Observed water tight door seal failure at the following: Port side bottom forward door and STBD bottom half aft of the FWD door (loading area). Repairs shall be completed and witness of water tests shall be done in presence of a Coast Guard Marine Inspector.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by 46 CFR 185.510 of this part. Training conducted on a sister vessel may be considered equivalent to the initial, monthly, and quarterly training requirements contained in paragraphs (a) and (b) of this section. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Company's drill logs showed that majority of the crew members were not meeting the quarterly drill requirements, and vessel was unable to demonstrate adequate fire drill during the 2025 Annual Inspection. Ensure all crew members complete required drills quarterly and schedule a training program audit with the local USCG inspections office. Crew members who do not meet the quarterly drill requirements may not operate the vessel until they meet the applicable requirements. This deficiency is to be rectified by 23 September 2025. Vessel's steering gear pins are installed upside down. Reinstall the steering gear pins so that the cotter pins are on the bottom. To be rectified by 23 September 2025. Cite: 46 CFR 182.100.

Electrical equipment on a vessel must be installed to protect the vessel from electrical hazards, including fire. Observed two circuit breakers wired up in series for the steering system. Vessel corrected the deficiency on the spot. Cite: 46 CFR 183.200(b).

Vessel's Annual Inspection was due by 21 August 2025. Complete Annual Inspection prior to operating with passengers. Cite: 46 CFR 176.500.

A vessel must be provided with steering gear that is of adequate strength and capable of steering the vessel at all services speeds. Vessel reported a loss of steering. Provide technician report to local OCMI. Prior to carrying passengers a sea trail must be conducted in the presence of a Marin Inspector.

Vessel experienced a steering failure while underway with passengers. Upgrade steering system to MSC approved electric-hydraulic system, prior to carrying passengers. Conduct seat trial with attending marine inspector.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation. Vessel experienced loss of main propulsion.

A universal joint between the transmission and outdrive PTO unit failed, causing a loss of propulsion and damage to surrounding equipment. Identify all damaged equipment and provide a repair plan for OCMI approval. Upon receiving OCMI approval, complete repairs and conduct an operational test of all repaired equipment in the presence of a USCG marine inspector.

On 08June2023. Vessel lost steering / maneuverability near the Shelter Island Pier with passengers on board. Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel must trouble shoot the cause of the loss of propulsion, submit a technicians report attesting to the cause as well as satisfactory operation of the propulsion system and make the vessel available for inspection to the local OCMI.

Identify and repair the cause of the vibration in the propulsion system.

Diagnose and repair the source of engine overheating.

Make effective repairs on shaft yoke due to broken bolt. Prove proper operation while underway to the attending Marine Inspector.

Linkage had two scres come loose causing link to seperate and lose steering.

Demonstrate proper operation of the steering gear.

Demonstrate proper operation from the report of the loud metallic know from the vessels engine and/ drive train
