The material must be in strips at least 50 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. Retroreflective tape for life floats was found deteriorating, and need replacing. 46 CFR 160.010-4(p)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel’s grab line on aft deck life ring was found deteriorated and is a potential sharp hazard. Replace life ring grab line. 46 CFR 176.808(b)

Twist-on type wire nuts is permitted if:The connections must be made within an enclosure & the insulated cap of the connector must be secured to prevent loosening due to vibration; & Twist-on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Dead end wires for removed fan on overhead in galley were found exposed and not in an enclosure. Provide enclosure to satisfaction of MI. 46 CFR 183.340(i)

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment, except that the space forward of the collision bulkhead need not be fitted with a bilge suction line when the arrangement of the vessel is such that ordinary leakage may be removed from this compartment by the use of a hand portable bilge pump or other equipment, and such equipment is provided. Vessel has no means of dewatering void space forward of the collision bulkhead. Provide hand portable bilge pump or other means acceptable to Marine Inspector. 46 CFR 182.510(a)

Electrical equipment on a vessel must be installed and maintained to:(c) Minimize accidental personnel contact with energized parts. Breaker box had an opening/missing breaker creating a potential shock hazard. Remove hazard to satisfaction of MI. 46 CFR 183.200

Internal structural framing is wasted in several areas where the framing connects to deck and aft bulkhead located at the starboard-aft corner of rudder room near access ladder. There is also bent framing in the corner where the starboard side meets the stern. Crop and renew to satisfaction of Marine Inspector prior to next credit drydock. 46 CFR 177.100

An internal structural examination...may be conducted while the vessel is afloat or out of the water and consists of a complete examination of the vessel's main...hull plating. MI Identified wastage resulting in a hole through the overhead on portside of void #2 next to a deck drain piping coming down from weather deck. Hole leads up above into the bottom of a vertical deck rail post next to deck drain. Monitor and further investigate. Notify MI if any repair is intended to be made. Follow-up with MI within 90 days. 46 CFR 176.610(b)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel's current stability letter is invalid. Stability is under review by the Marine Safety Center (MSC) following replacement of original aluminum tanks with plastic tanks. 46 CFR 185.100

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Bilge high level alarm in bow thruster compartment did not create visual or audible alarm. 46 CFR 182.530(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure, including the inspection of all accessible parts of the exterior and interior of the hull. Identified significant corrosion in lazarette and on starboard stern tube. Gauge suspect areas and provide a report to the attending MI. 46 CFR 176.802(a)(1)

A space containing diesel machinery must be fitted with at least two ducts to furnish natural or powered supply and exhaust ventilation. The engine room only had one ventilation duct remaining in service. 46 CFR 182.465(c)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The closure device for the ventilation duct was seized. 46 CFR 182.465(h)

The master shall conduct sufficient abandon ship, man overboard, and fire drills and give sufficient instructions to make sure that all crew members are familiar with their duties during such emergencies. No record of drills completed onboard since 2022. 46 CFR 185.520(a) 46 CFR 185.524(a)

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. The waterlight onboard the vessel was not the approved version for ring life buoys. 46 CFR 180.70(d)(1)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The retroreflective material on the orange ring life buoy (2nd deck,above stairs) was deteriorated. 46 CFR 185.604(i)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. The life jackets stowed on the main deck do not have markings identifying the number, sizes, and location. The 2nd deck stowage container markings indicate the incorrect number of life jackets stowed inside and are blocked by AV equipment. 46 CFR 185.604(f)

Identified power strips plugged into one another for speaker equipment on 2nd deck. 46 CFR 183.200(b)

Identified exposed insulation nails in engine room that are a hazard to crew. 46 CFR 177.100

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. Charts and Navigation Rules book were not up to date. No Coast Pilot [name withheld], Tide tables, or Current tables onboard. 46 CFR 184.420(a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. The first-aid kit did not have all required items and the majority of contents were expired. 46 CFR 184.710

The rules concerning shapes shall be complied with by day. No dayshapes onboard vessel. 33 CFR 83.20(d)

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test and make available for inspection all items in the ship's outfit, such as navigation lights and equipment which are required to be carried by the regulations in this subchapter, as necessary to determine that they are fit for the service intended. The anchor light was found inoperable and would trip circuit breaker upon activation. 46 CFR 176.816

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the inspection of each portable fire extinguisher. The bracket on the portable extinguisher in the pilot house did not fit around the extinguisher. 46 CFR 176.810 (a)

Each vessel must have permanent draft marks at each end of the vessel or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. Vessel’s draft marks were painted over and are not visible. Fix prior to next credit drydock. 46 CFR 185.602(c)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include replacement, other than replacement in kind, of tanks and alterations affecting stability. Vessel replaced original aluminum water tank with 3 plastic tanks without OCMI approval. Provide specifications of new tanks and details of installation to the attending MI. 46 CFR 176.700(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure, including the inspection of all accessible parts of the exterior and interior of the hull. Identified significant wastage on port side of transom and stern tube. Submit repair proposal to satisfaction of MI prior to commencement of work. 46 CFR 176.802(a)(1)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Damage survey following allision with day marker must be completed prior to carriage of passengers.

