Each dead cover must be designed so that it can be secured watertight. Vessel is outfitted with wooden dead covers (Allowed by CG Letter Dated 06DEC1983) only to be used aft of the cargo doors. Vessel was found using wooden covers on the cargo doors. Vessel to replace wooden dead covers with suitable materials, to the satisfaction of the OCMI, prior to 2027 operating season. 46 CFR 171.117(d)1

The two required fixed power bilge pumps shall be self-priming. MI observed vessel unable to adequately prime aft bilge pump. 46 CFR 182.225-10 (c)

Where liquid level indicating gages are installed, they shall be of heat resistant materials, adequately protected from mechanical damage and provided at the tank connections with devices which will automatically close in the event of rupture of the gage or gage lines. MI discovered a plastic sight gage on the fuel return tank (Port Forward). 46 CFR 182.20-25

All electrical installations shall be to the satisfaction of the Officer In Charge, Marine Inspection. MI discovered dead end wires in the lazarette and BOSN stores, fire pump breaker was oversized, missing plus in MSD breaker panel and wires pulled out of stuffing tubes on the MSD controller. 46 CFR 183.01-15(a)

MAIN ENGINE EXHAUST COOLING PUMP FAILED TO PROVIDE COOLING WATER. PUMP IMPELLER FAILED.

An A-class fire door may not have an undercut greater than 1/2 inch. The double-leaf fire door located on the forward side of the third deck stairtower has a gap that exceeds this limit. Adjust the diamond plating so that the undercut for each door leaf does not exceed 1/2 inch.

At each inspection for certification, the vessel must be fully prepared for inspection and testing of fire protection equipment, including fire-confining appliances such as doors. The A-class fire door between the stairtower and lobby on Deck 1, near the purser’s office, is required to be self-closing per the vessel’s FCP. The door’s self-closing device currently makes contact with a smoke detector mounted in the false overhead.

The master is required to post a passenger safety bill in each passenger cabin, stateroom, and accommodation space. Each safety bill must indicate the assigned embarkation station and the number and location of the survival craft assigned to each occupant. The current passenger safety bills posted on the back of each stateroom door list muster stations but do not include embarkation station designations or survival craft assignments. Provide a means to ensure each passenger is notified of their specific embarkation station and assigned survival craft. A trip-specific addendum is acceptable to meet this requirement.

At each inspection for certification the owner or managing operator shall make available for inspection all items in the ship’s outfit, such as navigation equipment. Two GPS units on the bridge have experienced GPS rollover and no longer display the correct date, which affects the positional accuracy provided by the units to the electronic charting and/or VHF-DSC. Repair or replace. Prior to 2026 operating season.

Vessels described by 46 CFR 114.110(e) must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Discrepancies were noted between the approved fire detection plan (dated 01/24/2025, MSC letter E2-2500916) and the onboard installation: Heat detectors in the galley and laundry are Autronica BD-501, but plans specify BD-300. All heat detectors should be verified for correct part numbers. The sea cabin is shown with a V-430 smoke detector with sounder (“S” symbol), but a standard optical head (“B” symbol) without sounder is installed. Ensure onboard installations match approved plans. Submit revised drawings to MSC if changes are necessary. Prior to 2026 Operating season.

A minimum clear opening of a door or passageway used as a means of escape must not be less than 32 inches in width. Aft exterior stairwell from 4th deck leading down to 3rd deck is the secondary means of escape for passengers from the spa/workout area, but is only 18 inches wide. Provide a proposal to the OCMI on modifications to this stairwell to meet the relevant means of escape requirements from new K. Prior to 2026 operating season.

The following discrepancies were identified in the vessel’s FCP dated 5/5/2025: 1) FCP shows an A-class bulkhead between the sea cabin and pilothouse, but there is an approx. 3 ft x 3 ft opening in the bulkhead, 2) FCP indicates a General Alarm and Talkback Speaker in the captain’s cabin (Deck 3), but only a standard PA speaker is installed, and 3) Only port/starboard amidships cargo doors are marked as embarkation stations, but the aft swim platform is also used as a primary embarkation station. Update the FCP to reflect actual vessel arrangements, including the additional corrections already/previously identified by the crew. Submit the revised plan to Sector Puget Sound for verification/approval. Prior to 2026 operating season.

At each inspection for certification all fire hazards must be corrected. Passageways (i.e. corridors) shall contain only fire resistant furnishings. The following structural fire protection and arrangement issues were identified within category (3) corridors: 1) there is only a wooden accordion style partition separating the mechanical closet on third deck forward from the adjacent corridor, near the Captain's stateroom. The items in the mechanicals closet are not permitted in corridors, and therefore the class A bulkhead shown on the FCP must be maintained in the door penetration, 2) there is a wooden cabinet with linen stores in the third deck forward corridor outside the captain’s stateroom, and 3) there is a wooden cabinet at the forward end of the second deck, starboard side, corridor outside room 201. These current arrangements present an unacceptable fire load in the category 3 corridors. Provide a plan to the OCMI for how these issues will be addressed prior to the 2026 season.

