Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board. Vsl had no immersion suits on board. 46 CFR 199.273 (a)

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. During deficiency check MI identified new steel repair in vicinity of the locker room and the exterior portside aft hull bulkhead in way of the tug notch that was not reported to the Coast Guard. 46 CFR 91.45-1

All vessels shall have efficient guard rails or bulwarks on decks and bridges. There are no rails present on the port aft side of the deck located on top of the accommodation space due to missing mate’s shack. Replace mate’s shack or install compliant rails prior to dredging operations. 46 CFR 92.25-5(a), 705c prior to operations.

Where the extinguishing agent can be released by remote control, the system must have a manual local control at the cylinders. Fixed halon firefighting system can be released by remote controls located near the entrance/exit of the protected space. Release instructions direct the actuator to “proceed to the FM-200 local actuation station and follow the operation instructions.” There are no operation instructions posted locally at the FM-200 bottles, nor was the crew in attendance aware of how to actuate the system locally. Rectify prior to dredging operations. 46 CFR 95.15-5(a), 705 prior to operations

The owner of a vessel subject to inspection under section 3301 shall: (1) ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel’s master key of which access shall only be available to the individuals described in paragraph (2); (2) establish a list of all crew, identified by position, allowed to access and use the master key and maintain such list upon the vessel, within owner records and included in the vessel safety management system; (3) record in a log book information on all access and use of the vessel’s master key, including—dates and times of access; the room or location accessed; and the name and rank of the crew member that used the master key; and (4) make the list under paragraph (2) and the log book under paragraph (3) available upon request to any agent of the Federal Bureau of Investigation, any member of the Coast Guard, and any law enforcement officer performing official duties in the course and scope of an investigation. Vessel is not equipped with a vessel master key control system and log book. 46 USC 3106, 50c

On a merchant vessel of the United States the construction of which began after March 4, 1915: (5) each crew berthing area shall be equipped with information regarding- (A) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and (B) procedures and resources to report crimes, including sexual assault and sexual harassment, including information- (i) on the telephone number, website address, and email address for reporting allegations of sexual assault and sexual harassment to the Coast Guard; (ii) on vessel owner or company procedures to report violations of company policy and access resources; (iii) on resources provided by outside organizations such as sexual assault hotlines and counseling; (iv) on the retention period for surveillance video recording after an incident of sexual harassment or sexual assault is reported. Crew berthing areas were not equipped with the required information. 46 USC 11101, 50c

The provisions of this subpart apply to all industrial vessels of 300 gross tons or more but less than 4,000 gross tons, contracted for on or after July 1, 1968, which carry in excess of 12 industrial personnel. The boundary bulkheads separating the accommodations from galleys shall be of “A” Class construction. The two doors separating the galley space from to the accommodation space were not of “A” class construction. 46 CFR 92.07-1(b), 46 CFR 92.07-10, 50c

Each battery deck box must be weathertight. The battery deck box located on top of the accommodation space is wasted such that it is no longer weathertight. 46 CFR 111.15-10, 50c

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. There were dead end wires located on the second level of the crane house, stbd side. Corrected on the spot. 46 CFR 111.01(a)(2), 10c

Each life raft must be stowed to permit manual release from its securing arrangement. Life rafts are stowed on top of the accommodation house unable to be launched from their stowage location into the water. 46CFR199.130(c)(1)

On vessels in Oceans and coastwise service, suitable storm rails shall be installed in all passageways. Handrails on stairwell heading down to engineering space were wasted and holed. 46CFR92.25-10(a)

All piping shall run insight whenever practicable, protected against mechanical injury and secured to against excessive movement and vibration. Piping supports missing for fuel supply & return lines in Crane Tub. 46CFR56.50-70(b)

The construction of each lighting fixture for non-hazardous area must meet UL 1598A or IEC 60092. LED Light above wire spool in crane tub was missing cover and had multiple bulbs not illuminate. 46CFR111.75-20(a)

Openings in enclosed deck houses or super structure shall be protected for weather tightness. STBD Mate shack is wasted away. Remove or C/R at next drydock. 46CFR42.15-40(b)

Nothing in this subpart shall be construed as limiting the inspector from making such tests and inspections as he deems necessary to be assured to the seaworthiness of the vessel. IAW 47 CFR 80.103 when using DSC (digital selective calling) techniques, coast stations and ship stations must use maritime mobile service identities (MMSI) assigned by the commission. The vessels VHF radio is not currently connected to GPS and the MMSI is not input into the radio. Correct installation to allow for the DSC button to properly notify when pressed during an emergency. 46 CFR 91.27-15

