The speed of door closure shall, if necessary, be controlled so as to prevent undue danger to personnel. PSCOs observed each fire door tested in every stair well (to inlcude FD 236, 155, 817, 820, 106A, 727, 121, 005, TWD 06, TW03, 119, 117, 112, 109, 105 and more) slammed closed between 2-4 seconds. The weight and speed of door closure could cause undue danger to any person in way of door closure.

Routes of escape from Four Seasons restaurant, FD 238, FD 239, and route leading FWD from theater near FD 306 were obscured with curtains.

The speed of the door closure shall, if necessary, be controlled as to prevent undue danger to persons. Spot-checked various types of fire doors throughout all main vertical zones, across all decks and observed an excessive number of fire doors closing in a manner that may cause harm to passengers and crew members based on the speed of closure (approx. 2-6 seconds)
