Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. The drills of the crew must take place within 24 hours of the vessel leaving a port if more than 25 percent of the crew have not participated in abandon-ship and fire drills on board that particular vessel in the previous month. The last fire and abandon ship drill, prior to drydocking the vessel on 26FEB24, was performed on 21DEC23 and was not properly documented. Prior to 21DEC23, the last abandon ship drill performed was on 29MAY23, approximately 7 months earlier. Vessel shall develop a process to ensure crewmembers are performing the required drills on a monthly basis prior to departure and provide CG with evidence of owner/operator plan to prevent future non-compliance.

Each vessel owner or operator subject to this part must ensure that copies of the following documents are carried on board the vessel and are made available to the Coast Guard upon request: For vessels operating under a Coast Guard-approved Alternative Security Program (ASP) as provided in § 104.140, … a letter signed by the vessel owner or operator, stating which ASP the vessel is using and is in full compliance.

Owners or operators must implement an approved Alternative Security Program in its entirety to be deemed in compliance with either part 104…The vessel could not provide any evidence that a security drill had been performed onboard since the vessels last annual inspection. Vessel shall perform a security drill, properly document as outlined in the ASP, and ensure the vessel security officer conducts and documents a security drill at quarterly intervals as required by the vessels approved ASP being implemented.

Port M/E manual trip would not operate. Repair and show manual trip is functional to the satisfaction of the attending MI. 46 CFR 96.03-1 46 CFR 62.25-15(d)

Port M/E exhaust piping has puncture in heat shielding. Repair to the satisfaction of the attending MI. 46 CFR 56.50-1(k)

Port and starboard bilge pumps unable to draw suction during testing. Demonstrate proper function test to the satisfaction of the attending MI. 46 CFR 56.50-55

Distribution panel on the bridge is not fully enclosed due to replacement of breakers and upper E/R panel has a hole in side.Ensure panels are fully enclosed preventing access to components inside the panel and watertight. 46 CFR 113.25-15

Vessel electronic chart has no redundancy and vessel was not completing chart corrections since the last annual inspection. Vessel must ensure accurate updated electronic or paper charts are carried in duplicate with chart corrections maintained. 33 CFR 164.33

Immersion suit box on bridge wing not secured and "IMMERSION SUIT" label is faded and missing quantity of suits stowed within the box. 46 CFR 199.70(d)

Excessive power strips were found throughout crew berthing and pose a hazard. Remove power strips. 46 CFR 111.01-1(a)(2)

Emergency light on the bridge not operable. Ensure vessel E-lighting is operable. 46 CFR 112.39-3

Portion of sewage system piping located on main deck covered with tape and epoxy. Vessel shall inspect MSD system piping exposed on main deck and provide repair plan to Coast Guard replace piping as necessary with approved materials. 46 CFR 126.150

Each pump shall be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 psi. Vessel fire pump supply hose is in need of replacement.

Materials used in the manufacture of tubing, pipes, valves, flanges, and fittings shall be selected from those specifications that appear in 46 CFR 56.60; or they may be selected from the material specifications of section I or section VIII of the ASME Boiler and Pressure Vessel Code if not prohibited by the section of this subchapter dealing with the particular section of the ASME Boiler and Pressure Vessel Code. Marine inspector identified an unapproved modification of hydraulic jacking system to include; approximately 10' of clear vinyl tubing connected to approximately 30' of flexible non metallic hose. This hose assembly led to the hydraulic fluid tank fill opening on deck therefore bypassing the installed return system.

The system shall be so designed that proper functioning of any unit shall not be affected by the back pressure in the system. Crew indicated that bypass was installed due to back pressure which rendered hydraulic jacking system inoperative.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by § 91.45-1. Provide plan to repair installed hydraulic jacking system.