Approved hand portable fire extinguishers and semiportable fire extinguishing systems shall be installed in accordance with Table 95.50–10(a). Portable Fire extinguishers were observed not installed in brackets. 46 CFR 95.50-10

Each marine shipboard cable must meet all the requirements for construction and identification of either IEEE 1580, UL 1309, IEC 60092-353, or NPFC MIL-C-24640A or NPFC MIL-C-24643A. Connections made to battery in port control house were without any type of lug, just bare wire wrapped around threaded battery post. 46 CFR 111.60-1

Each battery tray must be chocked with wood strips or their equivalent to prevent movement. Battery noted in deficiency No. 03 was not installed in a tray. 46 CFR 111.15-5

Each marine shipboard cable must meet all the requirements for construction and identification of either IEEE 1580, UL 1309, IEC 60092-353, or NPFC MIL-C-24640A or NPFC MIL-C-24643A. Bushing for wire penetration entering HVAC unit on top deck was pulled free from installation. 46 CFR 111.60-1

Each marine shipboard cable must meet all the requirements for construction and identification of either IEEE 1580, UL 1309, IEC 60092-353, or NPFC MIL-C-24640A or NPFC MIL-C-24643A. Conduit for wiring to emergency lighting battery bank was disconnected at union on top deck, outboard of fixed halocarbon stowage. 46 CFR 111.60-1

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel. 46 CFR 199.70 (a)(2)

Branch lines connected to the fire main for purposes other than fire and deck wash shall be so arranged that adequate water can be made continuously available for firefighting purposes. Valve for branch line off of Fire Station 2 was missing a handle and could not be closed. 46 CFR 95.10-5 (f)

An aggregate of more than 7.6 liters (two gallons) of flammable or combustible liquids stowed outside of an accommodation, control, or service, space (other than a paint locker) or outside of a machinery space must be stowed in a paint locker. Several cans were found stowed just outside of aft paint locker. 46 CFR 147.45 (b)

A ship of 300 gross tons or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil...fill pipe. Both diesel fill pipe connections were noted extending beyond the containment and the drain plug for the port side containment was missing. 46 CFR 155.320

Each fire hydrant must have at least one length of firehose, a spanner wrench, and a hose rack or other device for stowing the hose. Fire Stations Nos. 5 and 6 were missing spanner wrenches. 46 CFR 95.10-10 (g)

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Emergency lighting at base of internal accommodation stairway was inoperable. 46 CFR 111.75-15 (c) (2)

Each cargo vessel must carry the number of lifebuoys prescribed in table 199.271 of this section (a vessel of under 100 meters must carry a total of 8). Vessel outfitted with 6. Vessel route is for "Oceans" and does not qualify for alternative (6) allowed by § 199.640. 46 CFR 199.271

The holder of a credential may not voluntarily part with it or place beyond his or her personal control by pledging or depositing it with any other person. Vessel's Certificate of Inspection requires that there be two certified life boatman on board when the vessel is manned. The life boatman was unable to show proof of his credential and will need to have credential on board the vessel to the satisfaction of the attending MI. 46 CFR 10.233

All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any water tight compartment. The suction point in the aft section of the Port spud room was unable to draw suction. Demonstrated suction from this point to the satisfaction of the attending MI. 46 CFR 56.50-50 (a)

Each bilge suction must lead from a manifold except as otherwise approved by MSC. As far as practicable, each manifold must be in or be capable of remote control operations from the same space as the bilge pump that normally takes suction on the manifold. The bilge suction manifold labeling is covered in grime and paint, renew or clean to the satisfaction of the attending MI. 46 CFR 56.50-50 (c) (1)

Alarm details- Be provided with a means to test audible and visual annunciators. The ship service diesel generator alarms required by 46 CFR 62.35-50 were tested and have a visual annunciator but the audible annunciator is OOC, repair or replace to the satisfaction of the attending MI. 46 CFR 62.25-20 (e) (iii)

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight doors. Both Port and STBD spud rooms have an escape scuttle and ladder way that are located on the other side of the winch. Persons would be required to move over the top of moving machinery to exit the space in the event of an emergency. Install a means to escape safely to the satisfaction of the attending MI at the vessel's next dry dock. 46 CFR 92.10-5

Each generator switch board must have a disconnect switch or link for each generator conductor. Vessel is not set up for parallel operations, there is not a safety interlock on either switch for the 2 gen sets, off line generator is to be locked out to prevent inadvertent closure of the switch. Corrected on the spot. 46 CFR 111.30-25

The applicable standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), may be used as the standard for the design, construction, and testing of main and auxiliary machinery except as modified in this subchapter. 3412 Generator/HPU has fuel leak from double jacketed fuel system and missing guards.

