Not less than one half of the total number of lifebuoys shall be provided with self igniting lights complying with the regulations of 31.2. Two lifebuoys in the vessels stern had nonfunctional lights attached. 74SOLAS(1997)III/7.1.3

Two means of escape shall be provided from a machinery control room located within a machinery space, at least one of which will provide continuous fire shelter to a safe position outside the machinery space. Wood blocks were observed preventing the emergency escape hatch from opening.

After any survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in the structural arrangements, mechanical, equipment ,and other items covered by the survey, without the sanction of the Administration. Vessel has modified its fuel oil piping from the fuel transfer pump to the purifier return line. using a plastic hose to connect the steel pipes.

The administration may permit the use of oil-level gauges with flat glasses and self-closing valves between the gauges and fuel tanks. Vessel has installed plastic hoses as sight glasses on its main engine service tank, setting tank, M.E. lube oil and aux lube oil tanks.

The company should ensure that any non-conformity is reported, with its possible cause, if known. Vessel's fuel transfer pump experienced air lock, causing the crew to make modifications detailed in deficiency #1. The non-expected defect/non-conformity was not reported in accordance with the vessel's ISM manual, section 10. Additionally, deficiency #2 has been addressed previously by the USCG. Provide plan to ensure vessel compliance with its ISM manual.

The condition of the ship and its equipment shall be maintained to conform to the provisions of the present regulations to ensure that the ship in all respects will fit to proceed to sea without danger to the ship or persons on board. The vessel incurred a controllable pitch propeller malfunction while departing its berthing in Fall River, MA. Effect sound repairs to the satisfaction of the U.S. Coast Guard, as well as the vessel's Classification Society, prior to movement.

The machinery, boilers, and other pressure vessels, shall be of a design and construction adequate for the service for which they are intended. Vessel experienced undetermined issue preventing complete operation of main engine while attempting to depart. 74SOLAS(04)II-1/26.1

The normal equipment of every rescue boat shall consist of... a first -aid kit(outfit) in a waterproof case capable of being closed after use... and a searchlight with a horizontal + vertical sector of 6° and measured luminous intensity of 2,500cd which can work continously for not less than 3 hours. Rescue boat is missing first aid kit & searchlight.

A alarm system complying with regulation 51 shall be provided for all important pressures, temperatures, and fluid levels and other essential parameters (periodicaly unattended machinery spaces). The main engine crankcase oil level sensor is inoperable.

Every company should develop, implement and maintain a Safety Management System with the following functional requirements: .2 Instructions and procedures to ensure the safe operation of ships and protection of the environment in compliance with relevant international and flag state legislation. Maintenance of machinery is not being implemented in accordance with the company's procedures. The main diesel engine failed while underway resulting in a marine casualty. The vessel was improperly loaded with Packaged Dangerous Goods and no Dangerous Cargo Manifest OR equivalent was available or created. Deficiencies 2 thru 5 are objective evidence of non-conformities relevant to the ISM Code elements 7 & 10, an external audit is required. 74 SOLAS (2020) IX/3.1, ISM 1.4.2 Code 30, AC

Carriage of Dangerous Goods shall be in compliance with the IMDG Code. Vehicles flammable liquid powered, not meeting the conditions of special provision 961 shall be assigned class 9 and shall meet the following requirements; for flammable liquid power vehicles the fuel tanks shall be not more than one fourth full. Installed batteries shall be protected from damage and short circuit and accidental activation during transport. A spot check of two vehicles on deck noted fuel tanks were at one half full and batteries were operational & not protected from short circuit/activation. All vehicles to be inspected for compliance with IMDG. Objective evidence ISM Code 7. 74 SOLAS (2020) VII/3, IMDG 3.1.

Machinery shall be of a design and construction adequate for the service for which they are intended. The vessel’s main diesel engine failed while transiting outbound Narragansett Bay, RI. Cylinder No. 7 overheated during the voyage. Follow-up inspection noted abnormal cooling water temperature for No. 3 cylinder and contaminated main diesel engine coolant water. Each ship carrying dangerous goods in packaged shall have a special list, manifest or stowage plan in accordance the IMDG Code. The vessel did not have a dangerous cargo manifest or stowage plan in accordance with the IMDG Code. Objective evidence ISM Code 7. 74 SOLAS (2020) VII/4.2 17ac

The company and ship shall comply with the requirements of the International Safety Management Code. The company should establish procedures to ensure that the ship is maintained in conformity with relevant rules, regulations, and additional requirements established by the company and ensure inspections are held at appropriate intervals. Vessel has multiple overdue maintenance items for the main diesel engine: Cylinder head checks No’s 1,2,5,6 are overdue by more than 6k hrs. Intake and Exhaust valve overhauls No’s 1, 5, 6 are overdue. Fuel injector testing on No’s 1,2,5,6 are overdue by appx 1.8k hrs. 74 SOLAS (2020) IX/3.1 ISM Code 10.2.1

Electrical installations shall ensure the safety of the crew and ship. There is a power strip installed below the crew made condensate drain of the A/C unit in the Engine Control Room. Cite: 74 SOLAS (04) II-1/ 40.1.3 Action: 17 ac

The ship shall be fitted with an approved oil filtering equipment. The oily water seperator was taken apart and could not be inspected properly. Cite: MARPOL 78 1/14.1 Action: 17 ac

Electrical installations shall ensure the safety of the crew and ship. There are dead end wires not properly secured behind the main electrical panel and the overhead of the steering gear room. Cite: 74 SOLAS (04) II-1/40.1.3 Action: 16 ac

All electrical apparatus shall be so installed as to not cause injury when handled or touched in the normal manner. The boiler burner electrical control panel cover was found broken and inadequately repaired with duct tape. 74 SOLAS (04 CONS) II-1/45.1.3 50ac

Manually operated call port in engine control station was damaged during engine room repairs and is not capable of immediate operation

Emergency generator did not start. Battery bank was discharged. Jumper wire was found installed on battery charging system.

Not less than 2 lifebuoys shall be provided with self activating smoke signals. Due to bad weather the vessel lost of of their smoke signals
