A vessel that is exposed to salt water more than 3 months in any 12 month period must undergo a dry dock examination and internal structural exam at least once every 2 years. Vessel failed to complete dry dock exam prior to 29FEB2024. Vessel must complete satisfactory dry dock exam prior to returning to service as an inspected small passenger vessel.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Vessel lost its anchor and ran aground aground in Long Beach Harbor. Vessel required to submit repair plan to USCG Sector LALB of intended repair. Vessel restricted from carrying pax.

Except as otherwise provided in this section, each space accessible to passengers or used by the crew on a regular basis, must have at least two means of escape, one of which must not be a watertight door. The vessel’s galley has only one means of escape, leading aft. Vessel is prohibited from carrying overnight passengers or utilizing overnight accommodations until regulations or alternative compliance is met to OCMI satisfaction.

The two required means of escape must be widely separated and, if possible, at opposite ends or sides of the space to minimize the possibility of one incident blocking both escapes. The vessel’s galley has only one means of escape, leading aft. Vessel is prohibited from carrying overnight passengers or utilizing overnight accommodations until regulations or alternative compliance is met to OCMI satisfaction.

The minimum clear opening of a door or passageway used as a means of escape must not be less than 810 millimeters (32 inches) in width, however, doors or passageways used solely by crew members must have a clear opening not less than 710 millimeters (28 inches). The sum of the width of all doors and passageways used as means of escape from a space must not be less than 8.4 millimeters (0.333 inches) multiplied by the number of passengers for which the space is designed. The doorways for all passenger berthing rooms are 30 inches in width. Vessel is prohibited from carrying overnight passengers or utilizing overnight accommodations until regulations or alternative compliance is met to OCMI satisfaction.

A means of escape which is not readily apparent to a person from both inside and outside the space must be adequately marked in accordance with § 185.606 of this chapter. Doors and/or hatches that serve as a means of escape are not properly marked “EMERGENCY EXIT, KEEP CLEAR”. Vessel is prohibited from carrying overnight passengers or utilizing overnight accommodations until regulations or alternative compliance is met to OCMI satisfaction.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. The lifeline must be as furnished by the manufacturer with the approved life float. Each paddle must be of at least 4 feet in length, lashed to the life float to which it belongs, and buoyant. Life floats must not be stowed in tiers more than 4 feet high. When stowed in tiers, the separate units must be kept apart by spacers. The name of a vessel must be marked or painted in clearly legible letters and numbers on each life float and each life float paddle. One paddle was missing from one of the life floats, the lifelines of numerous life floats were found to be deteriorated, many paddles were not lashed to their life float, spacers were missing from between many of the life floats, the marking of the vessel's name was found to be faded on two of the life floats, and the vessel's name was not marked on numerous life float paddles.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. The starboard engine belt was found to be missing its cover/guard.

Vessels having overnight accommodations for passengers must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. The fire detector located in the galley was missing.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and the vessel from electrical hazards, including fire and electrical shock. MIs observed two missing bulkhead emergency lights and one that was inoperable as well as 18 open lighting receptacles in passenger accommodation areas.

Each distribution panel must be protected from falling debris and dripping or splashing water. The engine room panel is missing its door.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The container for child life jackets was not labeled with the life jacket storage capacity and size.

Employers must ensure that they and their crewmembers meet the requirements of this part (46 CFR 16). While conducting a DAPI spot check, consortium enrollment confirmation, an accurate crewmember list, and most recent MIS report could not be provided. Additionally, alcohol testing equipment could not be provided.

Except on a vessel limited to daytime operations, the magnetic compass must be illuminated. The vessel could not prove illumination of the magnetic compass.

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 0.5 inches high: “DISTRESS SIGNALS”.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. Only the third/last page of the stability letter has been posted and is visible at the operating station.

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons to be transported. Deficiencies 02-05 provide objective evidence that the vessel is substandard and not proactively managing technical and operational requirements. Vessel must complete a satisfactory expanded annual inspection prior to any movement.

At each inspection, all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. The vessel had approximately 20 gallons of flammable liquids stored throughout the vessel in unapproved storage locations. Additionally, the vessels flammable storage cabinets on the uppermost deck were no longer in a serviceable condition.

A passenger accommodation space must be maintained to minimize fire and safety hazards and to preserve sanitary conditions. Passenger accommodation areas had a strong smell of sewage and were cluttered with excessive trash that obstructed the means of emergency egress.

Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. The vessels rail courses on the uppermost passenger deck were either missing, deteriorated, or very loose.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and the vessel from electrical hazards, including fire and electrical shock. The vessel had numerous wiring issues which posed a major hazard to passengers and crew. The hazards identified include: 01 non operational emergency light, open receptacles in passenger accommodation areas, exposed wiring in the overhead of berthing areas, a deteriorated shore power cable, and an air compressor which was being powered off of a 30 foot extension cord.

Each ring life buoy on a vessel must be approved in accordance with 46 CFR 160.050. Vessel's ring buoy on the starboard bridge wing is no longer in serviceable condition.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. The vessel's life float light was not operational.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Vessel had approximately 10" of standing water in the machinery space bilge.

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast guard upon request. Vessel did not have an official logbook and lacked adequate documentation of passenger manifests, drills, and monthly EPIRB testing.

At each inspection, all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Vessel has a leaking fuel hose on the port diesel generator.

Each life jacket must be marked in clearly legible block capital letters with the vessel's name. The vessel had numerous life jackets which were labeled with a different vessel's name.

The float-free link required by 180.130(a)(1) must be of proper strength for the size of the life float as indicated on its identification tag. Vessel had 02 life floats which were missing their identification tag for the weak link.

