Class 2 - hand operated sliding doors when operating hand gear the time necessary for the complete closure of the door with the vessel upright, shall not exceed 90 seconds. Watertight doors located in the below cargo deck do not close in the required time - 60 SOLAS II/13(h) - 50bc

The Oil Record Book shall be in the form specified in Appendix III to this Annex. Multiple list of items to be recorded in the Oil Record Book Part 1 are inaccurately written according to this prescribed Annex. MARPOL 2017 1/17.1 - 50c

The capacity in which the holder of certificate is authorized to serve shall be identified in the form of endorsement in term identical to those used in the applicable safe manning requirements of the administration. 02 able seafarer and 01 able seafarer engineer do not possess flag endorsements as required by minimum safe manning certificate - 95 STCW(11 cons) Reg I/2.9 - 15bc

Objective evidence discovered during an expanded ISM exam revealed the following non-conformities: the crew failed to fully implement the requirements of the ISM Code through their SMS procedures. These identified deficiencies are evidence that the ship and/or company are not meeting the SMS requirements. Recommend an external audit. See Form B.

The Administration shall satisfy itself that the general structural strength of the hull is sufficient for the draught corresponding to the freeboard assigned. The material strength of the main deck, which carries thousands of pounds of cargo including dangerous goods in greater than limited or excepted quantities, is severely wasted and compromised; there are currently eight confirmed wastage holes allowing water to enter directly into the main machinery space and under-deck cargo space. The vessel has no proof of recent deck gauging, including during the 2016 dry-dock, and the SOLAS Cargo Ship Safety Construction Certificate lists the previous dry-dock date as "UNKNOWN."

The emergency fire pump is leaking at a rate of approximately five gallons per minute, so excessively that a bucket was placed under it to mitigate water level.

Notwithstanding paragraph 4.2, for all ships, not later than the first scheduled dry-docking after 1 July 2014, but not later than 1 July 2019, lifeboat on-load release mechanisms not complying with paragraphs 4.4.7.6.5 to 4.4.7.6.6 of the Code shall be replaced with equipment that complies with the Code. The last dry-dock was 22 November 2016, and the lifeboat hooks are not the on-load release type complying with paragraphs 4.4.7.6.5 to 4.4.7.6.6.

For ships constructed before 1 July 1998, the Administration shall: ensure that when life-saving appliances or arrangements on such ships are replaced such life-saving appliances or arrangements, in so far as is reasonable and practicable, comply with the requirements of this chapter. Launching arrangements shall comply with the requirements of regulation 16. Unless expressly provided otherwise, launching and embarkation appliances complying with the requirements of section 6.1 of the Code shall be provided. The davit is not certified for the rescue boat which was recently installed to replace the previous one. The Flag Administration issued a "Letter of Comfort" indicating the davit should be replaced by 30 April 2018; however, the davit has not been replaced.

At least eight lifebuoys of a type which satisfies the requirements of Regulation 21 of this Chapter shall be provided. Every lifebuoy shall: if it is intended to operate the quick-release arrangement provided for the self-activated smoke signals and self-igniting lights, have a mass of not less than 4 kg. The quick-release ring lifebuoys were not the 4 kg type.

The port side generator had significant oil leaks creating a fire hazard. The jacket water pump serving the same generator had soft patches, was highly corroded/wasted, and was leaking at approximately four drops per second.

Satisfactory means (in the form of guard rails, lifelines, gangways or underdeck passages, etc.) shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work of the ship. There are platforms above the bulwarks running nearly the length of the ship on either side of and ten feet above the cargo deck. The crew's normal practice is to use these platforms when loaded with cargo to maximize usable deck space. In the event of a deck fire, these platforms would be used for fire-fighting purposes. Externally, efficient guard rails are provided in accordance with the 66 Load Lines Convention. Internally, stanchions are fitted with nylon lines for all three rungs (top "rail" and bottom two courses) running the entire length of the platforms, which is not in accordance with the unified interpretations of Load Lines. In addition, there are multiple guard rails in the engine room which are fractured.

Multiple light fixtures throughout the engine room are missing covers, leaving the fluorescent bulbs exposed and creating an ignition hazard.

A hazmat employee must receive the training required by this subpart at least once every three years. The master and deck officer could not provide proof of HAZMAT training, and the vessel regularly carries dangerous cargo.

Bulkheads at exposed ends of enclosed superstructures shall be of efficient construction and shall be to the satisfaction of the Administration. A hole was drilled through the control station (navigation bridge) bulkhead to run wires for navigation equipment and was then sealed with silicone.

The normal equipment of every lifeboat shall consist of: a food ration as described in paragraph 4.1.5.1.18 totalling not less than 10,000 kJ for each person the lifeboat is permitted to accommodate. The starboard side lifeboat was missing one food ration.

Every ship to which Chapter 1 applies shall carry an appropriate portable atmosphere-testing instrument. As a minimum, these shall be capable of measuring concentrations of oxygen, flammable gases of vapors, hydrogen sulfide and carbon monoxide prior to entry into enclosed spaces. Suitable means shall be provided for the calibration of all such instruments. The vessel does not have a means to calibrate the portable atmosphere testing instrument onboard.

The International Load Line Certificate (1966) shall not be delivered to the ship until the officer or surveyor acting under the provisions of article 13 of the present Convention has certified that the marks are correctly and permanently indicated on the ship’s sides. The Load Line markings are in Spanish whereas the certificate’s Load Line markings are in English.

Each vessel must have the following: a gyrocompass. The vessel has a magnetic and satellite compass but does not have a gyrocompass required by 33 CFR for vessels over 1,600 gross tons.

74 SOLAS (2004) III/REG 19.2.1 PSCO's observed that the fire team members were unfamiliar with the ships procedures, techniques and tactics for combating fire. In particular the fire team members were unable to properly don the ships fire outfits and SCBA's. They were also unable to properly communicate due to a language barrier between varying crew members.
