The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 122.510, when applicable, the duties listed in the station bill required by § 122.514. Did not conduct drills during annual exam. Perform crew drills in the presence of attending MI.

Each individual bilge suction must be fitted with a suitable strainer having an open area of not less than three times that of the suction pipe. In addition, a mud box or basket strainer must be fitted in an accessible position between the bilge suction manifold and the pump. MI observed bilge suction strainer in void space #3 deteriorated. Renew as original.

Doors in A-Class fire control boundaries must meet the following additional requirements: (1) A door in a bulkhead required to be A-60, A-30, or A-15 Class must be of hollow steel or equivalent material construction, solidly filled with approved structural insulation, and capable of meeting the requirements of an A-15 Class bulkhead" MI observed fire door between machinery and engine room not meeting fire control boundary with 1/2 inch gap under fire door. Submit repair plan.

Each portable fire extinguisher on a vessel must be of a type approved by the Commandant. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by table 1 to § 118.500(b) and other provisions of this section. 03 portable fire extinguishers removed for hydrostatic testing.

All pressure vessels other than unfired steam boilers shall be protected by pressure relieving devices. The pressure vessel in the pilot house does not have a pressure relief valve.

Batteries shall be secured against shifting with motion of the boat. The battery located in the pilot house under the helm were not secured to prevent movement. 46 CFR 183.05-20(b) OLD T

Double doors capable of independent operation and latching may have a clearance between the doors of not more than 3.2 millimeters(0.125 inches). However if one door must always be closed first, means shall be provided to ensure that the doors close in the proper order.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following:(1) The location of emergency exits, survival craft embarkation areas, and ring life buoys;(2) The stowage location(s) of life jackets;(3) Either:(i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or(ii) That passengers may contact a crew member for a demonstration as appropriate,(4) The location of the instruction placards for life jackets and other lifesaving devices;(5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master; and(6) If the vessel is operating with reduced manning or equipment requirements in § 176.114 of this chapter. Vessel's passenger safety orientation script did not include all of the required information. 46 CFR 185.506(a), 16c

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. No portable extinguishers are being subjected to monthly inspection. 46 CFR 176.810(b), 16c

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Install metal supports in engine room and engine room vestibule.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Replace missing bulb and cover on main distribution panel in engine room.

KY02: Replace the doubler on the port side, side shell IVO where vessel rubs on fender at the pier (submit plan proposal for new plate thickness, size and possible frame changes).

Replace the missing steel clips for the windows that have aluminum frames. 46CFR72.05-30(a)

Replace the missing insulation on the soft patches in the engine room overhead.

Provide Safety Radiotelephony Certificate. Restricted to less than 1000ft from shore.

Provide 2018 Coast Pilot and Light List publications.

Clean grease from galley extraction hood.

Trace and remove ground from 120V system.

Enclose wires for high water alarms in junction boxes in bow thruster and pumproom spaces.

1) Enter MMSI number into VHF/DSC capable marine radio.

AF01- Repair hole in bottom fire door leading to the galley.

AF02- Remove electrical outlet on distribution panel in refrigeration void.

AF03- Clean galley hood.
