On a vessel of at least 7.9 meters in length, a visual and audivle alarm must be provided at the operating station to indicate a high water level in each of the following unmanned spaces. (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarete; (2) A machinery space bilge, bilge well, shaft alley bilge or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertigtclosure, such as a space with a non-water-tight hatch on the main deck. MI observed that all high water alarms shared one common visual indicator.

All machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Vessel has through hull fittings and water piping going through the engine battery storage compartment. 46CFR 182.01-5

Machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads of double diagonal wood, marine plywood, steel plate or equivalent construction. Vessel had multiple bulkhead penetrations that were not vapor/water tight. 46 CFR 182.35-1

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Wet exhaust system has pinhole and is leaking water and exhaust fumes. Repair exhaust system.

The master shall conduct sufficient drills to make sure all crew members are familiar with their duties. CG Inspectors must witness underway drills in accordance with a COI Inspection. Coordinate with Marine Inspector to witness drills.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Delamination is occurring on forward starboard main deck. Repair deck.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. There are four loose overhead beams (two port, two starboard) in the engine compartment. Provide proof of all beams sufficiently anchored.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Delamination is occurring in the berthing compartment on the port forward bulkhead. Repair delamination.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The starboard rudder post is leaking. Repair leak.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Strut backing plate in the fuel tank space is leaking. Repair leak.

At each inspection for certification and at every other vessel inspection all observed unsafe practices must be corrected. Broken glass was found in the child lifejacket storage area. Remove all broken glass.

Each ring life buoy must marked with Type II retroreflective material approved in 36 CFR 164.018. The arrangement of the retroreflective material must be as specified by IMO Resolution A.658(16). Ring life buoy retroreflective material was not sufficient. Add retroreflective material to ring life buoy.

A vessel must carry either a first-aid kit approved under approval series 164.041 or a kit with equivalent contents and instructions. Eye wash in first-aid kit is expired. Replace eye wash. 46 CFR 184.710

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in applicable unmanned spaces. Visual indicators for all spaces were inoperable and unlabeled. Repair visual alarm and label each alarm.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio-beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80. Vessel Radio Station and Radio Telephony Certificates showed different call signs. Obtain a new Radiotelephony Certificate with the correct call sign. Vessel FCC Certificates display a different MMSI number than what is entered into the VHF radio. Obtain a new FCC inspection to issue new Radio Telephony and Radio Station certificates and update emergency calling placards. A Coast Guard Marine Inspector must be present during the inspection.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio-beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80. Operator did not have FCC Marine Radio Operator Permit onboard. Permit must be with the operator.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Retroreflective material and general condition of 04 excess child life jackets were significantly deteriorated. Remove or replace life jackets.

Vessel 15-character beacon ID is not identical to the manufacturer installed UIN. Provide proof that NOAA was contacted to resolve the discrepancy.

The fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Reach rod is held in place with zip ties. Replace with a reliable form of holding the reach rod in place.

Vessel has lapsed on its dry dock dates. Vessel shall not operate according to its COI until a satisfactory dry dock exam is conducted.

Found support stringer in fwd anchor line storage to be split, not capable of supporting deck as it was designed, and its where passengers walk. Repair or replace with support board to meet initial strength and support for deck above.

Vessel COI expired on 14MAY2020. Vessel operator notified they cannot operate vessel until a COI examination is completed.

Nautical publications: Coast Pilot [name withheld], Tide Tables, and Current Tables for the vessel are not up-to-date. Provide vessel with updated publications.

Charts not current. Update or replace charts.

Life buoy waterlight battery inoperable. Replace battery.

Overboard sewage valve not secured. Secure valve with locking mechanism.

Insufficient retro tape on 01 adult life vest. Replace retro tape.

Bilge in berthing compartment needs to be smoothed out so water does not build up under the resin within that space.

Update charts and pubs of area you work. IAW 46 CFR 184.420

Label bilge and high water lights to tell which space. Also FWD compartment and tank room needs bilge pump light to work. IAW 46 CFR 182.530

SOLAS tape and label PFD’s that were left out during inspection. IAW 46 CFR 180.75

Catch up EPIRB log. IAW 46 CFR 185.702

Shore power line has insulation pulled back at boat plug. IAW 46 CFR 176.800

Add hose to hand pump so it can reach all spaces and go out of cabin to go overboard. IAW 46 CFR 182.520

Thru hull valve needs plug in FWD space starboard side. IAW 46 CFR 179.212

Double clamp and single clamps need added in engine room. Check all hoses to verify. IAW 46 CFR 176.800

Port engine exhaust cooling needs new hose out board side, and weld area still leaks, make repairs as needed. IAW 46 CFR 182.430

Put proper MMSI number on black face radio. IAW 47 CFR 80

Provide maintenance to all life jackets. Re-label and replace retro tape. Separate all child and adult life jackets label with correct number.

Provide high water alarm with audible and visual indicator to fwd compartment. Automatic bilge pump should discharge overboard. A check valve need be installed on discharge line. The ball valve should be located close to through hull fitting.

Pre-engineered system discharge indication panel needs to be at operating station.

Make bilge indication light in center compartment operable.

Add chaffing gear to sea water suction supply line on port and stbd side.

Replace/ repair lagging on stbd outboard exhaust and re-clamp. Re-clamp port inboard exhaust lagging.

Alternator belt on generator is loose needs to be tightened. Adjust rotating guard that covers alternator.

Water leak on port engine exhaust pipe repair as needed.

Replace generator sea water discharge line at muffler.

Add proper cover to receptacle outlet in eng room on fwd bulkhead

Wiring above generator needs to be cleaned off.

Collision bulkhead port and stbd side main deck needs to be strengthened deck is weak.

Check all clamps and add new clamps on drain line for sink and water supply lines for generator.

Add mousing to anchor shackle.

Add sign to aft ladder going topside. Sign should say CREW ONLY.

STBD aft bulkhead top, port side in engine room needs to be sealed. Small 1/2" hole.

All around anchor light being blocked and is not visible . Move to a different location.

Post Emergency and radio broadcast instructions at operating station. Add fuel shut off to emergency instructions.

Provide update copy of navigation rules.

Provide supervisory training for master.

Vessel made structural alterations without the approval of the CG OCMI. Vessel is prohibited from operating with passengers onboard until a new simplified stability test is satisfactorily conducted for the vessel and drawings, sketches, and/or written specifications describing the details (electrical, structural, mechanical, etc.) of the alterations are submitted to the CG OCMI for approval.
