(a); Vessel is missing a required portable hand pump. Provide portable hand pump.

All vessels must have navigation lights that are in compliance with the applicable sections of International and Inland Navigation Rules; Vessel's stern light is inoperable. Repair stern light.

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80; Vessel's FCC Safety Certificate is not present on board vessel. Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons and the vessel from electrical equipment, and the electrical shock. Wiring located in engine space is uncovered. Provide cover for wiring in engine space to eliminate shock hazard.

EPIRB battery expired in 2016. Replace or service EPIRB and register new EPIRB, if replaced, with NOAA.

Provide 2 additional hand flares and 4 additional orange smoke distress signals.

Provide a valid FCC Safety Certificate.

Provide a FCC Radio Operator permit for the Master.

Provide 2017 Coast Pilot, tide tables, current tables, and current nautical charts for operating area.

Provide an emergency broadcast placard next to the VHF installation with vessel specific emergency instructions.

Provide vessel specific emergency instructions to include: rough weather at sea, flooding, fire, and man overboard procedures.

Prove proper operation of bilge pumps required to be in each watertight compartment.

Prove operation of the main engine compartment fire suppression system detection circuit. Removing detection circuit for one engine must automatically secure individual engine.

Make engine compartments vapor tight to seal fixed fire extinguishing agent in compartment when released.

Provide manufacturer specifications for newly installed alternator on main engine.

Provide electrical drawings for all systems currently installed on the vessel.

Install 30 x 30 flame screens on fuel vent pipe outlets port and starboard.

Provide suitable covers for terminal connections located in the engine compartment.

Provide a suitable hull strainer for the circulating salt water intake line on the diesel propulsion engine system.

Provide three 2 1/2 gallon fire buckets with an attached lanyard.

A metal strainer or filter shall be provided in the main engine fuel supply line. Stainers must be of the top opening type. Strainer and filter bowls shall be metal. Glass strainers are accepted if approved under ASTM F1201 standard.
