15108 - The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. PSCO’s observed unapproved modifications to watertight door alarm sensors and the pumps and piping arrangements on the heavy fuel oil purifier system. The non-conformities and modifications were not reported to either class or Flag. Due to the objective evidence in the below deficiencies, the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. An external audit conducted by the Flag or RO is required within 30 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port. 74 SOLAS (20) IX/3.1 ISM Code 10.1 30 B / C

02101 - All watertight doors shall be kept closed during navigation except when necessarily opened for the working of the ship, and shall always be ready to be immediately closed. During the cargo tunnel examination, the PSCO observed 9 out the 9 water tight doors did not close properly. 4 watertight doors on the port side failed to close properly, 4 watertight doors on the starboard side failed to close. The watertight door leading from the engine room to the tunnel also failed to close properly. The failures were caused by leaking hydraulic lines, frozen wheels, and modification to the sensor detection closed position. 74 SOLAS II-I/13(n) 30 A / C

03103 - The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such systems. PSCO observed gangway rigged at an angle that does not comply with shipboard SMS procedures and the gangway technical requirements. Also, the vessel's SMS states the gangway should be suitably rigged to compensate for the movement of the ship, however it was rigged in such a way preventing the rollers at the bottom from coming into contact with the pier rendering it ineffective and creating high stress areas on the gangway. 74 SOLAS (14)IX/3.1 ISM Code 10.3 40ac

03103 - Satisfactory means of safe passage in the form of guardrails, gangways, etc. shall be provided for the protection of the crew in getting to any space used in the essential operation of the ship. The exit route toward the starboard side lifeboat had an open area near the lifeboat that had no courses preventing a person from falling to the deck below. ILLC 25(4)

09201 - Lockers or housing must be vented to the open air near the top and bottom for circulation of vapors. PSCO observed secured ventilation at acetylene storage locations. 46 CFR 147.60 10c

01315 - The Oil Record Book Part 1 shall be completed on each occasion whenever discharge overboard or disposal otherwise of bilge water which has accumulated in machinery spaces. PSCO observed two receipts of bilge water transfer to reception facility, both of which, were note entered into the Oil Record Book Part 1. MARPOL (2017) Annex I/17.2.4 10c

07126-The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed fuel leaks in #1 and #2 MDG's. MDG #1 cylinder #3 was leaking at the pump and fuel lines and #2 MDG Cylinder #3 and #5 fuel pumps were leaking. 74 Solas 14 I/II (a)

07122- Plans and booklets shall be kept up to date; any alterations there to shall be recorded. Fire control plan onboard the vessel conflicted with the ships structure. In addition the symbols do not match the provided key in the firecontrol plan. 74 SOLAS II-2/4

74 SOLAS (14) I/11(a): The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed fuel oil covering the aft starboard side of the main engine; the aft portion of the main engine including 3 pipelines into the engine; dripping from the second deck of the main engine; leaking from gaskets on the main engine; pooled in an unapproved container below a gasket; and pooled under fuel oil purifier.

74 SOLAS (14)II-2/14.2.2.1: Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. Multiple fire hose nozzles were packed with granular material and connection points were severely corroded.

74 SOLAS (14) IX/3.1 & ISM Code A 10.1: The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. PSCO’s requested maintenance checklist for the engine room and firefighting equipment. Ship’s crew provided completed “Algoma central corporation pre-inspection checklist” where the engine department had checked “yes” for engine room machinery clean and free of oil. They had also marked “yes” for steering gear save-all clean and free of oil, overall cleanliness of engine room acceptable, and no leaks in the purifier room. Completed “Algoma Ship Tech LTD Firehoses/Hydrants/Nozzles and attachments” maintenance logs were also notated as “Checked All SAT” weekly, over the last two months. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel’s SMS under the ISM Code. An external audit conducted by the flag or RO is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to the USCG prior to departure from port.

74 SOLAS (14) II-2/14.2.1.1.2: The fire detection and fire alarm systems shall be kept in good order so as to ensure their required performance if a fire occurs. The smoke detector in the “Take Up Hydraulic Station” was damaged.

74 SOLAS (14) II-2/15.2.4.1: Plans and booklets shall be kept up to date; any alterations there to shall be recorded. Fire Control Plans (FCP) on board the vessel conflicted with the ship’s structure. Additionally, the fire control plans on board did not match each other, observed discrepancies within galley pantry between the corridor FCP and the FCP provided by the Master.

74 SOLAS (UnAmend) II-2/23(f): Fire doors in main vertical zone bulkheads and stairway enclosures shall be of the self-closing type. Emergency escape fire door in bottom deck engine room failed to latch properly when closed.

74 SOLAS (UnAmend) II-1/27(a)(v): Lighting fittings shall be arranged to prevent temperature rises to the wiring and prevent surrounding material from becoming excessively hot. Numerous light covers in the engine room had cracks and holes in them.

ICLL (66) Article 15: After any survey of the ship under article 14 has been completed, no change shall be made in the structure, equipment, arrangements, material or scantlings covered by the survey, without the sanction of the administration. PSCO observed sunlight penetrating the forecastle door seal rendering the watertight integrity compromised.