Vessel altered the installed bilge and fireman piping system with a portable pump and tubing in order to redirect oily bilge water. No alterations shall be made without the knowledge of the OCMI. Vessel must remove all parts of the unapproved modification and return the system to previously approved vessel designs at satisfaction of attending inspector prior to movement. 46 CFR 91.45-1

Vessel must replace or repair leaking shaft seals in order to ensure that the vessel is equipped and has the ability to retain on board all oily mixtures until it discharges them to a reception facility or has an approved oily-water separating equipment onboard to handle the oily bilge mixtures being produced. 33 CFR 155.350 (a)(1) & 46 CFR 50.20-35(a)

Vessel had expired alcohol testing strips. Vessel must ensure it has sufficient number of alcohol testing devices readily accessible onboard. Operations are restricted to those that allow vessel to conduct the required alcohol tests at a facility within 2 hours from time of occurrence of a serious marine incident. 46 CFR 4.06-15

All vessel MDE and Generator fuel filter bowls do not meet ASTM F1201 standard. Vessel must ensure that bowls are replaced or outfitted with the appropriate equipment. 46 CFR 56.15-5(b) & 56.60-1(b)

Vessel did not have tags or documentation for 02 safety relief valves installed on machinery space pressure vessels. Pressure relieving devices shall be located and installed so that they are readily accessible for inspection. 46 CFR 54.15-5(3)

Crane hand signal placard is not serviceable, vessel shall ensure placard is displayed in accordance with API specifications. 46 CFR 108.601

Vessel failed to properly maintain the Oil Record Book. 46 CFR 151.25

Each OSV, except OSVs operating in the Gulf of Mexico or on other routes between 32 degrees north latitude and 32 degrees south latitude, must carry an immersion suit of the appropriate size for each person onboard. Only 12 immersion suits were onboard during the inspection. Total persons allowed is 12 until additional immersion suits are provided.

Submit repair proposal for sections of rack that are damaged on the aft leg due to failure in #3 hydraulic motor prior to jacking.

Vessel has damaged rack teeth in STBD aft leg, vessel is restricted from lifting beyond that point until permanent repairs are made.

Two life jackets attached lights were expired, but were replaced on the spot as required per (OLD I) 46 CFR Part 94.42-1(a).

Life jacket stowage locations were not plainly marked as required per (OLD I) 94.40-15 (b)(1), but was corrected on the spot.

Both of vessel's fire axes were rotted.

Rescue boat first aid kit was expired.

Both engineroom walkways were not adequately illuminated with emergency lighting to escape hatches.

Overhead welds for an existing insert in port engineroom contained defects. Shipyard to gouge and reweld.

Per NVIC 8-91 vsl hydraulic jacking system must substantially meet the requirement of 46 CFR 58.30 and 56.30.

Automation testing procedure require USCG approval.

Demonstrate compliance with the 8th U. S. Coast Guard District's policy "Rescue Boat Requirements on Offshore Supply Vessels" dated 12NOV98 to the satisfaction of the attending Marine Inspector.

PROVIDE FOR REVIEW G-MSC APPROVED PRINTS FOR THE FOLLOWING NEWLY INSTALLED EQUIPMENT: HYDRALIC JACKING SYSTEM, BILL OF MATERIALS, HYDRALIC JACKING SYSTEM ELECTRICAL SYSTEMS, AND PANEL LAYOUT.

PLACE ON BOARD AN AMENDED OPERATIONS MANUAL AND STABILITY LETTER REFLECTINGTHE ADDITIONS OF SPONSONS, CRANE, AND ADDITIONAL LEG LENTH.

OBTAIN A LOAD LINE ASSIGNMENT AS REQUIRED BY U.S. CODE 5102 DATED 08OCT86.

SUBMIT STABILITY CALCULATION IN ACCORDANCE WITH 46 CFR SUBCHAPTER "S" TO U.S. COAST GUARD MARINE SAFETY CENTER, WASHINGTON, D.C.

SUBMIT OPERATIONAL MANUAL FOR COAST GUARD APPROVAL TO U.S. COAST GUARD MARINE SAFETY CENTER, WASHINGTON, D.C.

INSTALL OR REPAIR PUMP IN FIRE MAIN SYSTEM IN ACCORDANCE TO 46 CFR 95.10-5 (C)(D).
