Additionally, a wire run was routed through the vent opening, preventing the protected space from vapor tightness. Remove wire run and replace vent closure.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. Make necessary repair to high water alarm.

An adult life jacket must be provided for each person carried on board a vessel. 04 Adult PFD's and 02 Child PFD's were not is working condition. Replace/maintenance as required.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency Instructions were not posted at operating station. No later than Friday January 16th, 2026, Post Emergency Instructions at the operating station and ensure all crew is familiarized.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Vessel has extensive blistering in the hull, provide a repair proposal prior to the carriage of passengers.
