Electrical installations should be such as to minimize the risk of fire and explosion from flammable products. Four explosion proof lights were found with cracked lenses in the cargo compressor and one above the starboard compressor. 74 SOLAS (20) VII/13.1 - IGC (93) 10.1.2

Gas Detection - When such equipment is located in a gas - safe space the following conditions should be met: Exhaust gas from the detector should be discharged to the atmosphere in a safe location. The gas-tight steel cabinet that houses the fixed gas-sampling instruments has over 30 holes drilled into the door. 74 SOLAS (20) VII/13.1-IGC(93)13.6.5.2

Decontamination shower/eyewash - The showers and eyewash should be operable in all ambient conditions. The STBD eyewash was clogged, and the port station did not have water available. 74 SOLAS (20) VII/13.1 - ICG(93) 14.4.3

The fire resistance of doors shall, as far as practicable, be equivalent to that of the division in which they are fitted. Multiple fire doors in the accommodation space were not marked with the fire rating, and others showed signs of on board repair. Class should review all fire doors to ensure they are equivalent to the division they are fitted. 74 SOLAS (97)II-2/47.1

Hot fresh water or means of heating water shall be available in all communal wash places. Hot water was tested in multiple areas around the ship and never warmed higher than room temperature. 46 USC 70002 - ILO-147 Pg. 37

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. While attempting to transit outbound from the Oxychem cargo terminal on 25Jan2022, the vessel was unable to maintain propulsion and engine control after taking in mooring lines. The onboard Pilot directed the vessel to return to Oxychem. Vessel is required to conduct a satisfactory power trial with class attendance and provide a class report to ensure proper operation of all propulsion machinery and associated control systems. 74 SOLAS (20 Cons.) 09209 - Electrical equipment, cables and wiring should not be installed in hazardous locations unless is conforms with the standards not inferior to those acceptable to the organization. PSCO observed loose wiring on cargo tank #4 overfill sensor. In compressor room 03 gauges has loose wiring and 03 light fixtures has fractures on lenses. - 74 SOLAS (20) VII/13.1, IGC 1993 10.1.4 - 17b/c

11108 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Starboard side life raft was discovered to have cracked housing. - 74 SOLAS (97) III/19.2 - 17b/c

Each flammable gas detection system must have an audible and visual alarm for power failure and loss of gas sampling flow. Unable to test low flow alarm from sample points on fixed gas detection system. 46CFR154.1350(f) 17c

0799- On ships carrying flammable or toxic products, a water spray system for cooling, fire prevention, and crew protection should be installed to cover exposed cargo tank domes and any exposed parts of cargo tanks. 2045- Emergency Shutdown (ESD) valves in liquid piping should fully close under all service conditions within 30 seconds of activation. The STBD sdie ESD valves took 40 seconds to close and the PORT side ESD valves took 50 seconds to close. 74SOLAS(2014)VII/12...IGC Code (93ed.)5.6.4

1270- All access openings in bulkheads at ends of enclosed superstructures shall be weathertight when closed. 03 doors for cargo compressor room were found not weathertight. ICLL(88) Reg.12(1).

2550- The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requriements which may be established by the company. The vessel was required to use span gas for testing gas sample points.
