If one or more cylinders fail, a total of 50 per cent of the onboard cylinders should be tested. If further cylinders fail, all cylinders should be tested. The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. #2 ships service generator high jacketwater temp switch failed to secure the generator as per the ships automated test procedures.

Every crew member shall participate in at least one abandon ship drill and one fire drill every month. Vessel has minimum crew onboard and is currently not operating. Satisfactory drills to be conducted in the presence of a USCG Marine Inspector prior to operating.

Except as provided in paragraphs 3.4.4 and 3.4.5, each lifeboat shall be launched, and maneuvered in the water by its assigned operating crew, at least once every three months during an abandon ship drill. Vessel was only able to launch lifeboats twice in calendar year 2021. Due to port constraints vessel had not lowered lifeboats in the water since Aug 2021. Vessel must demonstrate to the USCG port lifeboat maneuvered in the water. Vessel to transit to Houston [name withheld] and lower at anchorage there.

The boundary bulkheads and decks separating the accommodations and control stations from cargo and machinery spaces, galleys, main pantries and storerooms, other than small service lockers, shall be of "A" Class construction.
Modifications made to the A-60 fire boundary door, between the engine room entrance and the crew changing area, have caused A-60 certification to be invalid. Additional fire boundary doors throughout the vessel shall be marked with proper classification labels and without modifications or alterations of fire rating.

It is the intent of this subpart to provide such markings as are necessary for the guidance of the person on board in case of an emergency.
Markings for the General Alarm are inadequate and shall be identified by red lettering at least 1/2 inch high.
Vessel owner must notify class society of deficiencies. Deficiencies are to be cleared to the satisfaction of ABS or Coast Guard.

During steering test from bridge, steering pedestal indicator did not match rudder angle indicator. Test conducted on system 1 both pumps with 5 degree difference between pedestal and rudder angle indicator. Rudder angle indicator did match fixed indicator at aft steering. Tests conducted in follow-up mode. Deficiency shall be rectified prior to departing Columbia River system to satisfaction of ABS and attending USCG inspector.

Chief Engineer is to monitor adequate dissel oil pressure during meneuvering.

Extensive diesel oil leakages noted in way of the #6 main engine fuel oil pump presenting a risk of fire hazard. Upon closer examination, additional leakages noted in way of the #2 and #5 fuel oil pumps. Main engine fuel pumps #2, #5, & #6 are to be overhauled and cleared of any leakages and #1, #3, & #4 to be examined internally to the satisfation of the attending ACP ACS (ABS) surveyor.
