The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. 1) The door leading from the galley to the engine room was unable to fully close and maintain the fire boundary. 2) Port side galley door doesn't latch properly when fire doors are activated.

All electrical installations shall be to the satisfaction of the OCMI. 1) Laundry room washer receptacle has ground pin broken off in one side of the outlet and extension cord with multiple plugs being utilized presenting an electrical hazard. 2) Junction box over the #1 SSDG is not mounted and just hanging. 3) Portable sump pump is being utilized as permanent installation using extension cord for galley ice maker drain. 4) Wires and connectors for equipment that was removed from the utility room on the 400 deck need to be secured.

Valves for removing water or impurities from fuel in water traps or strainers shall be provided with caps or plugs to prevent fuel leakage. Main and generator fuel filters are missing plugs for drain valves.

Each vessel must meet the applicable requirements in Subchapter S of this chapter...Penetrations and openings in watertight bulkhead penetrations must have means to make them watertight. Multiple cable penetrations at WTD #2 is loose and not watertight. Cable penetrations at WTD #1 are loose and not watertight.

A fire control plan must show the location of fire protection/emergency equipment. The fire control plan does not show the interconnected smoke detector system installed by A Gow on 23APR2024.

A vessel of more than 19.8 meters (65 feet) in length carrying more than 600 passengers or with overnight accommodations for more than 49 passengers must have:(a) Two generator sets;(b) An electrical power system that complies with the requirements of §§ 111.10-4, 111.10-5, and 111.10-9, in subchapter J of this chapter;(c) A final emergency power source, as defined by § 112.01-20 in subchapter J of this chapter, with sufficient capacity to power the loads listed in § 112.15-5 in subchapter J of this chapter for three hours; and(d) The final emergency power source located outside the machinery space. The vessel's starboard side generator shutdown while underway with 74 passengers on board. The vessel's emergency generator came on within 15 seconds and the port generator was brought on line with in a couple of minutes.46 CFR 120.312 (a)(b)(c)(d)

The construction and arrangement of a vessel must allow the safe operation of the vessel giving consideration to protection against fire. Fire Door on 2nd Deck leading to passageway for passenger accommodation did not latch. Inner spring found to be defective.

…the public address system must be […] audible during normal operating conditions throughout the accommodation spaces. Passenger cabins have the ability to mute the public address system when switched to Channel 1.

Each vessel must have adequate emergency lighting fitted along the line of escape… Various instances of emergency lighting throughout vessel, as designated on Fire Control Plan, not illuminated.

or each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse […]. The indicator must show whether the door is open or closed. Mechanism for activation switch on 02 watertight doors found to be bent and unable to accurately indicate status of door. Due by next DDX.

[…] marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Dead end wires in E/R and steering compartment.

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Live deadend wires located above both main engines and generators in the engine room. 46CFR120.200

Prove proper operation of fixed sprinkling system. Vessel completed extensive repairs of sprinkler system and system was not ready for inspection during USCG attendance. -46 CFR 181.05-5 ; 50C

Flow meter is currently an indication of pump running but not flow or pressure. Vessel intends to install alarms associated with current flow meter -46CFR182.20-10 ; 705C-Prior to carrying passengers in 2023

(Cleared) 46 CFR 122.524(a) – Conduct a fire and man overboard drill in the presence of an attending CG Marine Inspector. The vessel did not provide adequate crew during the COI inspection. (Combined 2 similar CG-835 items).

33 CFR 151.25(h) – Provide a corrective action plan and implement the same to ensure the Oil Record Book is being properly maintained to include Master’s signature, relevant entries with corresponding receipts and documents. (Combined 3 similar CG-835 items).

46 CFR 115.830(a) – Remove the combustible material stowed under the dry exhaust on the port and starboard side.

Air conditioning unit in bow thruster/laundry room was wired with a 'permanent' extension cord. Remove and rearrange wiring to be in accordance with 46 CFR 120.340(r).

Electrical outlet box was missing front cover, presenting shock hazard. Renew outlet cover in accordance with 46 CFR 120.200(b).

Light fixture in A/C flat was loose and not properly mounted/secured. Mount in a fixed position in accordance with 46 CFR 120.410(c)(2).

Laundry room/bow thruster room bilge had nearly two feet of water in the bilge. Bilges must be clean and dry for inspection in accordance with 46 CFR 115.802(a)(1).

Service or Replace EPIRB. Update any contact information as necessary in NOAA Registration.

Many small electrical lines had improper terminations, including some on main propulsion engines. Terminate ends in an approved manner or remove to provide safe environment, per 46 CFR 120.200(c).

Phone in lower passageway had un-mounted junction box with exposed wires. The box should be properly mounted in a way as to not present a shock/fire hazard, in accordance with 46 CFR 120.200(b).

Engine room had excessive oil and fuel in the bilge as a result of ongoing engine maintenance. Bilges to be pumped at conclusion of engine work. 46 CFR 115.802(a)(1).

Oily water separator was undergoing maintenance and not available for inspection. Complete work and make available for USCG inspection prior to sailing international. 46 CFR 115.804.

