Except under paragraph (a)(2) of this section, vessels that operate in salt water must be examined in accordance with the intervals set forth in Table 91.40-3(a)of this section. Where Table 91.40-3(a) indicates a 2.5 year examinations interval, its means a vessel must undergo two examinations within any five year period. No more than three years may elapse between any two examinations. Vessel shall complete a credit dry-dock and internal examination prior to movement.

Each cargo vessel must carry at least one rescue boat. The vessel does not have a rescue boat or launching appliance that meet the requirement of 46 CFR 199.262. USACE shall ensure a rescue boat and launching appliance is installed meeting the above reference or provide a letter IAW the MOU that the USACE District Commander has made a decision to allow the vessel to operate with its current rescue boat configuration, which is not in compliance with the applicable regulation.

If, during an internal structural damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the OCMI may require the vessel to be drydocked or taken out of service to further assess damage and to effect permanent repairs. Vessel must remove Temporary repair in #3 Stbd Void slope Weld seam leak, to include aft doubler.

Wasted Hull plating in #2 Void must be repaired.

If, during an internal structural damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the OCMI may require the vessel to be drydocked or taken out of service to further assess damage and to effect permanent repairs. Port side #3 void fuel tank deck insert must be repaired.

If, during an internal structural damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the OCMI may require the vessel to be drydocked or taken out of service to further assess damage and to effect permanent repairs.Wasted dredge collection pipes in the #5 port and stbd void must be repaired.

Failed hose test hatch repairs must be made and NDT'd.

Vessels in ocean, coastwise, or Great Lakes service, except unmanned barges, shall be fitted with anchors, chains, and hawsers in general agreement with the Standards established by the American Bureau of Shipping. Starboard windless motor brake failed and the drum clutch casing structurally failed during anchor deployment. This event rendered that system incapable of raising the starboard anchor during mooring or anchoring operations. Repair starboard windless system, replace clutch and test system to the satisfaction of the attending USCG Marine Inspector. Also have the Portside windless clutch casing non destructively tested to ensure casing is not structurally compromised or damaged.

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. (b) Means shall be provided for stopping all fans in ventilation systems serving machinery and cargo spaces and for closing all doorways, ventilators and annular spaces around funnels and other openings to such spaces, from outside these spaces, in case of fire. Replace or repair inoperable engine room supply fan motor to the satisfaction of th attending USCG Marine Inspector.

Electric installations on vessels must ensure: (1) Maintenance of services necessary for safety under normal and emergency conditions. (2) Protection of passengers, crew, other persons, and the vessel from electrical hazards. (3) Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. Properly tag out electrical power switch and secure exposed dead end wires to previously installed space vent fan motor.

Arrangements shall be made whereby water in the compartments will drain to the suction pipes. Efficient means shall be provided for draining water from all tank tops, other watertight flats and insulated holds. Peak tanks, chain lockers and decks over peak tanks may be drained by eductors, ejectors, or hand pumps. Provide repair plan detailing repairs to, non functioning, fore and aft peak tank de-watering system remote control valves.

Ships port davit failed to lift lifeboat after man overboard drill. Davit shall be in proper operation at all times, show proper operation in the presence of Marine inspector.

Ships rudder angle indicator is not properly indicating at the local operating station and the Bridge. Show proper operation of the rudder angle indicator and steering gear. Show proper operation in the presence of Marine inspector.

Each cargo vessel must carry at least one rescue boat. Vessel's crew stated that they do not have a designated rescue boat. Provide proof of exemption or alternative to the regulations.

Fuel oil purifier has leaking hoses. Repair leaks to the satisfaction of attending Marine Inspector.

Marine cable transit in the Engine Room had cable pulled and not blocked with appropriate blank. Replace with blank or reinstall cable to make water and vapor tight.

Starboard steering gear hydraulic hose has damage exposing the interior braided metal. Repair/replace hose to the satisfaction of attending Marine Inspector.

Flexible hoses attached to two of the CO2 bottles in the emergency generator space have hoses that are exhibiting dry-rot and kinked. Provide servicing/repair report from an approved fire servicing company to the satisfaction of a Marine Inspector.

Port steering motor inop.

Fire Dampers located on exhaust stack for machinery space pinned-open and prevent closure of space for fire protection. Provide safe/efficient means of closure and/or demonstrate satisfactory operation of protective fire dampers in presence of CG Marine Inspector. 46 CFR 92.15-10(b)

01) Currently the EDG shuts down on high J/W temp. 46CFR 112.50(g) states alarm only not shutdown. Correct issue so the EDG does not shut down on high J/W temp. 30 Day Compliance date (15Sep2019) to the satisfaction of the Coast Guard.

02)Currently the EDG has no Audible alarm in the EDG room, nor in the engine control room. IAW 46CFR 112.50(h). Until the issue is resolved when the EDG is run, the EDG room must be manned by a qualified engineer. 30 Day Compliance date (15Sep2019) to the satisfaction of the Coast Guard.