Repairs to the hull must not be initiated without prior approval from the cognizant Officer in Charge, Marine Inspection (OCMI).During the underway portion of the Certificate of Inspection (COI), the vessel made contact with a moored fishing vessel, resulting in damage to the starboard-side gunwale and third-deck railing. The owner/operator is required to submit a detailed repair proposal to the OCMI for review and approval prior to beginning any corrective work.

In areas with fire resistant furniture and furnishings on passenger vessels, the carpet must either be wool, or other materials having equivalent fire resistant properties. Provide evidence that newly installed carpet in corridors and stairways is fire-resistant (NVIC 9-97 CH1, Enclosure 1, Section 2.8.1 lays out 5 different acceptable test criteria) 116.423 (a)(4) and (b).

On vessels required to have a final emergency power source by § 112.05-5(a) of this chapter, the following emergency lighting and power loads must be arranged so that they can be energized from the final emergency power source. Vessel’s emergency generator was inoperable during inspection.

All glass panels or windows located in the first tier of the superstructure or deckhouse above the bulkhead deck are considered potential downflooding points during intact stability evaluations, regardless of the vessel’s operating route. Provide deadlight covers for all windows in the first tier of the superstructure, as specified in Section 5 of the vessel’s stability letter.

Penetrations of watertight bulkheads by a rigid plastic pipe system is prohibited unless, each penetration is accomplished using an acceptable metallic fitting and an acceptable metallic shutoff valve is installed adjacent to the bulkhead. Only one valve is required if operable from above the bulkhead deck. If not, two valves are required (one on either side of the bulkhead). Two bulkhead penetrations on the starboard side of the forward engine room bulkhead are of the rigid plastic type. Cite: 46 CFR 182.40-1(a)(1)

Where rigid nonmetallic piping is permitted, the material specification shall show that the rigid nonmetallic material possesses characteristics adequate for its intended service and environment and shall be approved for use by the OCMI. The nonmetallic flange connection for the MSD discharge piping through hull penetration does not meet OCMI approval. Cite: 46 CFR 182.40-1(a)(4)

Machinery spaces shall be separated from accommodation spaces by a watertight and/or vapor tight bulkhead. One bulkhead penetration on the starboard side of the forward engine room bulkhead is loose and does not meet watertight requirements. Bulkhead does not meet requirements for structural fire protection. Cite: 46 CFR 182.35-1(a)

Exposed, non-current carrying metal parts of fixed equipment that may become energized because of any condition must be grounded. Both port and starboard shaft grounding straps are not connected.

Bilge and drainage system shall provide means for testing pumping of bilges. Bilges pumping/drainage system is unable to be tested due to large amount of debris. Cite: 46 CFR 176.25-10(a)(7)

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] found a soft patch on the seawater pump for the MSD. Make permanent repairs. 46 CFR 119.700

Multiple self-closing fire doors were not closing properly as required. Adjust/repair both sets of main deck fire doors in vicinity of lounge and bar. 46 CFR 116.435

Each distribution panel must be totally enclosed and of the dead front type. The hinged door for the galley stores panel did not close properly preventing total enclosure. 46 CFR 120.330(b)

All mattresses must comply with either 1. the U.S. Department of Commerce Standard for Mattress Flammability (16 CFR Part 1632), Subpart A and not contain polyurethane foam; or 2. International Maritime Organization Resolution A.688(17) "Fire Test Procedures for Ignitability of Bedding Components." Mattresses tested to this standard may contain polyurethane foam. Provide approval specifications to marine inspector within 14 days. 46 CFR 116.405(j)

Rugs and carpets are not permitted in areas where the spillage or leakage of flammable or combustible liquids is possible including areas immediately adjacent to bar service areas. Rug runs directly next to the bar. Provide plans to meet 46 CFR 116.423(a)(4)(ii) within 30 days.

