The security plan under ASP states the VSO is the Master [name withheld] stated that is not the case and they purposely didn't want the Master as the VSO. Provide a designation of an appropriate VSO.

Vessel CSO or VSO could not provide a schematic of vessel showing designated restricted areas to make the ASP more vessel specific. Provide a schematic with designated restricted areas of vessel.

Vessel security records show drills haven not been conducted once every three months. Conduct and log a security drill prior to carrying passengers.

CSO/VSO failed to notify the National Response Center of a Breach of Security after vessel crew member was notified of a "shooting incident" onboard the vessel during the charter. CSO/VSO must review vessel ASP and notification requirements for Breach of Security.

Each portable fire extinguisher on a vessel must be of a type approved by the Commandant. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by table 1 to § 118.500(b) and other provisions of this section. Portable extinguishers have not been inspected monthly and logged as required.

A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys of not less than 610 millimeters (24 inches) in diameter. Vessel is missing one of the required three life buoys.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel owner made alterations to the stern of the vessel around swim platform that were never approved by the local OCMI. Vessel is restricted from carrying passengers until alterations are removed and properly approved.

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Must be made in clearly legible letters of the Latin alphabet or Arabic or Roman numerals not less than four inches in height. Vessel is missing name on stern.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. A fire drill must be conducted at least once each month. Drills were not being conducted as required.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station man overboard conditions. Man overboard instruction did not contain procedures for using rescue boat.

Alcohol-testing devices must be used according to the procedures specified by the manufacturer of the testing device and by this part. Alcohol swabs onboard were expired.

Each portable fire extinguisher on a vessel must be of a type approved by the Commandant. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant [name withheld] witnessed three fire extingiushers with broken tamper seals. MI required new servicing report for those identified.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will results in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock of person. MI identified missing cover on electrical outlet in mens head.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Operator could not prove when electronic charts were last updated.

The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device:A self-propelled vessel that is certificated to carry more than 150 passengers. MI identified AIS vessel information is not update.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List;(4) Tide tables; and(5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel does not have the required publications for prior operating AOR and not Miami AOR.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. MI identified 19 adult and 12 kid life jackets with improper vessel name marking.

The owner, managing operator, or a qualified servicing facility as applicable shall conduct the following inspections and tests:For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by NFPA 10, (incorporated by reference, see § 114.600 of this chapter) with the frequency specified by NFPA 10. Vessel does not have a log for conducting monthly fire extinguisher checks.

Vessels on rivers route must carry:Three hand orange smoke distress signals approved. Vessel currently has two orange smoke distress signals.

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States:vessel of 20 meters or over. Vessel does not have required Bridge-to-Bridge cert.

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Flare container not properly labeled.

When a marine employer determines that a casualty or incident is, or is likely to become, a serious marine incident, the marine employer shall take all practicable steps to have each individual engaged or employed on board the vessel who is directly involved in the incident chemically tested for evidence of drug and alcohol use as required in this part. Vessel operator does not have required kits to test for drug/alcohol.

Rails must be permanently installed except that the following rails may be removable:Rails in way of embarkation stations and boarding locations. MI identified on the upper deck multiple rails/gates thats can be opened and allow passengers to fall directly overboard. Gates shall be locked to prevent this action.

All persons requiring unescorted access to secure areas of vessels, facilities, and OCS facilities regulated by parts 104, 105 or 106 of this subchapter must possess a TWIC before such access is granted, except as otherwise noted in this section. A TWIC must be obtained via the procedures established by TSA in 49 CFR part 1572. CSO and deck hands do not have required TWIC.

Each vessel must meet the applicable requirements in subchapter S of this Chapter. Vessel removed its paddle wheel and added a stern deck, submit weight changes and any other requirements to the Marine Safety Center (MSC) to determine if the current stability letter is still valid. Prior to carriage of passengers.

A request for an amended Certificate of Inspection must be made to the cognizant OCMI by the owner or managing operator of the vessel at any time there is a change in the character of a vessel or in its route, ownership etc. Vessel has new owner, new name, and is operating in a new OCMI zone. Prior to carrying passengers schedule and receive a new to zone inspection by the local OCMI and provide any and all changes to the vessel that may affect the info on its COI.

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] and exterior profile drawings must be submitted to MSC for review.

All piping, valves, and fittings shall be in accordance with good marine practice & suitable for the purpose intended. Support bracket for aft fire main pipe sheered. 46CFR181.15-5(c) Old T

Suitable covers, guards or rails should be installed in way of all exposed and hazardous places such as gears, machinery, etc. Port MDE had exposed belt and pulley outside of installed guard. 46CFR177.35-15 Old T

All Electrical installations shall be to the satisfaction of the officer in charge of Marine Inspections. Dead end wires found in switchboard/electrical closet. 46CFR183.01-15(a) Old T

The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Fuel tank sight glass leaking from lower connection. 46 CFR 116.405 (a)

At each vessel inspection, the marine inspector shall examine applicable spaces to determine that they are in a sanitary condition. Sewage line leaking in engine room. 46 CFR 176.25-45 (a)

Vessel must be fitted with a satisfactory arrangement for draining any watertight compartment. Vessels bilge suction piping for the FWD compartment had a pin hole leak.

A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average temperature on the unexposed side does not rise more than 139 °C (250 °F) above the original temperature, nor does the temperature at any one point, including any joint, rise more than 181 °C (325 °F) above the original temperature. Engine room STBD side overhead and portside bulkhead had missing pieces of fire protection insulation.

Found pitting in fore peak during inspection fwd of the collision bulkhead. Pitting measured at 1/4". Must meet ABS aluminum rules per ABS Survey After Construction. Crop and renew to the satisfaction of the Coast Guard at next dry dock.

Found upper deck passenger capacity labeled incorrectly at 186 passengers as per stability letter, instead of 172 passengers as per COI.

Found broken guard on starboard main diesel engine.

The vessel is currently broadcasting (1) Improper Call Sign, (2) Zero Length and Beam, (3) Zero Draft.

Vessels call sign on FCC Radio Authorization and FCC Safety Radio Telephony Certificate do not match. Correct so call signs match on FCC Documentation.

Marine inspectors identified cracking on forward superstructure on port and starboard sides. OCMI approved temporary repair method of drill stops to be inspected upon completion. 46CFR115.700.

Vessel has experienced damage to one of her propellers and rudders, as observed by a CG Marine Inspector. The vessel must be drydocked and made available for inspection by CG MI prior to operating. Due Date: PRIOR TO SAILING. Cite: 46CFR115.600(d); 46CFR115.670; 46CFR2.01-10(b).

Make any propulsion or steering gear that was removed available for inspection by a CG MI, including the damaged propeller and rudder. Due Date: PRIOR TO SAILING. Cite: 46CFR115.670; 46CFR2.01-10(b).

Vessel has experienced damage to one of her propellers and rudders, as observed by a CG Marine Inspector. Complete repairs to propeller & replace rudder to the satisfaction of the attending CG MI, to include an underway operational test. Cites: 46CFR115.600(d); 46CFR115.670. Due date: PRIOR TO SAILING.

Due to damage to vessel's running gear from underwater obstruction, vessel's crew must complete & log an internal structure exam of each watertight space on the vessel prior to each trip with passengers, to check for water intrusion. Cites: 46CFR115.670; 46CFR2.01-10(b). Due date: UNTIL NEXT DRYDOCK
