Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. The drills of the crew must take place within 24 hours of the vessel leaving a port if more than 25 percent of the crew have not participated in abandon-ship and fire drills on board that particular vessel in the previous month. The last fire and abandon ship drill, prior to drydocking the vessel on 26FEB24, was performed on 21DEC23 and was not properly documented. Prior to 21DEC23, the last abandon ship drill performed was on 29MAY23, approximately 7 months earlier. Vessel shall develop a process to ensure crewmembers are performing the required drills on a monthly basis prior to departure and provide CG with evidence of owner/operator plan to prevent future non-compliance.

Materials used in the manufacture of tubing, pipes, valves, flanges, and fittings shall be selected from those specifications that appear in 46 CFR 56.60; or they may be selected from the material specifications of section I or section VIII of the ASME Boiler and Pressure Vessel Code if not prohibited by the section of this subchapter dealing with the particular section of the ASME Boiler and Pressure Vessel Code. Marine inspector identified an unapproved modification of hydraulic jacking system to include; approximately 10' of clear vinyl tubing connected to approximately 30' of flexible non metallic hose. This hose assembly led to the hydraulic fluid tank fill opening on deck therefore bypassing the installed return system.