Each life jacket carried on a vessel on a coastwise route must have a life jacket light approved IAW Subpart 161.012. Several life jacket light batteries were not being replaced annually and one had a light that did not operate. Batteries replaced and labeled. 46 CFR 117.75(a)

One set of self-closing fire doors on deck 3 did not close properly. The wheel was removed from the base. Attending MI witnessed proper closure. 46 CFR 116.435

Protect passengers, crew, other persons and the vessel from electrical hazards including fire caused by or originating in electrical equipment. Unattended items being charges and multi-outlet device plugged into single outlet in crew, focsle berthing. Chief mate unplugged devices and will conduct training with crewmembers on electrical safety. 46 CFR 120.200(b)

Protect passengers and crew from electrical hazards. Dead-end wires were found in the engine room and fwd crew berthing area. Properly remove dead-end wires. 46 CFR 120.200

The main and emergency switchboard were both missing a lens covering a light bulb on the front of the panel. Install lens cover over the openings on the panel. Protect passengers and crew from electrical hazards. 46 CFR 120.200

The power cord for the dryer, in the main deck laundry, was pulled out of stuffing tube, exposing bare conductors. Properly re-install power cord. 46 CFR 120.200

All cable and wire must be installed with metal supports spaced not more than 24" apart. Several wires and wire runs in the engine run were only supported by plastic wire ties. Ensure all wire runs throughout vessel are supported by metallic supports. 46 CFR 120.340

Sightglass for the daytank in the engine room is non heat resistant. Meet requirements listed in 46 CFR 119.

Waste receptacle on the starboard bow is constructed of combustible material and is open on the bottom. Remove garbage can from the vessel. 46 CFR 116.405

The following spaces must be equipped with a fire detection and alarm system of an approved type: a space containing propulsion machinery and a space containing an internal combusion engine of more than 50 hp. Each accommodation space, control space and service space must be fitted with a smoke actuated fire detection system of type approved by the Commandant and a manual alarm system that meets the requirements in 46 CFR part 76. Fire detection system was reported inoperable. Prove proper operation to the attending marine inspector.

Multiple exposed electrical hazards must be corrected: Thermostat and heating element in fwd crew berthing, 110V distribution panel opening in office fwd of galley. Ensure electrical hazards are repaired.

Vessel did not have nor could display "Not Under Command" with lights at night time. Procure/install NUC nav lights to display as required.

Multiple fire door self-closing devices were not closing properly as required. Adjust/repair doors to close and latch as required.

Vessel did not have a waste management plan posted. Post vessel's waste management plan.

Vessel did not have emergency instructions posted. Post emergency instructions.

Restore fire control boundaries in fwd and inboard bulkhead in laundry space and deck going to the tank space.

Repair leaking salt water pipe in tank space on starboard side heading to the stateroom above.

Adjust dogs on water tight door to ensure all are making contact with jam.

Repair exhaust leak on both main engines and the fwd Generator exhaust piping.

Repair water leak in aft generator stainless steel exhaust pipe at overboard starboard side.

Replace all leaking salt water cool pipe for aft generator in the area of starboard side where it attaches to exhaust pipe at is leaking. Currently there are two patches and other leaks.

Repair leaking salt water exhaust cooling line on port main by overboard exhaust pipe.

Repair all leaking piping for the sewage water flushing system on the port side of the engine room fwd.

Repair electrical conduit wire plug on the bottom of Engine room fire pump controller box on the port side fwd engine room.

Replace all expired items in both the laundry room and galley First aid kit and ensure there is the Min. contents of 46 CFR 160.041-4 are in them.

Replaced all expired eye wash bottles in the galley and main passage way.

Replace expired adult epinephrine injection pen

Secure washer and dryer to prevent them from moving and falling over.

Provide wire support/chaffing protection for wire running into fwd. lower hot water heater by crew berthing.

Remove all flammable items stored in the bouson shop or provide a flammable storage locker.

A-Class bulkheads must be capable of preventing the passage of smoke and flame for 1 hour. The forward double doors on the 300 deck stairway have a large gap at the base of the door and the deck.

A fire door in a A-class boundary must be of the self-closing type. Purser door leading to the alcove is not a self-closing type.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Conduct a fire drill to the satisfaction of the attending Marine Inspector.

The vessel is required to have an 2 power bilge pumps connected to a bilge suction line. The emergency bilge pump could not draw suction.

Remove all dead-end/open wires from the lazarette.

Locate water leak and repair of deck in way of aft, 300 deck and passageway below as marked.

Replace all combustible trash receptacles.

Replace two torn lifejackets. Until replaced passenger count will be reduced by two passengers.

Install guards electrical Switchboards to prevent contacting energized parts in galley.

Install guards on all rotating machinery on both main engines.

Fix water leaks on both main engines and generator exhaust cooling system.

Remove temporary electrical cord from refrigerator and properly wire as per 46 CFR 116.530

Provide update temporary fire control plan before carrying passenger and provide final fire control plan for approval in one month

Hydro test or renew with tested hoses all high pressure flex loops and one 75 lb high pressure CO2 bottle that expired on 05/16. Hoses to replaced prior to carrying passengers and hydro of bottle to be completed at the next port call to Juneau, AK or 7 May 2016.
