Lifebuoys must have the following attachments and fittings: At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Vessel had 2 SM-3 float lights that were not fitted with lithium ion batteries. Replace currently installed batteries with batteries specified in 46 CFR 161.010. Vessel restricted to daylight operations only. 46 CFR 199.70(a)(3)(ii) 17 C

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. A hydrostatic release unit used in a float-free arrangement must be approved under approval series 160.162. Both of the vessel's two hydrostatic release units (HRU) are expired. Replace expired HRUs with new units. 46 CFR 199.130(c)7 16C

There must be a sufficient number of deck officers, able seamen, or certificated persons on board the vessel to operate the survival craft and launching arrangements required for abandonment by the total number of persons on board. Vessel failed to provide 2 lifeboatmen to satisfy requirements of this section. Provide credentialing for 2 lifeboatmen. 46 CFR 199.100(b) 50C

Electrical cable runs in main storage space on the 02 housing deck have sustained severe fire damage that deems it not serviceable. Must replace all damaged cable runs to the 120V distribution panel marked "Panel C" located on the main housing deck. All cable replacements must be in kind in accordance with the vessel electrical plan and 46 CFR 111.60 (Wiring Materials and Methods) to the satisfaction of the U.S. Coast Guard.

Must replace all damaged fire fighting appliances; e.g. fire extinguishers, caused by fire damage to the satisfaction of the U.S. Coast Guard.

Except as provided in paragraphs (b) through (g) of this section, each vessel must undergo drydock, internal structural, and cargo tank internal examinations as follows:(1) Except under paragraph (a)(2) of this section, vessels that operate in salt water must be examined in a must be examined in accordance with the intervals set forth in Table 91.40-3(a) of this section. Where Table 91.40-3(a) indicates a 2.5 year examination interval, it means a vessel must undergo two examinations within any five year period. No more than three years may elapse between any two examinations. Make the following spaces available for inspection, bow rake, port and starboard #3 voids, and center fuel tank. Spaces must be inspected by a USCG MI prior to the vessel receiving CG ISE credit. 46 CFR 91.40-3(a)(1)

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Corrosion was observed on the center stern rake’s forward bulkhead in the vicinity of the first and second bottom longitudinals adjacent to the space’s entrance ladder. Provide a 3 inch by 3 inch grid pattern gauging report of the forward bulkhead between the first and second bottom longitudinals from the bottom plate to 2 feet up the forward bulkhead. The gauging report shall be submitted and reviewed to the satisfaction of the attending USCG MI prior to the vessel receiving CG ISE credit. 46 CFR 92.01-10(a)

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Corrosion was observed on the center stern rake’s first and second bottom longitudinals from center in the space’s inner most bay. Provide a 3 inch by 3 inch grid pattern gauging report of the corroded bottom longitudinals or submit a repair proposal for the replacement of the longitudinals to the satisfaction of the attending USCG MI prior to the vessel receiving CG ISE credit. 46 CFR 92.01-10(a)

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. An inset was observed in bottom plate at girthbelt 7.5 in the #2 starboard void. Make the inset available for USCG MI external inspection prior to receiving CG ISE credit. 46 CFR 92.01-10(a)

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Corrosion was observed on the upper most side longitudinal between frames 9 and 10 in the #2 port void. Provide a gauging report of the corroded side longitudinal or submit a repair proposal for the replacement of the longitudinal to the satisfaction of the attending USCG MI prior to the vessel receiving CG ISE credit. 46 CFR 92.01-10(a)

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front area of the lifejacket. 09 of the onboard lifejackets have lights that are expired. 46 CFR 199.70

All electrical engineering details and installations shall be designed and installed in accordance with sub chapter J (Electrical Engineering) of this chapter. Multiple dead end wires were found above upper generator and in the below deck storage compartment. Remove all dead end wiring. 46 CFR 90.25-1

When musters are held, details of abandon-ship drills, fire drills, drills of other lifesaving appliances, and onboard training must be recorded in the vessel's official logbook. Vessel has no record of fire or abandon ship drills. 46 CFR 199.180(h)

Pressure vessels that are required by 54.10-3 to be stamped must have the following information, manufacturer's name and serial number, maximum allowable working pressure, class, etc. The fwd and aft deck winch as well as the crane 2nd deck pressure vessels had data plates that were unreadable.

