The system shall also be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 parts per million.
PSCO observed affluent valve on line from fresh water to oil content meter was blocked causing inaccurate reading of the 15 ppm alarm.

PSCO observed numerous lighting fixtures which had degraded silicone seals around the wire junctions in addition to several fixtures missing grounding wires as they were originally installed.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board.
The crew could not display proper ventilation arrangements for the forward paint locker during the examination by PSCO. crew later stated that the ventilation fan was having problems and was waiting to be replaced, but this was not disclosed at the opening meeting when crew was asked directly if there were any issues or maintenance items on board.
