Before the ship leaves port and at all times during the voyage all life saving appliances shall be in working order. While retrieving the rescue boat into the cradle, PSCO observed the port quarter strike the had rail leading to damage in the gel coat exposing fiberglass. This is a recurring issue that has not been addressed. Additionally the keel showed the same type of wear from making contact with the cradle. Additionally port side life lines on rescue boat were detached at various anchor points - 74 SOLAS(2014) III/20.2 - 16BC

Resolution: PSCO attended vessel and observed rescue boat was renewed with new paint and gelcoat finish. Additionally permeant hand rail was relocated and boats cradle was repaired eliminating the wear on the rescue boat hull. Deficiency Cleared
Oil filtering equipment shall be of a design approved by the administration and shall be as such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil conten t not exceeding 15 PPM. During test of OWS, PSCO witnessed the failure of the overboard discharge pipe and were unable to verify the systems ability to ensure that any mixture discharged would not exceed 15 PPM.