ICLL (66) 12(1): All access openings in bulkheads at ends of enclosed superstructures shall be fitted so that the structure is weathertight when closed. The means for securing these doors shall be so arranged that they can be operated from both sides of the bulkhead. The weathertight door from the galley to the weather deck was missing the handle creating an opening in the door, making the condition non-weathertight.

74 SOLAS (14) III/34 & LSA Code 1.2.2.7: Life-saving appliances shall be fitted with retro-reflective material where it will assist with detection. The retro-reflective tape on the port side lifeboat is deteriorated.

74 SOLAS (UnAmend) II-1/27(a)(ii): The fronts of the switchboards shall be suitably guarded. There was an opening in the front of the switchboard in the engine control room where a breaker was removed.

74 SOLAS (14) I/11(a): The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed hydraulic oil actively leaking from pumps on hatch covers: 3 port, 4 port, 5 starboard, 6 port, 6 starboard, and 7 port.

PSCO observed a hose used for draining hydraulic oil from the hydraulic oil tank that was leaking on the steering gear pump, then into the small discharge containment.

PSCO observed condensation leaking from the pipelines to the refrigerated stores. The condensation was pooled on the deck and unable to drain due to a plugged drain.

49 CFR 176.200(c): When cylinders of Class 2 (compressed gas) materials being transported by vessel, the cylinders must be dunnaged 10 cm off any metal deck. PSCO observed oxygen and acetylene tanks stored directly on metal deck of the vessel.

33 CFR 151.2050(f): The master, owner, operator, agent or person in charge of any vessel equipped with ballast water tanks that operates in waters of the United States must remove fouling organisms from the vessel’s hull. PSCO observed an excessive amount of algae growing on the vessel’s hull.

74 SOLAS (04) III/13.1.3: Each survival craft shall be stowed in a state of continuous readiness so that two crew members can carry out preparations for embarkation and launching in less than 5 minutes. During operational test, it took crew members approximately 13 minutes to start the rescue boat engine. Suitable arrangements shall be mad

74 SOLAS (UnAmend) III/19: Suitable arrangements shall be made for embarkation into the lifeboats. The shackles to the guard rail chain at the port side life boat embarkation area were seized, causing an obstruction to embarking the port side lifeboat.

MARPOL I/15.1 Discharge into the sea of oil from ships shall be prohibited. While conducting bunker operations, vessel's sounding tube on Portside LSFO tank was giving false readings due to Heavy Fuel Oil build up in sounding tube. These readings were well below the actual levels in the tank, thus causing fuel oil to overfill the tank, discharge from the air vent, fill up the small containment and discharge approximately 15-20 liters into a U.S. navigable waterway.

1199 Vessel was not following their own management plan. The ships management plan did not allow for flow-through exchange method, which their records indicated was a primary exchange method.

The steering gear room has an excessive amount of water within the containment. Rags were stuffed in areas closest to the rudder stock. Accumulation of water is estimated at two to three gallons. Class to determine the source of water and attest to proper repairs prior to departure.

The S-VDR Control Panel on the bridge is reporting a "Capsule Failure" alarm. Class to attest to the proper repair and operation of the S-VDR unit prior to departure. 09 SOLAS CH V Reg 16.

There is a difference of 35 degrees between the magnetic compass and gyro repeater. A tech report attesting to proper operation is to be sent to the U.S. Coast Guard prior to departure.

The fire alarm panel for the cargo holds is showing an alarm for "Cargo Hold Number 7." This alarm cannot be reset. Class to attest to the proper operation of the fire detection system prior to departure. 74 Solas, Chapter I, Reg. 11

The symbols on the fire control plan are not accurate in regards to the installed fire doors. For example, an installed A-60 door has the symbol of a standard door. Class to verify the symbology of the fire control plan in regards to the doors actually installed. 74SOLAS Ch II-2 Reg 4

Multiple railings on the port side main deck are broken. The broken railings are in between cargo hold 1 and 2, across from cargo hold 5 and across from cargo hold 7. Class to attest to proper repairs.

A hydraulic oil leak coming from a storage tank located in the engine stores room is resulting in excesive fruid on the deck. This causes concern for crew safety. Class to attest to cleanliness of the space and repair to the hydraulic leak. 74SOLAS CH I Reg 11

The pneumatic pilot loading return valve failed. Caused by a worn out "O" ring. System did not respond to full stop to dead slow ahead for approximately four (4) minutes.

The port bridge wing main engine indicator for ahead and astern is not working in the astern mode.

The vessel was converted from a tank vessel to a bulk carrier. Some of the accommodation spaces have been re-designated on the new fire plan. However, the markers above the doors still reflect the old fire plan. All spaces need to be looked at and re-designated according to the new fire plan. Repair to satisfaction of USCG.

Most of the fire doors in the upper deck space do not close properly. The self closing devices need to be adjusted or some are missing altogether. All doors in this area need to be inspected and work according to the new fire plan. Repair to satisfaction of USCG.

The brake on the starboard life boat failed. The brake had to be held down in place for the system to retrieve the boat. When he did not apply pressure the brake would automatically disengage and fail to recover the boat. Class must inspect prior to departure.

Vessel was issued a Letter of Deviation to enter the Port of Corpus Christi, Texas, without a properly operating echo depth sounder 33CFR164.35(i). Based on the understanding that the equipment will be operational prior to departing the port.

The subject vessel has an inoperable secondary radar. The vessel needs to submit a service report before departing.
