To maintain a valid Certificate of Inspection, the operator must complete the annual inspection within the periods specified in § 176.500(b)(1) and the Certificate of Inspection must be endorsed. The new owner/operator failed to complete an annual inspection within the 90 day annual window. The vessel's 90 day annual window expired on August 9, 2026. The COI was voided on this date. 46 CFR 176.502

The owner or managing operator shall make a vessel available for drydock examinations, internal structural examinations, and underwater surveys (UWILD) required by this section. A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel is due for a hull examination no later than April 30, 2026. Issued 01 CG-835 restricting passenger operations until a hull examination is completed. 46 CFR § 176.600

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: The pump discharge line is suitably supported and a positive shutoff valve is installed at the hull penetration. The discharge piping on the stbd engine compartment bilge pump and lazarette bilge pump was found disconnected at the pump. The port and stbd engine compartment bilge piping is also missing a positive shut off valve at the skin of the vessel. 46 CFR 182.520

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge; and (3) A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. The port and stbd main engine pre-engineered fixed gas fire extinguishing systems were tested and the marine inspector found that the audible alarm and means to reset devices used to shut down the engine are inoperable. 46 CFR 181.420

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The port engine compartment, protected by a fixed gas extinguishing system, has one inoperable ventilation closure located aft of the space. The closure is frozen open. 46 CFR 182.465

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The visual indicator for the port side engine compartment bilge pump is inoperable. 46 CFR 182.530

Electrical equipment on a vessel must be installed and maintained to the satisfaction of the OCMI. The port and starboard main engine starting electrical conductors were found installed in the bilge and unsupported. 46 CFR 183

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Electrical conductors in the lazarette and engine compartments were found with screw-type wire connectors. These conductors are required to have a pressure type connector and shall be made watertight using a shrink wrap. 46 CFR 183.340

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. The stbd main engine battery tray is damaged and the battery is not secured from movement. 46 CFR 183.350

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. The port side navigation light housing was found painted over and the housing must be replaced. 46 CFR 183.420

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The owner admitted to using an open flame grill for food service onboard to passengers. The use of an open flame creates a fire hazard onboard due to the fiberglass hull and is not permitted. 46 CFR 176.830

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Wood decay was observed on the overhead beam stbd side immediately aft of the passenger embarkation. Additional wood decay and termites were observed in the wood installed on the side shell used to support the overhead beam. Make repairs IAW good marine practice. 46 CFR 176.700

All closure devices must be readily available and mounted in the vicinity of the vent. The vertical exhaust duct discharge opening for each engine space was not modified or designed with a closing device. The crew are unable to close ventilation in the event the pre-engineered fixed fire extinguishing system is used. 46 CFR 182.465 (h)

Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Recommended Emergency Check Off List” listed in § 185.25-5. The Emergency Check Off List does not include instructions for operating the fire pump or using the fire pump. 46 CFR 185.25-1(b)

Gages to indicate cooling water discharge temperature and lube oil pressure shall be provided for all propulsion engines and located to be in view of the operator. The oil and temp gauges are inoperable on the port main engine. 46 CFR 182.15-5(b)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. A first-aid kit may be considered acceptable if it meets all of the requirements of ISO 18813:2006(E) paragraph 4.12, except that it does not contain the burn preparations. It must be clearly marked on the first-aid kit that it does not include the burn preparations. The first aid kit does not comply with the requirements of ISO 18813:2006(E). 46 CFR 184.710

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must: (1) Be buoyant; (2) Be of at least 18.3 meters (60 feet) in length; (3) Be non-kinking; (4) Have a diameter of at least 7.9 millimeters ( 5/16 inch); (5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and (6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The ring buoy's life line was less than 60'. 46 CFR 180.70(c)

Where fire extinguishing equipment are installed but not required, the system or equipment shall meet the requirements of this part. The vessel is equipped with two fire pumps that are operated off of each engine. The stbd fire pump was inoperable due to flexible piping that was too short and fell off during a pressure test of the system. The port fire pump was found to be operational. 46 CFR 181.05-5

The owner or managing operator must provide satisfactory evidence of the required servicing of the fixed fire suppression system to the attending Coast Guard marine Inspector. If any of the equipment or records have not been properly maintained, a qualified servicing may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. The vessel pre-engineered fixed fire suppression system has not been properly been maintained.

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer.

vessel port MDE tachometer is not operable, vessel rep either need to remove the tachometer or conduct permanent repairs to bring the tachometer back in operation.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years.

Vessel new crew was not properly screened for drugs prior to hiring.

The port window was damaged, vessel rep needs to replace window.

Vessel rep had an expired COD, vessel must have a valid COD onboard or have the COD be deactivated and restrict the vessel to operate only within CNMI waters.

Vessel rep need to provide a crew that is read to conduct fire and MOB drills.

Vessel must have up-to-date charts for intended voyage as required by 46 CFR 185.20-35 (Old T). Provide proof of Charts published by NOAA or NOS for Tanapag harbor.

Deck light must be in serviceable conditions as required by 46 CFR 183.05-50 (Old T). Repair burnt out deck light on main deck and upper deck.

Vessel shall be equipped with suitable number of portable battery lights as required by 46 CFR 184.30-1 (Old T). Provide 2 portable flash lights.

All vessel shall carry pyrotechnic distress signal as required by 46 CFR 180.35-1 (Old T). Provide (03) red flares and (03) orange smoke with unexpired dates.

All Fire protection equipments must be serviced annual as required by NFPA. Provide proof of annual services for Portable CO2s and pre-engineered FGFE system.

Water light on ring life buoy must be maintained and and in good conditions as required by 46 CFR 180.30-1. Install approved missing battery on ring life buoy light.

Mounting bracket must be serviceable conditions as required by NFPA. Repair rusted & wasted mounting bracket for all portable CO2.

Vessel must have visual and audible high water level alarm as required by 46 CFR 182.530 (New T). show proper operation of bilge alarm for starboard engine room and #3 void port side.

All vessel must have navigation lights that are in compliance with Int'l & Inland navigation rules as required by 46 CFR 183.420. Repair inoperable STBD side (Green) light.

Fuel shutoff valve must be installed in the supply line as required by 46 CFR 182.455. Repair broken remote shutoff line.

Exterior light fixture must be guard or be made of higher strength material as required by 46 CFR 183.410. Repair damage upper deck light forward portside.

Vessel operates in lakes, bays and sounds must have (03) red flares and (03) orange smoke as required by 46 CFR 180.68. Replace expired (03) red flares.

Fixed gas fire equipment must be service annually as required by 46 CFR 176.810 and NFPA. Provide proof of annual service conducted on FGFE system.

Ring life buoy must have water light and lanyard with corrosion resistant clip as required by 46 CFR 180.70. Replace undersize metallic clip with proper size.

Vessel must carry first-aid-kits as required by 46 CFR 184.704. Replace expired eye wash in the first-aid-kit.

Vessel must have at least (02) portable battery lights as required by 46 CFR 183.430. Provide (01) missing portable battery light on board.

Vessel must carry adequate and up-to-date charts and nautical publication as required by 46 CFR 184.420. Provide copy of the following publications on board: U.S. COAST PILOT, COAST GUARD LIGHT LIST, CHARTS, CURRENT TABLE, TIDE TABLE and NOTICE TO MARINERS.