Approved hand portable fire extinguishers and semiportable fire extinguishing systems shall be installed on all vessels, as set forth in subpart 95.50. Paint locker CO2 needs to be serviced

An automatic emergency lighting and power system is one in which a reduction in potential from the ship's service power and lighting. E/R e-light was extinguished

Crane hoist engine has hydraulic oil leak and oil leak on the turbo.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. Crane main engine is missing guards

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. Expansion joints were installed in 2009.

Approved fire extinguishing systems may be used or required in locations delineated in this section on the following vessels. FM-200 system was overcharged due to heat in storage location.

Strainers. A suitable twin strainer shall be fitted in the fuel supply line in the engine compartment. Strainers shall be of the type opening on top for cleaning screens. A drip pan shall be fitted under the strainer. 3412 Strainer has drain valve that can not be plugged and leaking.

Each production device must be legibly marked in accordance with paragraph (b) of this section with the following information: (1) The name of the manufacturer. (2) The name and model number of the device. (3) The month and year of completion of manufacture. (4) Serial number. (5) Whether the device is certified for use on an inspected or an uninspected vessel. Placard is missing

Each device must have a placard suitable for posting on which is printed the operating instructions, safety precautions, and warnings pertinent to the device. The size of the letters printed on the placard must be one-eighth of an inch or larger. Placard is missing

Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Embarkation lighting battery box had multiple wire nuts installed.

Each general emergency alarm signal must be marked “GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION” in red letters at least 1/2 inch high. General alarm markings were missing.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. engines have plastic oil catch cans which are a fire hazard.

The load line certificates for which the Government of the United States of America assumes full responsibility may be issued by the Commandant, the American Bureau of Shipping, or a recognized classification society when appointed as an authorized assigning and issuing authority for specifically designated vessels. Can not operate beyond boundary line till Load Line Certificate is endorsed.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Ensure all machinery and its vital components are installed as per applicable CFR requirements, manufacture requirements and safe to operate. Prove proper operation of all emergency lighting. Prove all required fire extinguishing appliance are ready for use. Ensure all items are cleared prior to operations.

A damage survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Damage survey must be conducted for fire onboard prior to operating. Crew member tested positive for COVID-19 after direct contact with other personnel onboard. Vessel deemed unsafe/hazardous for Marine Inspectors. Provide sufficient COVID-19 mitigation plan to allow for proper damage survey of vessel.

The lifejackets stowage positions must be marked with the words “LIFEJACKETS” or “CHILD LIFEJACKETS” as appropriate, or with the appropriate symbol from IMO Resolution A.760(18). Stowage locations not marked.

It shall be the duty of the owner, master, or person in charge to require and have performed at least once in every 12 months the tests and inspections of all hand portable fire extinguishing systems. Found more than half portable fire extinguishers not serviced. (46 CFR 97.15-60(b))

No repairs or alterations affecting safety of the vessel with regard to hull, machinery, or equipment shall be made without the knowledge of OCMI. Found two unapproved repairs and one bolt welded onto fire line. (46 CFR 91.45-1(a))

Electrical installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Found screen on generator 3412 dirty; caked with residue. (46 CFR 111.01-1(a)(2))

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Found all ring life buoys fitted with both self-igniting lights and lifelines. (46 CFR 199.70(a)(3)(ii))

Complete remaining worklist items prior to operating under the authority of COI and to the satisfaction of the attending CG Marine Inspector.

The submitted welding procedure for the welding of the vessel's seawater piping system called for a backing strip to be utilized. The seawater pipes were subsequently welded without backing strips in place. The removal of the backing strips is considered a change of an essential vairable as defined in ASME IX QW-400. Resubmit an approved welding procedure without the use of a backing strip along with corresponding welder qualifications for the procedure once it is tested and approved. Upon receipt of this documentation the seawater piping repairs will be considered permanent.

Pressure vessel safety valves shall meet service requirements for which they are intended. Safety valve did not adequately relieve excess pressure as designed. Provide servicing to valve to the satisfaction of the attending CGMI.

There must be enough receptical outlets throughout vessel for electrical equipment for safe operation. Remove daisy chain wiring and provide sufficient electrical outlets to the satisfaction of the attending CGMI.

Provide proof of crew members enrollment in a random drug testing program.

Two life buoy storage areas are not properly labeled. Label life buoy storage area.

3 life buoys require additional reflective tape. Repair or renew 3 life buoys.

Lifejacket donning placard missing. Provide proper placard.
