The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by 46 CFR 185.510 of this part. Training conducted on a sister vessel may be considered equivalent to the initial, monthly, and quarterly training requirements contained in paragraphs (a) and (b) of this section. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Company's drill logs showed that majority of the crew members were not meeting the quarterly drill requirements, and vessel was unable to demonstrate adequate fire drill during the 2025 Annual Inspection. Ensure all crew members complete required drills quarterly and schedule a training program audit with the local USCG inspections office. Crew members who do not meet the quarterly drill requirements may not operate the vessel until they meet the applicable requirements. This deficiency is to be rectified by 23 September 2025. On 08June2023. Vessel lost steering / maneuverability near the Shelter Island Pier with passengers on board. Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel must trouble shoot the cause of the loss of propulsion, submit a technicians report attesting to the cause as well as satisfactory operation of the propulsion system and make the vessel available for inspection to the local OCMI.

Resolution: Company reported there was a broken carrier bearing on the PTO driveshaft that powers the outdrive. Their maintenance team has changed the bearing out and tested the system. The system is working properly with no vibration.
