Each window and its means of attachment to the hull or deck house must be capable of withstanding the maximum load from wave and wind conditions expected due to it’s location on the vessel. During dry dock inspection, forward lounge port window had stress cracks (3”-6”) radiating from fittings, and a forward lounge starboard window was missing most of its bolts from the frame.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaced located below the main deck. Vessel did not have adequate emergency lighting along 01 of the escape routes from the galley.46 CFR 120.432(a)

Each vessel that relies on electricity to power... the vital systems listed in 119.710 of this chapter must be arranged so that the loads can be energized from two sources of electricity... A vessel with batteries of adequate capacity to supply the loads for three hours. Vessel's fire detection panel battery backup did not last more than 1 hour. Vessel must provide proof that fire detection panel battery backup will last for three hours.46 CFR 120.310(a)(1) 46 CFR 120.310(a)(2)

Area of refuge means an area that is separated from the effects of fire and flooding where passengers and crew can gather to await disembarking in the event of fire or flooding. Vessel was storing chairs and other objects in the designated area of refuge, making it not operational as required in the event of emergency.46 CFR 114.400

Vessel experienced a mechanical failure on the port main engine while underway. The error code indicated a “Microcommander” fault.

All microprocessor or computer based propulsion engine control systems must meet the requirements of part 62 in subchapter F of this chapter. A qualitative failure analysis (QFA) of the vessel’s propulsion engine control system (throttle) shall be submitted to MSC. A DVT Procedure (DVTP) document that incorporates all failure modes identified on the QFA should also be submitted for review to MSC. The QFA should specifically address the failure of the microprocessor based control system hardware and those inputs to the control system whose failure could affect the propulsion system’s ability to default to the required conditions in 46 CFR 121.620(c). The QFA must indicate continued compliance with 46 CFR 121.620 so that power failure to the processor or processor failure will not result in an increase of shaft speed or propeller pitch. The QFA should be based on single, non-concurrent system failures (including loss of power). Plans shall be submitted to [email withheld], submittal shall indicate vessel project number (P016675). Following approval from MSC the DVTP testing shall be accomplished to the satisfaction of the attending Marine Inspector.

Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Nonmetallic flexible hose must have factory-assembled end fittings and must comply with SAE J1475. Appx 10 feet of a 15 plus foot run of flexible Aeroquip 2556-8 (1/2”) SAE 20R2, 250 psi lube oil hose is hanging between overhead and bow thruster tunnel. While this does not meet requirements, it appears satisfactory for its service on the pressure head tank to bowthruster. Concern is hanging 10' section with nearly 90 degree bend. Review and install per manufacturer specification 5" radius and supported along run per distributor website, which operator shall verify.

General construction of the vessel shall be such as to minimize fire hazards. Vessels constructed on or after 1 JUL 1961 which carry more than 150 passengers shall meet the requirements of subpart 72.05. Stairway enclosures adjacent to public spaces over 500 square feet, or adjacent to machinery spaces, or boundaries of main vertical zones shall be A-60. Wire penetration at top of machinery space vestibule (looking forward from door) is incomplete.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. MI identified hanging wires not supported by metal supports for the following: Bilge float switch located in steering gear space, light wiring located in steering gear, wire runs in port galley, wire runs aft of stbd main engine shaft seal access plate.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI identified the following unsafe electrical conditions: loose power outlet located in lazarette, loose power outlet located on port side 02 deck, and port fwd head distribution panel door not secured near port storage entry.

A door in A-class fire control boundary in a stairtower must be the self closing type. Self closing door in galley stair access to main deck does not fully close. Gap less than 1/2" may continue operations until fixed.

Penetrations and openings in watertight bulkheads must have means to make them watertight. Penetrations in A Class fire control boundaries for electrical cables, pipes, trunks, ducts, must be constructed to prevent the passage of flame and smoke for one hour. MI identified wire penetration between steering gear space and aft storage space (a MWTB listed on vessels stability letter) bulk head not watertight or smoke/fume tight.

A power driven vessel underway shall exhibit a stern light as required by 33 CFR 83.20. Stern light inoperable during inspection.

