PSCOs observed presence of oily mixture under main engine and oily rags underneath no.2 generator.

PSCOs discovered combustible materials in the engine room such as oily rags and oil stained carboard.Multiple water leaks found on main engine and bucket with lube oil under shaft seal.

07109-Machinery, Boilers and other pressure vessels, associated piping and fitting shall be of a design and construction adequate for the service intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. PSCO discovered the vessel's CO2 quick release valves did not have safety pins installed in the manual release handle. These CO2 quick release valves where of a type that normally have pins installed. A review of the vessel's manual for the system confirmed the pins were to be installed. Vessel's crew re-installed the safety pins. 74 SOLAS (04) II-1/26.1 10c
