Vsl had dead-end wire in the bridge console, wire nuts in the bridge console, overhead on the second deck and on the gen battery. Vsl is required to make proper installation and make all wiring safe.

Main deck distribution panel had an opening blocked by duct tape. Vsl is required to fill the opening with a breaker or a proper blank.

Battery charger in the bridge console did not have an amp meter (only a visual indicator light bar was observed).

Vsl had 4 large lead acid batteries in the seating compartments on the main deck. Vsl is required to remove the batteries.

Vsl had a hole that was leaking rain water from the second deck overhead ceiling. Vsl is required to repair/replace the overhead ceiling in that location to prevent water intrusion.

A vessel must be provided with bilge pumps. Bilge lines to shaft alley and laz space were not re-installed.

Inspection of the condition of the superstructure. Remove doubler on lower port and stbd aft corners. Submit repair proposal and complete repairs prior to annual exam.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew and the vessel from electrical hazards. Remove/replace dead end wires in galley, broken antenna on wheel house, phone cable stbd side wheel house, cables to Garmin in wheel house, and light fixtures. Hard wire portable A/C unit in wheel house.

At each initial inspection and subsequent inspection for certification and reinspection the marine inspection shall examine and the test the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory and is fit for the service intended. The main and auxiliary steering apparatus shall be tested by operation and the inspector shall examine the mechanism thereof throughout its length. Vessel steering control at the operating station was being centered to amidships by bungie cords. 46 CFR 176.25-35 (Old T)

Provision shall be made by means of plugs, covers, etc, to prevent the admission of air into the space protected. The class 1 watertight door gasket leading into the engine room is deteriorated. 46 CFR 181.20-35 (Old T)

At each initial and subsequent inspection for certification the marine inspector shall examine and test all boilers and unfired pressure vessels to the extent necessary to determine the condition is satisfactory and that they are fit for the service for which intended. The plug on the receiver was damaged. 46 CFR 176.25-30

At each inspection for certification the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. The AC raw water pump and piping arrangement was not permanently secured. 46 CFR 176.25-50

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gas tight throughout its length and shall be properly supported by noncombustible hangers or blocks. The Port Main Engine has an exhaust leak. 46 CFR 182.15-20

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its design limitations. The Port Main Engine was found to have an excessive leak on the accessory drive distibuting oil onto the overhead. 46 CFR 182.05-1 OLD T

Life preservers for children when provided, shall be stowed separately. The adult lifejackets and child sized lifejackets are stowed in the same locker without separation. 46 CFR 180.25-10 OLD T

A vessel on a Rivers route must carry three red flares and three orange smoke distress signals. The orange smoke may be substituted for three red flares. The distress signals were expired. 46 CFR 180.70(b)

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems... to determine they are in suitable condition and fit for the service intended" An unidentified hydraulic leak in steering system. There is a five-gallon bucket installed on the bridge w/ hand pump to refill the steering gear hydraulic system. 46 CFR 176.25-35

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock. AC Condenser condensation being collected in galley serving tray, and buckets hanging over passenger accommodation spaces and dripping onto electrical equipment. 46 CFR 183.01-05

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock. Unsafe electrical maintenance practices were observed throughout vessel. Numerous power strips and extension cords were found being used as jumpers in the main galley, service room and bridge, dead end wires were found throughout the vessel, several splices were observed and not in an enclosed electrical box. Terminal strips under the bridge console are not secured and are hanging by wires. 46 CFR 183.01-5(a)

At each initial and subsequent inspection for certification the firefighting equipment of all vessels shall be inspected and all deficiencies shall be corrected. The flexible connection on the co2 fixed fire suppression system is kinked and restricted. 46 CFR 176.25-25

All vessels except non-self propelled vessels shall be provided with suitable steering arrangements. The Jockey bar between both rudders has a deflection of 6”-12” when hard over due to misalignment of rudder arms. 46 CFR 182.30-1

The vessel is a SPV-T (Old) and therefore is not required to have a safety management system per regulation. Requirement 1 through 5 are objective evidence that a serious substandard condition has been proactively managed by the vessel's owner/operator leading to a lack of safety culture onboard the vessel. 46 CFR 176.830

Examine watertight closures in bulkheads. WTD between steering compartment and aft auxiliary space could not be secured. WTD between aft auxiliary space and engine room could not be secured. 46CFR 176.25-5

Comply with stability letter. Two unapproved poly tanks were discovered in a void fwd of the engine room. 46CFR 170.120

Fire main and hydrants shall comply with 181.15-5. Single fire hydrant as provided cannot reach all parts of vessel with single length of hose. 46 CFR 181.15-5

Inspect vessel for unsafe practices. 46 CFR 176.25-50 Wood blocks, screws and hose clamps are used to secure the galley steam kettles, sewage pump and a/c r/w pumps.

Inspect vessel for unsafe electrical installations. 46 CFR 183.01-5 Permanently repair a/c condensate drain pan by 01Jan2025.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens. The flames screens must consist of a single screen of corrosion resistant wire of at least 30X30 mesh. Wasted flame screen on fuel tank vent.

