M/V INDIAN RIVER QUEEN

Vessels are required to display navigation lights for the vessels size. The stern light is inoperable. Correct prior to operating at night or in periods of restricted visibility.

Life ring buoys must be ready for deployment and suitable for service. The grab line on one buoy is wasted and retro-reflective tape is missing from two life ring buoys. Correct prior to the carriage of passengers.

The following spaces must be equipped with a fixed gas fire extinguishing system. (5) A paint locker. Vessel has manufactured paint locker in the water/fuel tank compartment aft of the engine room. Remove all paints to a proper flammable storage locker above deck or install a fixed fire extinguishing system to the satisfaction of the attending MI.

Shutoff valves, installed to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferable from an accessible position on the weather deck. The vessel had two fuel oil lines coming from the fuel tank. Once had a remote valve closure from the weather deck the other valve is missing the remote closure. Install to the satisfaction of the attending [name withheld] fuel valve must be kept in the closed position until this is corrected.

An exposed hazard, such as gears or rotating machinery, must be protected by a cover, guard or rail. The shafts in the water/fuel tank space do not currently have guards in place over them as in the liquor locker space. Install guards over the shafts to the satisfaction of the attending MI.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel has a deep sink port side aft that is not attached to the bulkhead and gives way when pulled on. Repair to the satisfaction of the attending MI.

Nonmetallic piping materials, including nonmetallic flexible hose assemblies mut meet the requirements of 46 CFR 56.60-25 in subchapter F of this chapter. The potable water supply line running through the lazarette up to the aft port side kitchenette is dripping water at the deck penetration. The piping goes from PVC to metal nipple back to PVC. Repair to the satisfaction of the attending MI.

All unfired pressure vessels must be installed to the satisfaction of the OCMI. The small air compressor in the engine room is sitting up on a shelf with no means to prevent it from falling while underway. Secure for sea to the satisfaction of the attending MI.

A suitable metal marine type strainer, meeting the requirements of the engine manufacture must be fitted in fuel supply line in the engine compartment. Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. 03 fuel oil filters in the engine room do not have flame impingement bowls on them. Add flame impingement bowls or prove the manufacture does not require them to the satisfaction of the attending MI.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol test kits were not onboard.

50% of the crew must possess valid first aid / CPR cards. First aid cards for the crew were expired.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant OCMI. The fwd and aft escape hatches in the engine room are not marked on both sides. Mark both hatches appropriately as stated above.

The following vessels must have on board a properly installed, operational Coast Guard approved AIS Class A device: A self-propelled vessel that is certificated to carry more than 150 passengers. Vessel's AIS exception has expired and vessel currently does not have a AIS installed. Install and prove proper operation of AIS Class A device.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel does not have a valid safety radio certificate. Provide valid safety radio certificate.

Demonstrate operation of electric fire pump, providing at least 50 psi with a single hydrant open.

Reduce, remove, reroute extension cords & surge protectors when re-building PA/Stereo cabinet above stairway to bridge.

Ensure fuel tank vent is installed with a flame screen.

Extend the machinery guards on each engine/rotating machinery to cover hazard adequately.

Outfit ring life buoys as prescribed on the COI; 1 w/line, 1 w/light, and 1 w/neither.

Provide current proof of enrollment in drus consortium and provide annual MIS report.

Renew grab lines on upper deck ring bouy.

Renew retroreflective tape on PFD's identified during inspection.

Provide faceplate cover on upper deck bar receptacle.

Repair and prove proper operation of bilge pump.

Renew fire detection system sensor in engine compartment.

Properly label circuit breakers for main saloon panel in engineroom and verify circuit breakers are sized correctly.

Install belt guard over main generator alternator belt and drive.

Remove oil soaked SFP insulation in fwd eng overhead IWO main generator crankcase breather and replace with approved insulation

Properly seal bulkhead penetration in aft bulkhead of water tank storage space.

Provide proper 1st aid kit.

Repair port and starboard running lights prior to operating at night but NLT 30DEC16.

Hydrostatically test (3) CO2 bottles for the engine room system and provide report.

Provide (3) additional orange smoke or red hand flares prior to operating.

Provide cover for 110 VDC outlet and cover for exposed light in the upper deck bar.

Repack fire main valve #7 located in engine room.

Provide Marine Operator Permit.

Provide emergency broadcast placard.

Provide and post emergecny instructions.

Prove proper operation of the E light on the second deck.

Renew lifeline and provide clip for ring buoy.

Renew deck support IWO starboard side deck 2 bar at next drydock.

Repair / replace J Box at deck one port side aft stairwell.

Crop and renew three areas on hull at next dry dock. Area under port and starboard bow, starting six feet back from stem and from keel 2.5 feet up; under starboard fuel tank 4 feet x 4 feet; and under sewage tank 70 inch wide x 45 inch long.

Strainer for the bilge suction line in the #2 compartment was found wasted. Strainer is required to be renewed.

Fire hose on the 02 deck was found to be holed and leaking during testing. Hose is required to be renewed.

Vessel is required to undergo a complete hull and internal structural examination NLT 31JAN08 - see narrative for details.

Vessel is to install a plug in the missing electrical junction box located on the main deck aft.

Vessel is to reattach the unsecured wiring on the 01 deck overhead with proper metal fastenings.

Vessel is to install proper 4" rail courses for the rails located in the passenger area on the bow. Additionally, rails are to be provided IWO openings on the port and starboard sides of the same area.

Provide USCG Marine Safety Center approval for revisions noted in "as built" drawings for wastewater treatment system.

Renew 2 orange smoke flares.

Renew handle for emergency fuel shutoff valve.

Provide proof of current enrollment in a drug testing program.

Properly install machinery guards on both propulsion engines.
