PSCO observed a significant amount of foul-smelling water pooled at the entrance to the CO2 room, which could not be explained by the ship's crew but was identified as grey water by the Chief Engineer.

THE VESSEL'S ENGINEERING CREW WAS UNABLE TO PROVIDE ANY DOCUMENTATION AS REQUIRED IN THE VESSEL'S SMS FOR ANY DEFECTIVE ITEMS. DEFICIENCIES #3 - #9-PROVIDE EVIDENCE OF SERIOUS LACK OF EFFECTIVENESS OF PLANNED MAINTENANCE SYSTEM AND REQUIRED ROUTING INSPECTION/ROUNDS BY THE SHIP'S CREW. DUE TO THE OBJECTIVE EVIDENCE INT HE ABOVE DEFICIENCIES, THE VESSEL IS NOT IN SUSBTANTIAL COMPLIANCE WITH RELEVANT CONVENTIONS.