A drydock examination conducted in compliance with 176.600. The following must be completed for credit drydock: a. Washed out welds on port and starboard stern tubes. b. Aft paddle wheel deck transverse frames found wasted, 4 on starboard side and 3 on port side. c. UT shots needed in the following locations: starboard side shell amidships, starboard bow, port bow, and paddle wheel decking. d. Side shell inset port side of stem. e. Clean through hull fittings of marine growth. f. Wastage on starboard side paddle wheel bearing housing. g. Port and starboard rub rail renewal including bow. h. Transverse frames in lazarette and sewage tank room. i. Longitudinal and vertical stiffeners on port side of sewage tank room j. Cap off old through bulkhead penetration between sewage tank room and engine room. k. Properly seal the penetrations on the port side of the collision bulkhead

The power fire pump shall be self priming and of such size as to discharge an effective stream from a hose connected to the highest outlet. Hose stream is ineffective to serve as a fire fighting means.

A length of fire hose shall be attached to each fire hydrant at all times. Fire hose is currently kept disconnected and stowed in a closet nearby. 181.15-10(g)

Unless otherwise authorized by the Coast Guard each device for which certification under this part is requested must meet the requirements of this subpart. The MSD pump is consistently leaking.

EAP trianing program: Supervisory personnel must receive at least 60 minutes of training. DAPI audit revealed supervisor has not received supervisory training.

Connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and fire-resisting properties of the cable. All connecting devices must be suitable for copper stranded conductors. There were melted wires around the exhaust of the A/C unit. There were dead end wires found in the forepeak, operating station, and store room. Repair, replace or remove melted wiring above exhaust. Properly insulate dead end wiring.

Each entrance to a space storing carbon dioxide cylinders or a space protected by carbon dioxide systems must be conspicuously marked as indicated in 46 CFR 185.612(g)(1) and (2). A/C room and engine room has no markings. Mark A/C room and both engine room doors as indicated above.

A life jacket must be provided for each person carried on board a vessel. Only 108 life jackets are on board vessel. Outfit vessel with the proper amount of life jackets.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Pump in A/C room missing guard, which exposes rotating machinery. Replace guard.

Al wires and cables shall be suitable for marine service and for the particular installation used, hot water heater pump switch is hung on the bulkhead with zip ties. Permanently mount electrical switch to the bulkhead. Old T cited not listed in MISLE; 46 CFR 183.10-20

Drip pans fitted with flame screens may be required under fuel strainers other than those mounted on the engines. Installed drip pans on the generator and port engine have holes. Repair replaced damaged drip pans. Old T cited not listed in MISLE; 46 CFR 182.20.40(b)(5)

The general construction of the vessel must be such as to minimize fire hazards in so far as reasonable and practicable. Engine room overhead has several sections of insulation missing. Install missing portions of insulation in overhead. Old T cited not listed in MISLE; 46 CFR 177.10-5

Each life jacket must be approved in accordance with subchapter Q specifications, or other standard specified by the Commandant. 99 life jackets had worn or missing retro reflective tape. Replace with the approved retro reflective tape.

When necessary for safe operation, the cognizant OCMI may place specific stability restrictions in a stability letter or on the Certificate of Inspection of a vessel not more than 65 feet in length. There was an excess of 13 chairs on the upper passenger deck than were allowed on the stability letter. Remove excess chairs.

The maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. The sign for the maximum passengers allowed on upper deck was painted over. Mark each entryway with the proper information.

Each stowage container for life jackets must not be capable of being locked. Container was capable of being locked. Remove the latch on the container.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Post instructions on the outside of the life jacket container.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Captain and two crewmembers were not on enrollment letter. Ensure all crewmembers are tested and on the consortium.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Mark the life jacket container on the upper deck with the proper information.

A hatch exposed to the weather must be watertight. Gasket on hatch to lazarette was missing. Ensure all gaskets on the hatches are in good working order.

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Bulkhead in aft pump room had a gap in the wall IVO electrical cord. Ensure bulkhead in aft pump room is made watertight.

Minimize accidental personnel contact with energized parts. There were several dead end wires in the sewage pump room. Remove all dead wires.

The general construction of the vessel shall be such as to minimize fire hazards. Vessel need to replace SFP over Starboard main engine and reattach SFP in port side of engine room. 46 CFR 177.10-5(A)

Wiring joints and splices shall be mechanically secure and made in junction boxes. Dead end wires in engine room in way of entrance need to be removed. 46CFR183.05-50(B)

Provide a minimum standard for electrical installations that will result in a vessel reasonably safe from electrical shock od personnel. Replace face plates on 02 electrical plugs in galley. 46 CFR 183.01-5

All vessels shall carry pyrotechnic distress signals as follows: three re hand flares are expired. 46CFR 180.68 (B)(1).

The WTDs below deck did not have indicators, on the bridge, to alert the master that the doors were open. The vessel has been this way since operating.

Vessel had a first aid kit but the contents did not have required equipment.

Label/seperate upper deck child PFD storage.

The upper deck had heaters mounted in the overhead that were plugged in vs wired into distribution panel.

Replace or repair frayed rope light at the gangway.

The watertight bulkhead between the fuel tank space and the storage void has a wire run penetrating that is not watertight.

Install inner cover on the main distribution panel on the port side of the A/C chiller room.

Repalce (03) outlet face plates in the galley.
