All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Anchor light wiring was pinched and light was inoperable. Vessel is restricted to daytime operations until light is operational. 46 CFR 183.420

A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Vessel operator could not simulate shutdown of engine during Fireboy testing. The vessel must demonstrate that the pre-engineered fixed gas fire extinguishing system is working according to regulations. 46 CFR 181.420(a)(3)

Except as provided in paragraph (b) of this section, the weather deck on a flush deck vessel must be watertight and have no obstruction to overboard drainage. Engine room hatch was not watertight. Make repairs to hatch to ensure watertight integrity. 46 CFR 178.410(a)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. STBD fuel filter is leaking. Identify source of leak and repair to satisfaction of CG marine inspector. 46 CFR 176.800(a)

All cable and wire must: Have stranded copper conductors with sufficient current carrying capacity for the circuit in which they are used. STBD battery had exposed wiring on terminal. Port battery bank has corrosion and melted battery terminal cables. Repair or replace to satisfaction of CG marine inspector. 46 CFR 183.340(b)(1)

Each survival craft must be: Secured to the vessel by a painter with a float-free link permanently attached to the vessel except that a float-free link is not required if the vessel operates only on waters not as deep as the length of the painter. Life float does not have float free link attached to vessel. Attached life float to vessel as per regulation. 46 CFR 180.130(a)(1)

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Valve installation did not shield from flames. Install appropriate valve protection 46 CFR 182.455(b)(4)

Significant pitting identified on interior hull beneath accommodation space compartment, located centerline between 3rd and 4th frames. There is also pitting and corrosion in forward compartment, inside of chain locker. Measure wastage and repair during next credit dry-dock. 46 CFR 176.800(a)

In addition to meeting § 180.130, each life float and buoyant apparatus must be stowed as required under this section: (b) The float-free link required by § 180.130(a)(1) must be: (1) Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant; (2) Of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag; and (3) Secured to the painter at one end and to the vessel on the other end. The means used to attach the float-free link to the vessel must: (1) Have a breaking strength of at least the breaking strength of the painter; (2) If synthetic, be of a dark color or of a type certified to be resistant to deterioration from ultraviolet light; and (3) If metal, be corrosion resistant. - In current arrangement, life float is attached to the vessel with no weak link.

Life floats. Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light.The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. Currently, life float installed in subject vessel does not have paddles or the light required by the reference.

Each unpressurized fuel tank must be fitted with a vent pipe connected to the highest point of the tank. Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942. Where a flexible vent pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Such hose must overlap metallic pipe ends at least 11/2 times the pipe diameter and must be secured at each end by clamps. Flexible fuel vent hose installed in engine compartments shall be USCG Type A1, A1-15, or A2. - Currently, STBD side fuel vent hose is dry rotted and unserviceable.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock - MI found unterminated wires with bare conductors in the following systems: PORT, CENTER, STBD windshield wipers, STBD VHF radio, bilge system alarm panel, and machinery space J-BOX behind fuel FO filters.

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Gasket currently installed in both engine room doors is damaged and in a non-servicable condition.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Communications Act Radiotelephony Certificates may be obtained from the Commission's National Call Center - [phone withheld] - or from its forms contractor. Communications Act Safety Radiotelephony Certificate was not available for inspection.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel failed to undergo 2022 annual inspection.

A vessel exposed to salt water more than three months in any 12 month period must undergo a drydock and internal structural examination at least once every two years. Vessel has failed to schedule drydock and internal structural examination within time period granted by drydock extension request.

A pre-engineered fixed gas fire extinguishing system must: Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector. The pre-engineered Fireboy gas fire extinguishing system for the main machinery space, was not equipped with a means to manually activate the system from outside the space. Install a means to manually activate the system from outside the protected space.

Secure the life float to the vessel using a painter that meets 46 CFR 180.175 (f) (i-iv), and a float free link that meets 46 CFR 180.130, and 46 CFR 160.073. (no float free link is required if the painter is longer than the maximum depth of water the vessel is certificated to operate in.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. There were no markings indicating where the lifejackets were stowed. Provide markings that clearly indicate the number, and size of the lifejackets stowed inside the bench seats.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). The VHF Radio installation did not have functioning DSC. Provide VHF Radio installation with functioning DSC.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. The port and starboard main propulsion shafts where they exit the reduction gear, are un-guarded and prevent a hazard to personnel. Provide guards to protect personnel from contacting the rotating propulsion shafts.

Employee Assistance Program. Each education program must include the following elements: display and distribution of a community service hot-line telephone number for crew member assistance. Vessel missing employee assistance phone number. Post help line number onboard.

Life Jackets. A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Vessel missing one child life jacket. Provide adequate number of life jackets.

Watertight integrity. A hatch exposed to the weather must be watertight. Anchor chain locker hatch is not watertight. Make hatch watertight.

Vapor Barriers. Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present. Vessel lacking vapor tight integrity between E/R space and main void space. Provide adequate vapor tight integrity.

Fuel piping. Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. The shutoff valves at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Vessel has inadequate fuel shutoff valve locations. Provide proper location for valves.

Vent pipes for fuel tanks. The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. Vessel missing flame screens on discharge ends of fuel tank vent pipes. Provide adequate flame screens.

Bilge high level alarms. On a vessel of at least 26' in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the normally unmanned spaces. Vessel's audio and visual bilge alarms inoperable. Provide adequate alarms.
