Wire, other than in switchboards, must meet the requirements in ANSI/UL 44, ANSI/UL 83, MIL-DTL-76E (all three standards incorporated by reference; see § 110.10-1 of this subchapter), or equivalent standard. MI's observed 2 extention cords installed between the machinery workshop and the newly established ships office. Remove extension cords and properly install wiring with outlets as per referenced site.

The OCMI may require that a vessel and its equipment meet any test or inspection deemed necessary to determine that they are suitable for the service in which they are to employed. Unless a splice is made by an insulated pressure wire connector, it must be thoroughly soldered and taped with electrical insulating tape or the soldered joint must be otherwise protected to provide insulation equivalent to that of the conductors joined. Marine Inspector observed an uninsulated wire cable on the starboard side of the machinery space “Engine Room” that needs to be properly insulated and secured.

The master or operator shall ensure that personal flotation devices are at all times ready for use. 01 Type 1 lifejackets were condemned due to poor condition. Vessel replaced lifejacket immediately.

Each vessel must meet the applicable requirements of Subchapter S, specifically 170.122 Stability and Watertight Integrity. While at Pier 21 in Galveston, the crew reported a pin hole leak in the hull plating weeping at frame 68 Port. An epoxy patch was applied, which stopped the further ingress of water. A cement patch will also be poured, similar to previous repairs made in 2018 (MISLE #6340484). All temporary repairs will continue to monitored for further ingress of water. Permanent repairs must be made to the satisfaction of the attending Marine Inspector at the next dry dock, but no later than the next scheduled dry dock in January 2021. 46 CFR 169.400

Relocate plimsol mark IAW the LL11D and full term ICLL certificate at next drydock January 2021

Anchor chain shackle attached to the anchor has a modified shackle pin. Replace with properly sized and type shackle prior to 05-05-2018

Due to vessel's draft lines used with the Load Line Marks (Regulation 6) were not applied to the vessel's hull. to be rectified at the first opportunity but no later than 5 Sep 2018.

Chain plate Palms where they attach to the round stock is worn/corroded to almost 50% in areas. Chain plate Palms are to be monitored IAW vessel's PMP and repaired/replaced as needed but NLT next drydock in January 2021.

Replace all non-flexible, worn flex hoses on all machinery in the E/R.

Properly support all elec. wire runs in the E/R.

Vessel needs to install permanent piping for shower and sink in FWD hold.

Permanently mount Air Compressor to deck framing.

Proved ventilation closure for mid hold.

Permanent repairs need to be made to the hole with temp patch on H strake between frames 79-80 on the starboard side during next dry dock to the satisfaction of the CG Marine Inspector attending.

Make permanent repairs to frames 69, 70, and 71 Port, within 4 feet of the keel.

Make permanent repairs to the fire main piping at stations 1 and 2 at the main deck.

Locate and repair the source of the electrolics.

Provide a "Hull Strength Analysis" report evaluating the Sheer Strake and Deck Stringer plate in its actual condition, by a qualified engineer.

Crop and renew the 56 plates idenified in the Garboard, A, B, and C strakes.

Install a bilge alarm system that is capable of being continuously monitored.

Prove proper operation of the vessel's #1 Genrator.

Vessel's over speed trips did not operate on both generators. Provide report to attending Coast Guard inspector from ship's engineer verifying proper operation of over speed trips on both generators prior to 25 March 2005

PROVIDE COPIES OF ALL SURVEYOR REPORTS CONDUCTED FOR DAMAGE SURVEY FOR FORWARD DECK AREA. NO PASSENGERS OR VISITORS ALLOWED FORWARD OF GALLEY HOUSE AFT BULKHEAD. PROVIDE REPORTS NO LATER THAN 02 MAY 02.

PROPERLY SECURE ALL ELECTRICAL WIRING ON THE MAIN DECK BENEATH THE PORT ANDSTARBOARD PIN RAILS.

TIGHTEN ALL INTERIOR HANDRAILS.

PLUG ALL STUFFING TUBES IN THE WATERTIGHT BULKHEAD BETWEEN THE ENGINE ROOM AND AFT HOLD.

PROVIDE REVISED SHELL PLATE EXPANSION DRAWING TO MSO HOUSTON/GALVESTON.

RENEW FOUR STUFFING TUBES ON MAIN DECK. THREE ARE LOCATED PORT SIDE UNDER PIN RAILS AND ONE LOCATED STBD SIDE UNDER PIN RAILS.

At next drydocking, weld external side of water spray piping, generator exhaust piping and main engine exhaust piping in way of passage through dryexhaust overboard / diverter box. All repairs to be made to the satisfaction of attending Coast Guard Inspector.

MAKE AVAILABLE FOR INSPECTION EXHAUST SYSYTEM AND STERN CONNECTION WELDING,BOTH INTERNAL AND EXTERNAL.

PROVIDE DIRECTORY FOR CIRCUIT BREAKER PANEL.

INSTALL BLANKS IN ALL UNUSED / OPENED SPACES IN CIRCUIT BREAKER PANEL.

Upon approval of a procedure to determine scantling condition as required in MIDR #1 this case, determine the scantling conditions to the satisfaction of the attending marine inspector.

Repair as required and restore to original strength the material condition of the scantlings and internal framing in way of the wind and water strakesto the satisfaction of the attending marine inspector.

Repair as required the partitioned section of the CVK in way of the forepeak to the satisfaction of the attending marine inspector.

Develop and submit for consideration by the OCMI a means for determining the thickness and material condition of the waterborne hull scantlings in way of "A" strake.

PROVE PROPER OPERATION OF BOTH GENERATOR OVERSPEEDS.

PROVE PROPER OPERATION OF LUBE OIL SHUTDOWN AND ENGINE OVERSPEED ON MAIN ENGINE.
