Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found on the bridge under the helm, in void #2 and in life jacket storage on the main deck near bar. Remove dead end wiring and update electrical plans.

MI found missing structural fire protection in the Bow Thruster room in the forward overhead. Replace missing structural fire protection and notify local OCMI.

MI found missing gasket on engine room door. Replace gasket on engine room do and notify local OCMI.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel’s stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. MI found no upper decks were marked with number of passengers allowed. Replace markings and notify local OCMI.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years. Vessel is overdue for drydock examination due December 31st, 2024. Vessel is to complete a satisfactory drydock examination prior to carrying passengers. 46 CFR 115.600 (c)(1)

Observed wasted steel in the overhead, aft, and starboard side of the thruster room. Vessel is to make temporary prior no later than 22DEC2024 (next scheduled trip) and will be limited to Lakes, bay, and sounds. Permanent repairs are to be made at next schedule dry dock (31DEC2024).

All glass in windows or airports shall be at least1/4 inch thick. However, greater thickness may be required for strength purposes in certain locations. All glass shall be fitted in steel or equivalent metal frames and shall be retained by steel or equivalent metal glazing beads or angles. Vessel has various cracked windows and will be replacing all windows at next dry dock (31DEC2024).

Vessel's first aid kit was found with expired items, renew all expired items.

Found 01 fire nozzle that was broken on the upper deck of the vessel. Replace with new nozzle and preform operational test in front of Marine Inspector.

Fire screen doors #3,#7,#8 did not fully close and latch shut when the doors were activated. Starboard midship fire screen door was not able to close do to floor is bulging and not allowing the door to close. Fix all non-closing doors and add metal threshold.

The bilge visible alarm on the bridge works, but the space that shows to be flooding was not the space the was showing on the bottom of the screen. Make the alarm that shows on the bottom of the screen match the space that is visually showing as active match, so it is easy to identify.

Identified first and second existing windows were cracked on upper deck STBD side forward, and two additional windows crack below main deck STBD side. 01 CG-835V issued. 01 def issued, 01 def cleared, and 01 defs remain.

Vessels port main engine failed and would not start up again.

Repairs and alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, accept during an emergency. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. During In-Service Inspection, vessel rep communicated that the Port MDE had been taken off line and was inoperable due to a leak detected on the turbo. Once Port MDE is repaired, Marine Inspector attendance will be required to witness satisfactory testing and operation of propulsion machinery prior to returning to operations.

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have vessel ready for inspection of its fire protection equipment. At time of In-Service Inspection, vessel's fire protection equipment annual servicing and inspection expired in November 2022. Annual servicing required and shall be verified by a Marine Inspector.

Port and starboard main engine turbo chargers found to be installed without protective insulation. Insulation required on all piping which exceeds 150 degrees F.

Electrical dead-ended cables not properly terminated; Engine room, Bow Thruster room at Aft bulkhead, and STBD upper deck overhead at electrical Junction box.

Electrical light fixture missing bulb and lens diffuser; Alcohol locker space and Galley.

Portable Co2 fire extinguisher service tag expired as of MAY2021.

ll escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 2” high: EMERGENCY EXIT KEEP CLEAR. Must mark aft exits from main passenger salon area.

Doors in A Class fire control boundaries must not be undercut more than .5” above the door sill or deck covering. Fire door located at the entry point to the upper deck salon area must be covered to meet fire control boundary requirement.

Items 10, 11 & 16 from CG-835V dated 11/16/2021 have not been rectified in a timely manner and are now 10 weeks beyond the original due date. Vessel is restricted from carrying passengeres until these three remaining items are corrected.

Overhead lighting in various areas of of Engine room not operational. Repair or replace.

STBD Generator switchboard amp meter did not display any amps while supplying electrical power to vessel while underway. Repair or replace.

Wall lighting fixture hanging by its electrical wiring in forward staging area STBD side Repair or replace.

15 Childs Life preservers found to have the wrong vessel name. Correct marking to vessel's name.

Identification for Fire Station 6 found to be illegible. Replace or repair current marker.

Finished length of grab lines on life-rings shall be 4x the external diameter of life ring attached at 4 equidistant locations. Properly repair grab lines on all life-rings and remark vessel name.

Doors in class "A" bulkheads, stairways, and Main Vertical Zones shall: Minimum 3/4 latch throw, less than 1/2" under door, no carpet/rug under door, self closing from control station, and self latching.

All Fire Screen Doors shall be numbered and labeled.

Vessel suffered a loss of propulsion due to an unforeseen failure. Make proper repairs and provide technician report indicating cause of failure, repairs completed and specific testing conducted to demonstrate unit is returned to proper operating conditions. Vessel is restricted from carrying passengers until deficiency is rectified.

Port side generator overheating due to cooling system contamination with sea grass.

REMOVE UNUSED/LOOSE ON PORT AND STBD GENERATORS.

PROVE OPERATION OF OVERSPEED TRIP ON PORT AND STBD GENERATORS.

MAKE PERMANENT REPAIRS TO BOTH LONGITUDINAL FRAMES IN WAY OF AFT BALLAST TANK

MAKE PERMANENT REPAIRS TO TRANSVERSE FRAME STBD SIDE IN FWD BALLAST TANK

MAKE PERMANENT REPAIRS TO ALL TRANSVERSE FRAMES IN WAY OF AFT BALLAST TANK.

PROVIDE BLANK AND REMOVE VALVE TO 4" SEWAGE DISCHARGE PIPE.

PROCURE ONE (1) HAND HELD FIRE PUMP

1. INSTALL ADEQUATE GUARDS FOR THE FLEXIBLE HOSES IN THE STEERING COMPART- MENT TO ELIMINATE THE POSSIBILITY OF FLUID UNDER PRESSURE COMING INTO CON- TACT WITH ANY SOURCE OF IGNITION.

THE RACOR DIESEL OIL FILTERS FOR THE TWO MAIN ENGINES AND TWO GENERATORS HAVE GLASS BOWLS BUT NOT FLAME SHIELDS. FLAME SHIELDS ARE REQUIRED.

THE GOULD IMPERIAL EASTMAN JUMPER FUEL HOSES BETWEEN THE DIESEL OIL FILTERSAND THE ENGINE FUEL HOSE ON THE TWO MAIN ENGINES AND THE TWO DIESEL GENER- ATORS ARE NOT APPROVED FOR FUEL OIL SERVICE. THESE HOSES MUST BE REPLACED WITH COAST GUARD APPROVED FLEXIBLE HOSE FOR FUEL OIL.