Starboard exhaust system found leaking corrosive fluid through gaskets near side-shell. Deck plate corroded & needs renewal. Make fittings leak-tight and arrest corrosion. 46 CFR 119.430(b).

Several parts of the navigation suite were tagged out for maintenance. Complete work and make ready for inspection per 46 CFR 115.804(b).

Fire Control Plan D, copy onboard vessel, did not have a USCG approval letter onboard. Plans must be approved by OCMI or MSC. Provide approval to OCMI's satisfaction, per 46 CFR 116.202.

Main diesel engines were not available for safe testing, due to other work taking place in engine room at the time. Make available for USCG inspection prior to carrying passengers. 46 CFR 115.804(a).

Smoke detector batteries were expired and units were not working. Renew batteries.

Engine room remote fuel shut-offs could not properly actuate. Overhaul remote connection so valve can actuate.

Generator was undergoing maintenance and was not available for inspection. Make generator ready for op-testing and inspection prior to carrying passengers.

Vessel not ready to get underway to perform drills. Vessel must get underway and perform required drills (Fire, MOB, Abandon Ship, Security).

IBA’s were being serviced at time of inspection. Make IBA’s ready for inspection prior to carrying passengers.

Bow thruster tunnel has wasted plate and holes beyond acceptable levels. Crop and renew plate to original scantlings, renew unit, or propose new design at next drydock.

Replace all yellow fire hoses that are not commericially lined and do not meet UL 19.

Bilge high level alarm inop in lazerette. 46 CFR 119.530

Renew emergency generator enclosure due to wastage and weather tight integrity.

Prove proper operation of 3 inoperable general alarm bells.

Provide loadlines markings in contrasting color on port and starboard sideshell.

Prove proper operation including remote closure capability of port side 200 deck fire door and port side main deck aft crew fire door.

Ensure lifejacket storage areas in passenger cabins are marked with appropriate SOLAS labels.

Replace missing FSD closure indicator on promenade deck.

Replace faulty relief valve on air receiver.

Replace angle bar under the deck plate that was found corroded and in need of replacement in the crew/AC flat and in the Laundry/Bow thruster room.

Replace worn valve housing for gate valve #48 and 8 other valves called out by company.

Provide junction box for cables joined with wire nuts in forepeak.

Restore engine to operational condition (#4 cylinder leaking coolant due to worn o-ring & cylinder liner).

Vessel is restricted on the SEADROME DOCK until repairs to the STBD Main Engine CAT 3512 are completed to the satisfaction of an attending marine inspector.

Vsl's P/S Mn Engines operation restricted to the manufacturer's minimum recommendation on the pyrometer temp readings (975 degrees F) to minimized pitting & fractures to cylinder liners & until all cylinder liners inspected by CAT authorized rep [name withheld] maintenance procedures on lube oil sampling tests & water coolant treatment program.

Send a report of the cylinder liner inspections for the Port & STBD Main Engines to the nearest OCMI upon completion. Report must be verified either by CATERPILLAR manufacturer representative or authorized dealer.

Repair the electrical connection to the battery charger of the Port AUX GEN wire set to the 12 volt forward battery bank. Two wires found charred & insulation wire exposed. This may have cause the spark leading to the battery exploding. Big safety electrical problem. Check vent cap (clear) to battery cover for proper venting.

Remove excessive oil in the engineroom bilges particularly under STBD Reduction gear and OWS foundation platform area.

Vessel shall conform to rail courses intervals (4") as per D17 (moc) Insp Note 01-02 in the 200 deck & 300 deck area.

Renew starboard overboard pipe penetration for sewege overboard located A/C space.

PROVE THAT THE INSTALLATION OF THE HYDRAULIC PUMPS ADJACENT TO THE P &S LIFERAFT STOWAGE WILL NOT INTERFERE TO THE RELEASE OR DEPLOYMENT OF THE LIFERAFTS AND PROVIDE ADEQUATE ACCESSIBILITY TO LIFERAFTS.

PROVIDE APPROVED PLAN FOR THE RELOCATION OF ACCESS TO THE STEERING GEAR ROOM.

PROVIDE PLAN OR INTENTION OF THE VESSEL TO THE USE AND OPERATION OF TWO RUBBER BOATS STOWED AFT OF THE SUN DECK INCLUDING THE CARRIAGE OF SPARE GASOLINE IF THERE IS A NEED FOR IT.

RELOCATE RELEASING GEAR PINS ON PORT AND STBD. FORWARD MOST LIFERAFTS LAUNCHING GEAR, PRIOR TO SAILING.

THE NEWLY INSTALLED BOAT CRANE, PORT AND STBD., ARE NOT TO BE USED UNTIL THE INSTALLATION IS APPROVED BY THE COAST GUARD.

PROVE THE SATISFACTORY COMPLETION OF THE MODIFICATION TO THE FORWARD CABINSSUN DECK,PORT AND STARBOARD, AFT OF THE PILOTHOUSE, FR 33-38, INSTALLATION AND WORKMANSHIP IN ACCORDANCE WITH APPROVED PLAN OR DRAWING NO.86-002 AND COMMENTS LETTER BY G-MSC-1, 28NOV86.
