PSCO observed a 440V extension cable strung out from a connection amidships to th forward house used as a permanent fixture for welding supported by rope in various places to prevent sagging but altogether unprotected and exposed to the elements and mechanical wear. Additionally , chafing was noted at plug/cable connection ponts, making visible the individual conductors and metal sheathing through the external insulation.

Connections made in on-deck 440V feeder cable to extension cable strung along deck were visibly damaged in both the feeder connector and the final plug which the extension cable serviced. Previous attempts to wrap cable connector in electrical tape had been worn through and torn off from mechanical wear, exposing sheathing and internal conductors.
