A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. STBD fire-boy system did not shut down main propulsion engine and powered ventilation. 46 CFR 181.420(a)(3)

At each initial and subsequent inspection for certification the owner shall be prepared to conduct test and have the vessel ready for inspection of its fire protection equipment, including the testing of ventilation shut downs. Port engine room ventilation closure obstructed by hose. 46 CFR 181.420

In systems where flixible hoses are permitted to be clamped, double hose clamping is required. Double hose clamp raw water hoses in Engine rooms. 46 CFR 181.410(d)(1)

A vessel must carry a first-aid kit approved under ISO 188113:2006 paragraph 4.12.

Where flexibility is necessary, a section of flexible metallic hose may be used. Nonmetallic hose may be used for wet exhaust systems provided it is especially adapted to resist the action of oil, acid, and heat, has a wall thickness sufficient to prevent collapsing or panting, and is double clamped where practicable. Stbd engine room flexible hose for wet exhaust water cooling system is missing double clamps on one end of the hose. 46CFR182.430(e)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Raw water hose for stbd engine is damaged (torn) at hose clamp section, reducing effectiveness of double clamps.46CFR182.200(a)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: The pump is listed by an independent laboratory as meeting the requirements in UL 1113 (incorporated by reference; see 46 CFR 175.600). Submersible bilge pumps in all compartments do not meet standard. Vessel maximum passenger capacity reduced to 12 passengers until owner/operator provides documentation attesting to submersible bilge pumps in port and stbd engine room being UL listed. All other submersible pumps on-board to meet standard within 30 days. 46CFR182.520(e)

Pre-employment testing requirements. (a) No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Crew did not complete pre-employment testing. 46CFR16.210(a)

Except on a vessel limited to daytime operations, the compass must be illuminated. Compass is not illuminated. 46CFR184.402(c)

The servicing facility must issue a certificate to the life raft owner or owner's agent for each liferaft it services. Servicing facility did not issue a life raft servicing certificate. 46CFR160.151-57(p)

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Starboard, forward bilge pump indicator light was inoperable when tested. Restore functionality.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station IAW Part III of Title III of the Communications Act must carry a radio operator...Master (Ketch) did not have a marine radio operator permit or other appropriate license. Provide proof of license. 47 CFR 80.159(e)

Abandon ship, man overboard and fire drils and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include: the date of the drill/training and a general description of the drill scenario and training topics. Vessel log did not include drill entries. Provide copy of May and June training and drill records.