All marine engineering details such as piping, ..., shall be designed, constructed, and installed in accordance with the provision of subchapter F (Marine Engineering) of this chapter. The generator exhaust piping in the overhead of the downstairs forward storage reached a temperature of 250 degrees F while inspector was onboard with possible ignition material (cardboard boxes, absorbent pads, etc.) within close proximity.

All marine engineering details such as piping, ..., shall be designed, constructed, and installed in accordance with the provision of subchapter F (Marine Engineering) of this chapter. In the downstairs starboard aft storage there were 2 heating elements installed in the overhead with possible ignition material (cardboard boxes, absorbent pads, etc.) within close proximity.

Each vessel must carry lifebuoys approved under approval series 160.150. The forward lifebuoy has a deteriorated plug and no longer meets the Q spec approval.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. The salt water pump does not have a guard in place.

The type of piping joint used shall be suitable for the design conditions and shall be selected with consideration of joint tightness, mechanical strength and the nature of the fluid handled. Discovered exhaust leak on the upstairs generator. Demonstrate proper operation of exhaust piping under normal working conditions. Do not operate engine until corrected.

The use of oil-level gauges with flat glasses and self-closing valves between the gauges and fuel tanks is acceptable. Plastic guage was found installed on lube oil tank and waste oil tank. Install glass or other type of guage glass that will not melt in the case of a fire.

Replace damaged electrical cable for lower machinery space generator.

Replace blade type fuse with proper UL listed cartridge type fuse.

Replace bare unprotected wire with proper electrical cable.

Ensure splices are made in a junction box and support cables with metallic clamps every 24".

Provide data sheets for new machinery installations. Provide information for hoses, valves, and piping. Machinery located on the crane machinery deck to include new generators.

Prove proper operation of relief valves for air receivers.

Provide valid Loadline Certificate that corresponds with the markings on the port and stbd side of the vessel.

Provide 08 lifejackets for use on approved route.

Provide new batteries and corrosion resistant clip for water light attached to ring buoy.

Service and demonstrate proper operation of fuel shutoff valve for fuel tank located centerline of the barge.

Replace wiring and connections in crane operator space. Electrical connections must comply with 46 CFR Subchapter J.

Repair crane winch engine exhaust leak. 1" hole found on muffler.

Re-install missing exhaust lagging on exhaust side of crane prime mover turbo charger

REPLACE THE TRIPPED BRACKET THAT TIES THE 1ST SIDE LONGITUDINAL FROM THE OVERHEAD TO THE BULKHEAD IN THE #3S VOID.

CROP AND RENEW WHERE BUCKLED, THE 1ST VERTICAL SIDE STIFFENER FM THE FWD BULKHEAD BETWEEN THE OVERHEAD AND THE 1ST SIDE LONGITUDINAL FROM_x001C_ THE OVERHEAD IN # 3S VOID.

In # 2 S, crop and renew where buckled, the 1st vertical side stiffener from the forward bulkhead between side longitudinals # 1 & # 2 from the overhead.

In No. 2 S, arc out and reweld the fracture weld located where the 2nd sidelongitudinal from the overhead passes through the 2nd vertical side stiffener from the forward bulkhead.

IN THE AFT RAKE STBD COMPARTMENT NEAR THE FWD BULKHEAD, ARC OUT AND REWELD THE FRACTURED WELD THAT TIES A 2' PIECE OF ANGLE IRON TO THE 1ST UNDER DECKLONGITUDINAL FROM THE SIDE SHELL. WELDMENT IS FRACTURED WHERE ANGLE TIES TOSIDE SHELL._x001C_

FABRICATE AND INSTALL A METAL COVER FOR THE DECK WINCH ENGINE LOCATED ON THE MAIN DECK PORT SIDE FWD.

REMOVE FUEL OIL IN CONTAINMENT AREA LOCATED ON FWD END OF CRANE ROUND HOUSE.

REPAIR LEAKS IN ROOF OVER CRANE GENERATOR SPACE.

REPLACE THE CONTROL CIRCUITRY ELECTRICAL BOXES ON THE ELECTRIC GENERATOR INTHE CRANE GENERATOR SPACE.