On a vessel of more than 65 ft in length, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi the pump outlet. Fire pump produces a max output of 45 psi. Demonstrate operation of fire pump that meets psi requirements.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull. Noted pinhole in recent center lazarette insert located aftermost starboard corner of insert. Temporary repair authorized with permanent repair required within two weeks. Hourly rounds of lazarette required by crew while underway with passengers.

The name of a vessel must be marked or painted in clearly legible letters and numbers: On each lifefloat. The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float. Each rescue boat must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Upper port life float had faded vessel name and capacity and wasted retro.

The following spaces must be equipped with a fixed gas fire extinguishing system in compliance with 181.410, or other fixed fire extinguishing system specifically approved by the commandant: A space containing propulsion machinery. Heat rise detector has 3 inch inset and CO2 line connecting heat-rise detector and release mechanism is damaged by chaffing due to metal on metal contact with support / outer pipe. Operator shall provide written report to the extent of damage, condition of the pipe and serviceability of the system. 46 CFR 181.400(a)(1)

Excessive amounts of combustible materials such as spare fuel oil, used oil, spray paint cans, and household cleaners were observed throughout the vessel not properly stored in a paint or flammable liquid locker as required by 46 CFR 177.405(d). Make repairs to satisfaction of MI. 46 CFR 185.100

On a vessel of more than 19.8 meters (65 feet) in length, a power driven fire pump required by §181.300 of this subchapter may serve as one of the two fixed power bilge pumps required by this subpart, provided Stop valves and check valves are installed in the piping to isolate the systems during simultaneous operation and prevent possible flooding through the bilge system. There are no check valves or stop valves installed in the piping that utilizes the fire pump as a bilge pump. Make repairs to the satisfaction of the attending Marine Inspector prior to credit drydock. 46 CFR 182.520(i)(3)

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. The gray water system has been altered to flow directly overboard instead of into the gray water tank as in the original plan. Return the piping to the original condition to the satisfaction of the attending Marine Inspector prior to credit drydock. 46 CFR 176.700(b)

Where rigid nonmetallic material (plastic) is permitted for use in piping systems by this section, the following restrictions apply: through hull fittings and shutoff valves must be metallic. In the case of nonmetallic hulls, materials that will afford an equal degree of safety and heat resistivity as that afforded by the hull may be approved. The aft and forward cargo holds have multiple PVC pipe and rubber hose bulkhead and deck penetrations which do not have a metallic through-deck or through-hull fittings. Make repairs to the satisfaction of the attending Marine Inspector prior to credit drydock. 46 CFR 182.720(d)(3)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. The flame arrestors on the fuel tank vent are corroded have holes in it. Make repairs to the satisfaction of the attending Marine Inspector prior to credit drydock. 46 CFR 182.450(e)

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Aft steering space and forward cargo space have multiple cable transits that are not watertight. Make repairs to the satisfaction of the attending Marine Inspector. 46 CFR 179.320(c)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Both of the main engines and the forward generator do not have belt guards. Install guards to the satisfaction of the attending Marine Inspector. 46 CFR 177.960

Electrical equipment on a vessel must be installed and maintained to protect passengers and crew from electrical hazards originating in electrical equipment. Forward hold high water alarm is improperly mounted and secured by carpenter clamps. Properly secure electrical bilge alarm. 46 CFR 182.530(a)

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Aft steering access cover does not have a means to prevent its loss. Secure hatch to the satisfaction of the attending MI. 46 CFR 179.360 (b)

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Main Machinery Room small generator starting battery is not properly mounted. Make repairs to the satisfaction of the attending Marine Inspector. 46 CFR 183.350(b)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. The machinery space and cargo space have multiple wires that are improperly supported. Make repairs to the satisfaction of the attending Marine Inspector. 46 CFR 183.340(b)(4)

The space in front of the electrical panels on the upper deck is being utilized as storage. Make repairs to the satisfaction of the attending Marine Inspector. 46 CFR 183.330(e)

Sea rails, with suitable barriers to prevent accidental movement of cooking pots, must be installed on a cooking range. Galley range does not have suitable sea rails installed. Make repairs to the satisfaction of the attending Marine Inspector. 46 CFR 184.220(e)

