MARPOL ANNEX I/14.6 Oil filtering equipment shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. The oily water separator sample line leading into the oil content meter was obstructed preventing accurate sampling of oily water.

The Administration shall satisfy itself that the general structural strength of the hull is sufficient for the draught corresponding to the freeboard assigned. The material strength of the main deck, which carries thousands of pounds of cargo including dangerous goods in greater than limited or excepted quantities, is severely wasted and compromised; there are currently eight confirmed wastage holes allowing water to enter directly into the main machinery space and under-deck cargo space. The vessel has no proof of recent deck gauging, including during the 2016 dry-dock, and the SOLAS Cargo Ship Safety Construction Certificate lists the previous dry-dock date as "UNKNOWN."

For ships constructed before 1 July 1998, the Administration shall: ensure that when life-saving appliances or arrangements on such ships are replaced such life-saving appliances or arrangements, in so far as is reasonable and practicable, comply with the requirements of this chapter. Launching arrangements shall comply with the requirements of regulation 16. Unless expressly provided otherwise, launching and embarkation appliances complying with the requirements of section 6.1 of the Code shall be provided. The davit is not certified for the rescue boat which was recently installed to replace the previous one. The Flag Administration issued a "Letter of Comfort" indicating the davit should be replaced by 30 April 2018; however, the davit has not been replaced.

Satisfactory means (in the form of guard rails, lifelines, gangways or underdeck passages, etc.) shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work of the ship. There are platforms above the bulwarks running nearly the length of the ship on either side of and ten feet above the cargo deck. The crew's normal practice is to use these platforms when loaded with cargo to maximize usable deck space. In the event of a deck fire, these platforms would be used for fire-fighting purposes. Externally, efficient guard rails are provided in accordance with the 66 Load Lines Convention. Internally, stanchions are fitted with nylon lines for all three rungs (top "rail" and bottom two courses) running the entire length of the platforms, which is not in accordance with the unified interpretations of Load Lines. In addition, there are multiple guard rails in the engine room which are fractured.

Every ship to which Chapter 1 applies shall carry an appropriate portable atmosphere-testing instrument. As a minimum, these shall be capable of measuring concentrations of oxygen, flammable gases of vapors, hydrogen sulfide and carbon monoxide prior to entry into enclosed spaces. Suitable means shall be provided for the calibration of all such instruments. The vessel does not have a means to calibrate the portable atmosphere testing instrument onboard.