03)Currently the EDG PTSP states “Check Valve to Remain Closed” for the L/O testing. This check valve cannot be found. Correct the procedures or find the check valve. 30 Day Compliance Date (15Sep2019) to the satisfaction of the Coast Guard.

04)Currently the EDG L/O test did not go IAW the procedures, no Low L/O alarm happened only the shutdown did and that was above where the low alarm should have gone off. Correct the EDG alarm and shutdown for the low L/O. 30 Day Compliance Date (15Sep2019) to the satisfaction of the Coast Guard.

05)Currently at the EDG CO2 release station, there is no sign to state there are manual closures to be closed when activating the CO2 system. Install signage to include these closures. 46CFR95.15-10(h). This has been rectified prior to completion of today’s inspection. Issued and Cleared same date.

06)This goes along with deficiency number 2 issued today. In the engine control room there is in the view screen the previous EDG’s alarm system and controls, the current new EDG is not installed there. Only lacking a computer code to get it re installed. Correct this monitoring issue so alarms work in the control room. 46CFR112.50(h) 30 Day Compliance Date (15Sep2019) to the satisfaction of the Coast Guard.

07)Currently the PTSP’s for the SS/DG’s are not complete IAW 46CFR61.40-10. The step by step instructions are missing. Correct this part of the PTSP’s for the ships SS/DG’s. 30 Day Compliance Date (15Sep2019) to the satisfaction of the Coast Guard.

08)Currently on the STBD SS/DG there is a F/O leak at a F/O high pressure injection line at the #4 inboard head. Correct this issue by the end of today’s inspection. This has been corrected. Issued and cleared same day.

09)Currently fire stations #29 and #35 are in need of PMS, green corrosion covers the blank cap on the “Y” gate and the main stop valves for the stations. Correct before the end of today’s inspection. This has been corrected. Issued and cleared same day.

10)Currently the Galley roll top door will not close completely. This fire door must close completely. Correct issue with the door. The ¾” to 1” gap at the bottom needs to be filled. 30 Day compliance Date (15Sep2019) to research possible new door. To the satisfaction of the Coast Guard.

11)Currently the following deck penetrations are wasted and in serious need of replacement. Cable penetrations boxes PORT side main deck number F2, F3, and the PORT Trunnion winch penetrations. STBD side the F4, F5, F6, and the STBD Trunnion winch penetrations, crop and renew, replace all. 46CFR171.111(e). This item is to be completed at the scheduled shipyard period in September 2019. 90 Day Compliance date (15Nov2019) To the satisfaction of the Coast Guard.

12)Currently the High L/O temp alarm on the Port SS/DG is not working, correct problem and retest. Due to the constantly manned engine room a 30 Day Compliance Date (15Sep2019) To the satisfaction of the Coast Guard.

13)Currently the Center SS/DG’s reverse power relay is not working. Repair and call USCG to witness testing. This repair is required to be completed prior to sailing. A 30 Day Compliance Date (15Sep2019) to the Satisfaction of the Coast Guard.

14)Currently in the AMR upper FWD BLKHD Rox Box has been repaired with in correct rubber blocks, miss matched manufactures. Correct the previous repairs to the box and use correct manufactures blocks. A 30 Day Compliance Date (15Sep2019) to the satisfaction of the Coast Guard. 46CRF171.111(e)

15)Currently FWD Generator room BLKHD penetrations (Stuffing Tube Type) not securing wires in the penetrations and also the wires at the same location are not properly attached to the BLKHD. A 30 Day Compliance Date (15Sep2019) to the satisfaction of the Coast Guard. 46CFR171.111(e).

16)Currently in the AMR STBD BLKHD a Fixed CO2 system nozzle is being blocked by boxes, Remove the obstruction prior to the close of today’s inspection. This deficiency has been corrected prior to the end of today’s inspection. Issued and cleared the same day.

Prior to departure from Sector Mobile Zone, provide a technician report from SPERRY and CAPT conducting steering test under load while underway.

Prior to departure from port, vessel is required to provide tug assist.

Make permanent repairs to the main cable transit in the emergency generator room on the bulkhead behind the switchboard to the satisfaction of the attending Coast Guard Marine Inspector.

All leaks in the vessel shall be repaired and tested to the satisfaction of the attending marine inspector prior to movement. Vessel shall notify USCG Sector Mobile prior to any movemnet outside of the shipyard. Master shall ensure the safety of the vessel is maintained at all times. IAW 46 CFR 91.40-3

Provide annual servicing report for fixed CO2 system.

Provide GMDSS and Radio annual servicing report.

Vessel reported inoperative OWS, make proper repairs to installed OWS.

Replace damaged cotter pin on turnbuckle on starboard lifeboat aft.

Vessels helo deck feeder tubes were damaged.

Starboard trunnion service frame was damaged. Crop and renew inkind.