ARC OUT AND REWELD THE FRACTURED WELD LOCATED BETWEEN THE 1ST AND 2ND VERTICAL SIDE STIFFENERS FM THE FWD BULKHEAD WHERE THE SIDE SHELL TIES TO THE MAIN DECK IN # 3 P VOID.

BUILD UP THE INSIDE COAMINGS ON THE ACCESS HOLES FOR #1C, #2P, #3P, AND THEAFT RAKE STBD COMPARTMENT. BUILD UP WITH WELD METAL, GRIND SMOOTH, AND RETHREAD BOLT HOLES AS NEEDED.

RENEW THE TOP LADDER RUNG ON THE LADDER LEADING DOWN TO #1C.

PROVIDE ACCESS TO THE LOWER FRAMES IN THE LOCKER ROOM, TO ALLOW FOR COAST GUARD INSPECTION.

RENEW THE TOP 2 LADDER RUNGS ON THE LADDER LEADING DOWN INTO #3S.

CROP AND RENEW THE CONTAINMENT AREA AROUND THE STBD FUEL FILL AND VENT VALVE. EFFECT TEMPORARY REPAIRS UNTIL PERMANENT REPAIRS ARE MADE._x001C_ _x001C_ _x001C_

REMOVE PART OF THE WOOD FLOORING THAT COVERS THE FRAMES IN THE MACHINERY SPACES TO ALLOW FOR THE INSPECTION OF THE LOWER FRAMES.

REPAIR THE MSD AND THE TOILET AND PROVE OPERATIONAL.

PREPARE FOR CLOSER EXAMINATION, THE RAISED AREA ON THE DECK JUST AFT OF THE3RD VERTICAL SIDE STIFFENER FROM THE FWD BULKHEAD IN THE #1S VOID.

PLACE CURRENT LOADLINE CERTIFICATE ON BOARD VESSEL.

PROVE PROPER OPERATION OF GENERATOR PRIME MOVER TRIPS, ON BOTH GENERATOR SETS.

PROVE PROPER OPERATION OF THE SHIPS SERVICE GENERATOR SAFETY TRIPS.

PROPERLY MARK CRANE WITH THE SAFE WORKING LOAD IN ACCORDANCE WITH 46 CFR 91.37-45.

REPAIR(WELD) THE FRACTURED VERTICAL STIFFNER(#4 FROM STARBOARD) IN THE AFTER CENTER RAKE AT FRAME 15.

CROP AND RENEW A 20"X60" BUCKLED SECTION OF TRANSVERSE DEEP WEB FRAME #15, OUTBOARD STARBOARD RAKE._x001C_

LABEL THE WATER TIGHT DOOR IN THE 2ND TRANSVERSE BULKHEAD FROM THE STERN LEADING TO THE GENERATOR AND OIL STORAGE TANK TO READ, " TO BE CLOSED AND DOGGED WHEN VESSEL IS UNDERWAY."

INSTALL 30X30 MESH FLAME SCREENS ON THE TWO CRANE DAY TANKS, AND THE MAIN FUEL TANK VENT AND 20X20 MESH ON THE MSD TANK VENT.

REMOVE THE SHORE TIE CONNECTION FROM THE DISTRIBUTION PANEL OR PROVIDE A MEANS/SAFETY INTERLOCK TO PREVENT SHORE TIE FROM BEING PLACED ON THE SYSTEMWHEN THE GENERATOR IS ON THE LINE.

INSTALL BLANK ON THE 1/2 INCH SEA VALVE WHICH IS NO LONGER IN USE ON THE STBD SIDE SEA CHEST.

PROVIDE A PROTECTIVE COVER OVER THE BATTERY TERMINALS.

PROPERLY INSTALL GENERATOR METERS TO PREVENT ELECTRICAL SHOCK.

OBTAIN VALID LOADLINE CERTIFICATE.

INSTALL GUARD AT PUMP MOTOR COUPLING LDFT SIDE ON CRANE PRIME MOVER.

PROVIDE LIGHTS AND WHISTLES FOR LIFEJACKETS.

INSTALL GLASS GLOBES ON ALL INCANDESCENT LIGHT FIXTURES.

MAKE PRESSURE VESSELS AVAILABLE FOR INTERNAL INSPECTION OR HYDROSTATIC TEST.