The AC cooling pump assembly is not properly supported or secured. Pump was plugged into vessel outlet. Pump must be hardwired into vessels electrical system and securely mounted. Submit repair proposal to the satisfaction of the attending Marine Inspector prior to credit drydock. 46 CFR 177.100

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. There are numerous Carbon Dioxide cannisters unsecured in the forward cargo hold. Make secure to the satisfaction of the attending Marine Inspector. 46 CFR 147.60(b)(1)

A vessel is inspected for compliance with the standards of the subchapter. Machinery, equipment and materials not covered in this subchapter maybe inspected in accordance with standards acceptable to the cognizant OCMI as a good marine practice. Vessels expansion tank in the main engine space on the starboard side is improperly mounted to plywood with the use of plastic zip ties. Properly mount and secure expansion tank. 46 CFR 175.800

When required by the marine inspector, a portion of the inspection must be conducted while the vessel is underway so that the hull and internal structure can be observed. Underway checks were not completed, MOB and fire drills to be completed prior to COI annual endorsement. 46 CFR 176.802(c)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel has not had drydock examination and has lapsed. Resolve prior to carriage of passengers. 46 CFR 176.600(c)(1)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. Vessel's control panel failed to produce a sound for any space's high level alarm actuation.

Piping used in a vital system must be composed of ferrous materials. Bilge suction strainer in tank space is made of plastic.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguisher near stairwell must be secured/mounted properly to bulkhead.

The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light. The lanyard has deteriorated and is in need of immediate replacement on 2 of 4 life floats.

A vessel on a lakes, bays, and sounds, or rivers route must carry: Three hand red flare distress signals & Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Flares on board vessel are expired and require replacement.

Each vessel must be equipped with a public address system. On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. PA system on board inoperable.

Except as provided in paragraph (c) of this section, each vessel must comply with the Type II subdivision and damage stability requirements of §§ 171.070 through 171.073 and 171.080 of this chapter if it meets one or more of the following criteria: Is more than 19.8 meters (65 feet) in length; Carries more than 49 passengers. Vessel has openings between engine room and main deck at the site of main switchboard panels A & B.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). The vessel did not have the corresponding MMSI number programmed within the radio to ensure proper DSC transmission. 47 CFR 80.905(a)(1)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of piping, controls, and valves, and the inspection and testing of alarms and ventilation shutdowns, for each fixed gas fire extinguishing system and detection system to determine that the system is in operating condition. Annual service report for the required fixed CO2 engine room fire suppression system's heat activated detectors are inoperable.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel's port engine has significant damage due to overheating and is not capable of operation.

Owner must conduct inspections of portable and fixed fire extinguishing systems in accordance with NFPA standards. No proof of equipment servicing for extinguishers or fixed cylinders provide technician report of servicing. (July 12, 2020)

Each life float must be fitted with a light. Missing 01 light on life float.

Each vessel must have one ring buoy fitted with a floating waterlight. 01 buoy has inoperable light.

A passenger accommodation space must be maintained to minimize safety hazards. Ceiling hanging down with sharp metal edges on second deck dance floor.

A vessel must comply with certificates to be issued by the FCC. Bridge-to-bridge and radiotelephony are expired.

A vessel must carry up-to-date U.S. Coast Pilot, Coast Guard light list, tide tables, and current tables. No publications of the above on board.

Each survival craft must be secured to the vessel by a painter. 04 life floats had no painter attached.

Electrical equipment used in engine rooms must be drip proof. The 24V DC switch box housing for PTO fire/bilge pump (magnetic pickup) is cracked on two sides and needs replacement.

A dual voltage generator shall be of the grounded type, where the neutral bus shall be connected to the ground. For an alternating current system, ground detection must be provided that meets §111.05-27 in subchapter J of this chapter, which states that grounded neutral and high-impedance grounded neutral alternating current systems must have a suitably sensitive ground detection system which indicates current in the ground connection, is able to withstand the maximum available fault current without damage, and provides continuous indication of circuit status to ground. A provision must be included to compare indications under fault conditions with those under normal conditions. According to original electrical plan, vessel's distribution is ungrounded and vessel's dual voltage generators are not equipped with a ground detection system. Deficiency extended to 07APR2020 due to CG delay in plan review.

For a fixed gas fire extinguishing system, a distribution line must extend at least 51 millimeters (2 inches) beyond the last orifice and be closed with a cap or plug. In the engine room, the CO2 piping does not extend 2 inches past the starboard side nozzle.

Connections must be made to battery terminals with permanent type connectors. All batteries in the engine room are installed with wing nuts.

When life floats are stowed in tiers, separate units must be kept apart by spacers. Life floats were stacked, but not separated by spacers.