Means of escape. The dimensions of a means of escape must be such as to allow easy movement of persons when wearing life jackets. There must be no protrusions in means of escape that could cause injury, ensnare clothing, or damage life jackets. Galley escape had tray rack, extinguishing pipe with 1” cable run hanging from tape in place, and wood dunnage obstructing the egress. The fixed extinguishing pipe presents a low overhead but could be adequate by keeping the space open, and the section of piping (at head level for 6’ person) could be fitted with highly visible markings and no cables hanging or objects that could snag clothing or life jackets. 177.15-1 All vessels shall be provided two avenues of escape from all general areas accessible or where the crew may be mornally employed, so located if one is not available the other may be.

Bilge pumping line. During operational test, bilge discharge pipe had leak.

Electrical cables and fixtures. Each connection to a connector or terminal connection must be made within an enclusre. Stbd galley service pway found to have two loose pigtails and a refrigerator pump connectors exposed.

Lighting systems guards. Port galley in vicinity of water heater, and lower head(restroom) light fixtures were missing covers.

Mess deck and galley spaces. Upper metal outboard of ovens found lowered and zip tied in place, defeating heat deflecting properties. Secure as designed.

A vessel inspected for compliance with the standards required by this subchapter may be inspected in accordance with standards acceptable to the OCMI as good marine practice. During annual inspection, port rudder post found with wastage, which made a hole through a support strut at the upper connection to rudder post. 705 C defer to drydock (MAR 2024).

The master shall determine that the vessel complies with all applicable stability requirements. Vessel is currently awaiting approval of stability test and fire load analysis from MSC. Vessel may only operate with up to 300 maximum passengers until the plans are approved.

Upon completion of the current alterations, a stability test(either deadweight survey or inclining) should be conducted on the vessel and revised loading conditions / stability calculations be prepared as necessary to demonstrate that the vessel’s stability remains adequate for the intended service. Sewage tank between frames 12 and 21 is wider than original; and a gray water collection tank (appx 2.5'x3'x15') was added aft of frame 46 and has not been documented. This change to the sewage tank and addition of collection tank should be included in stability calculations. Vessel may operate with reduced passenger count, reduced from 600 to 500 (475 passengers, 8 crew, 17 other persons in crew crew), through scheduled COI renewal of 7 JUL 2022.

Repairs or alterations to the hull that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing details of alterations to the cognizant [name withheld] pumps, piping and a grey water collection tank, approx- imately 2.5'x3'x15', were discovered during drydock,

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered, the cognizant OCMI may conduct an internal structureal examination in any affected space and may require the vessel to be dry-docked or taken out of service to assess the extent of the damage and to effect permanent repairs. The OCMI may decrease the dry-dock examination intervals to monitor the vessels structural condition. Prior to relaunch, ensure marine inspectors witness satisfactory Non-destructive Testing (NDT) of the affected 17'9"x11' bottom plate, framing, and tank top repairs.

Bilge alarms did not alarm visually or audibly during operational test of system for clearing of prior CG-835v

All observed unsafe practices, fire hazards, and other situations must be corrected. Electrical wiring under the pilot house steering console was found to have dead end wires and exposed connections.

Spanner wrench must be provided for each fire hydrant. Two spanner wrenches were found to be missing.

Vessel did not have updated copies of FCC certificates onboard.

Vessel had a 70 amp breaker installed into the galley panel and was using it as a blank.

Vessel had a damaged power strip installed with a junction box, which showed a ground when tested.

top/open deck aft the vessel had 2 outlet boxes 1 one each side that had the steel piping rotted through leaving wires exposed to element as well as water intrusion into the electric boxes.

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. The vessel has not completed a satisfactory annual inspection. Vessel cannot carry passengers until a satisfactory annual inspection is completed.

A spanner wrench must be provided at each fire hydrant. Provide spanner wrench for hydrant #4.

The vessel COI was found posted in the vessel's pilothouse where passengers could not view the certificate. Post COI in a location where it can be seen by all passengers.

The lifebuoys had crew installed line that was not proven to be UV resistant and of an unknown breaking strength. Provide 60' of line that meets UV and strength requirements.

During the inspection it was discovered that the steering tie rod pin connecting the tie rod end to the STBD rudder arm was missing a cotter pin.

During the inspection it was discovered that the sewage overboard discharge valve was outfitted with a chain and lock, however the lock was not closed. The valve was closed at the time of the inspection.

A discrepancy in the the vessels drug and alcohol testing program was discovered when one of the deckhands was not listed on the testing enrolment crew list.

During the inspection it was discovered that the vessel's anchor light was extinguished.

Due to vsl alliding with the ground underwater, the vessel requires internal structural exam prior to carrying passnegers. Vessel is no sailed.