The construction and arrangement of a vessel must allow the safe operation of the vessel with the terms of its COI giving consideration to the provisions for a seaworthy hull. The vessel has excessive hull deterioration on the uppermost deck and on the starboard midships embarkation door. The vessel is required to conduct hull thickness gauging and must make repairs prior to obtaining credit dry dock.

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessel has not submitted plans or installed the required Watchmen system. Vessel is restricted from carrying overnight passengers until deficiency is rectified. 46CFR185.410(b)

An interconnected detection and alarm system must: (1) Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217 (incorporated by reference, see 46 CFR 175.600); (2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system; (3) Contain an independent power source; and (4) Alarm on low power. Vessel has not installed an interconnected Fire detection system as required. Vessel is restricted to no more than Limited Coastwise route until deficency is rectified. 46CFR181.450

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Local lifeguard reported inspected vessel collided with another recreational boat. Vessel not to carry passengers until after Marine Inspector approval.

The master of a vessel, except a vessel listed in § 122.502(a), shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. Operator could not provide a count for how many people were on board or how many people departed the vessel in the past 24 hours.

Except on a vessel limited to daytime operations, the compass must be illuminated. Compass light could not be energized.

Each portable fire extinguisher on a vessel must be of an approved type. Fire extinguishers do not mean type approval with out proper mounting. Found a fire extinguisher not mounted to a secure location.

The master of a vessel required to prepare a crew and passenger list by paragraph (a) of this section shall see that the list is prepared prior to departing on a voyage. The list must be communicated verbally or in writing ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The crew and passenger list shall be available to the Coast Guard upon request.

Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license.

A portable watertight container marked as required by § 185.614 of this chapter, carried at the operating station. Flares found in a nonwatertight container.

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage.

Gauges to indicate lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator Vessel needs to prove oil pressure gauges in pilot house are operational.

All enclosed spaces within the vessel shall be properly vented or ventilated. Where such opening would endanger the vessel under adverse weather conditions, means shall be provided to close them. Vessel shall fix port side goose-neck.

Each survival craft must be secured to the vessel by a painter with a float-free link. Vessel to replace old painters with satisfactory line.

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. Vessel to provide a means of opening port side crew accommodation escape hatch with handle.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of the COI giving consideration to necessary facilities for passengers and crew. Vessel to repair starboard side emergency ladder pin angle iron.

At each initial and subsequent inspection for certification, the MI shall require that all observed unsafe practices and hazardous situations be corrected. MI identified fuel leak on the Port Main Diesel Engine fuel pump. Vessel to make proper repairs.

A hatch exposed to the weather must be watertight. All (06) flush deck hatches are inoperable & do not seal.

At each initial & subsequent inspection for certification, the MI shall require that all observed unsafe practices & hazardous situations be corrected. All smoke detectors on vessel are outside manufactures 10 year renewal recommendations. Vessel not authorized overnight accommodations until detectors are renewed with UL217 detectors.

Each battery used in an item of life saving equipment must be replaced during the annual inspection if the expiration date of the battery has passed. Water light batteries are expired on all water lights. Renew all batteries & conduct maintenance on battery terminals. Vessel not authorized night time operations until batteries renewal.

At each initial & subsequent inspection for certification the MI shall examine & test for safe electrical condition. (02) outlets on pilothouse are loose and unserviceable. Vessel service outlets.

Pressure vessels must be tested & inspected in accordance with part 61 subpart 61.10. Vessel to make pressure vessel available for inspection.

Vessel to provide a copy of their 2018 MIS report.

Vessel to provide an updated copy of FCC safety certificate.

Vessel to remove refrigerator from pilot house & (03) daisy chained surge protectors from pilot house.

Vessel to remove accumulation of oil in engine room. Vessel had 8 five gallon pails of oil unsecured in stbd side engine room.

Vessel to remove unauthorized propane grill and (03) associated propane tanks from vessel.

All mattresses must comply with either: (1) The US Dept. of Commerce standard for mattress flammability (FF 4-72.16), 16 CFR Part 1632, subpart A & not contain polyurethane foam; or (2) IMO Res. A.688(17) (incorporated by reference, see 46 CFR 175.600). Mattresses that may contain polyurethane foam. Vessel to provide sufficient proof that all on board mattresses meet the required standards. Vessel to remove, replace/renew all mattresses from vessel if they do not meet required standard; as indicated.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Vessel does not have an anchor onboard because the line that attaches to the anchor chain frayed and parted.

46 CFR 180.175(f)(4) Light. The light must be a floating wateright approved under approval series 161.010 or other standard specified by the Commandant. Replace batteries in floating water lights attached to lifefloats; batteries must be of an approved make/model for the floating waterlight (Qty. 2); the installed batteries did not comply with the approved make/model as listed on the casing/body of the floating light.

46 CFR 180.175(f)(3)(iv) Painter. Be stowed in such a way that it runs out freely when the life float or buoyant apparatus floats away from a sinking vessel. Life Floats not stowed in a float free arrangement that would allow the free flow of the pointer lines. Reconfigure Life Floats and paineter line to ensure float free arrangements.

46 CFR 164.46(d)((iii) Operations AIS. The accurate broadcast of a properly assigned Maritime Mobile Service Identity (MMSI) number. AIS broadcasting incorrect vessel informatin, MMSI No. and Call Sign. Correct AIS to broadcast correct vessel information.

Provide covers for all open spaces on all circuit breakers.

Provide temporary cover for all rotating machinery and open piping while repairing bow thruster machinery.

Replace/renew STBD shaft due to current condition, as identified.

Make port shaft available for inspection or replace due to corrosion.

Replace 3 pipe couplings with permanent piping.

Repair forward weather tight doors.

Provide Professional Engineer statement as to background/stucture of blackwater tank. Aft transverse bulkhead is starting to buckle at integration on blackwater tank.
