OCEAN INTRIGUE

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The high water level alarm panel at the operating station only produced an audible alarm with no visual indication. Correct the high water level alarm panel visual alarm at the operating station and test proper operation in the presence of the attending marine inspector. 46 CFR 182.530(a)

An adult life jacket must be provided for each person carried on board a vessel. There are only 126 adult life jackets on board. Provide 07 additional adult size life jackets to the satisfaction of the attending marine inspector. The passenger count is reduced to 122 total persons. 46 CFR 180.71

Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. The watertight bulkhead separating the port side fuel tank space and the generator room is penetrated by a flexible nonmetallic fuel hose. Reinstall the fuel piping to maintain the watertight integrity of the watertight bulkhead to the satisfaction of the attending marine inspector. 46 CFR 182.720(e)(3)(ii)

For each watertight door, an indicator must be installed in the pilot house and at each operating station from which the door is not visible. The indicator must show whether the door is open or closed. Port QAWTD door indicator illuminated when door is closed and extinguished when door is opened, which is the opposite operation of the starboard door indicator. Wire the QAWTD indictor’s operation so they operate in the same manner and prove operation in the presence of the attending Marine Inspector. 46 CFR 170.270(e) Old “T”

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Port and stbd engine ECU power fuses are the blade type. Install UL approved cartridge type fuses to the satisfaction of the attending marine inspector. 46 CFR 183.380(l)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 or be specifically approved by the Commandant. The following restrictions apply: Flexible nonmetallic hose must be complete with factory-assembled end fittings requiring no further adjustment of the fittings on the hose, or field attachable type fittings may be used. Hose end fittings must comply with SAE J-1475. Field attachable fittings must be installed following the manufacturer's recommended practice. If special equipment is required, such as crimping machines, it must be of the type and design specified by the manufacturer. If field attachable type fittings are used, each hose assembly must be individually hydrostatically tested to twice the maximum operating pressure of the system. The generator fuel hoses installed do not comply with the above regulation. Install new fuel hoses as prescribed by the regulation above to the satisfaction of the attending marine inspector. 46 CFR 182.720(e)(1)

Alcohol-testing devices must be used according to the procedures specified by the manufacturer of the testing device and by this part. Alcohol testing swabs have expired. Replaced in kind to the satisfaction of the attending marine inspector. 46 CFR 4.06-3(a)(2)

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. Stbd engine space has a hole in the bulkhead permitting the extinguishing agent to escape the protected space. Seal hole to the satisfaction of the attending marine inspector. 46 CFR 181.420(a)(4)

Each circuit breaker must meet UL 489 or other standard specified by the Commandant and be of the manually reset type designed for: inverse time delay; instantaneous short circuit protection; and switching duty if the breaker is used as a switch. The two push button circuit breakers mounted on the forward inboard bulkhead of the port engine space are not UL 489 approved. Install UL 489 circuit breakers with the correct amperage to the satisfaction of the attending marine inspector. 46 CFR 183.380(m)

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. The port and stbd overboard sewage valves cannot be secured as per 33 CFR 159.7(b)(1-4). Secure the sewage overboard valves using one of the methods described in the regulation to the satisfaction of the attending MI. 46 CFR 184.704

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Chaffed cable forward and inboard of the port battery box, the generator terminal bar are exposed, and the stbd fuel boost pump electrical connection is not insulated. Repair cable and provide a protective cover to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support, except that on vessels of not more than 65 feet in length, installations in accordance with paragraph 14.h of ABYC E-8 and paragraph 15.h of ABYC E-9 are acceptable as meeting the requirements of this section. Numerous new cables installations are not supported or bundled with plastic wire tie wraps. Make the cable installations meet the regulations above to the satisfaction of the attending marine inspector. 46 CFR 183.340(b)(4)

An exterior lighting fixture in an electrical system operating at more than 50 volts must comply with the requirements of UL 595 or other standard specified by the Commandant. A lighting fixture in an accommodation space, radio room, galley or similar interior space may comply with UL 1570, UL 1571, UL 1572, UL 1573, or UL 1574 as long as the general marine requirements of UL 595 are satisfied. There is, 1) A fluorescent light mounted on the stbd engine space outboard forward bulkhead; and 2) LED light bars and strip lighting in the accommodation space. 1) prove the lighting that is in compliance with the above regulation to the satisfaction of the attending marine inspector; or 2) replace with lighting comply with the regulation to the satisfaction of the attending marine inspector; or 3) permanently remove the lighting to the satisfaction of the attending marine inspector. 46 CFR 183.410(d)

A hatch exposed to the weather must be watertight. Gasket material for all six of the vessel's deck hatches are suspected of not providing a watertight seal. 1) conduct a hose test in the presence of the attending marine inspector to confirm a watertight seal; or 2) replace the hatch gaskets in-kind to the satisfaction of the attending marine inspector. 46 CFR 179.360

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected, and all required guards and protective devices must be in satisfactory condition. The stbd fuel vent 30x30 mesh screen was covered with paint. 1) remove paint from screen; or 2) replace the screen in-kind to the satisfaction of the attending marine inspector. 46 CFR 176.830(a)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Stern light is not located the center of the beam and showing an unbroken light over an arc of the horizon of 135 degrees and so fixed as to show the light 67.5 degrees from right aft on each side of the vessel. Make stern light meet as prescribed by 33 CFR 83.21(c) to the satisfaction of the attending marine inspector. 46 CFR 183.420

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessels from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. There are three surge protectors used to extend an outlet circuit. Two supply the power for the LED light bar and strip lighting in the accommodation space and one to extend a circuit in the overhead bar area. 1) remove the surge protectors extending the lighting circuit; 2) permanently wire the approved lighting fixtures in accordance with 46 CFR Subpart 183; and 3) submit plans for additional electrical outlets for review and approval to the satisfaction of the attending marine inspector for the equipment in the bar overhead. 46 CFR 183.200(b)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. The aft inboard port battery box switch cables and the port and stbd main battery terminal bars are missing a protective cover. Provide protective covers to the satisfaction of the marine inspector. 46 CFR 183.01-5 Old “T”

Portable fire extinguishers shall be installed and located to the satisfaction of the Officer in Charge, Marine Inspection. Operating station fire extinguisher is mounted in a void space with signage. However, there is a large speaker in the space and the mounting location is not readily accessible. 1) relocate the speaker and mount fire extinguisher so it is readily accessible; or 2) Relocate the fire extinguisher to the port side and mount to make it readily accessible to the satisfaction of the attending marine inspector. 46 CFR 181.30-10(b) Old “T”

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for EPIRB expired. 1) install new EPIRB hydrostatic release unit; or 2) remove EPIRB to the satisfaction of the attending marine inspector. 46 CFR 176.808(a)(5)

The intent of this part is to ensure that the auxiliary machinery is suitable for the purpose intended. It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. The flexible hoses on the port side fuel tank well past their service life and no-longer flexible or in good repair. Replace in-kind to the satisfaction of the attending marine inspector. 46 CFR 182.01-5(a) Old “T”

Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. The watertight bulkhead separating the port engine space and generator space is penetrated by a flexible nonmetallic fuel hose. 1) remove hose and seal bulkhead; 2) reinstall the fuel piping as prescribed by the regulation to maintain the watertight integrity of the watertight bulkhead to the satisfaction of the attending marine inspector. 46 CFR 182.720(e)(3)(ii)

An adult life jacket must be provided for each person carried on board a vessel. Vessel only has 114 serviceable life jackets. The vessel is restricted to carrying no more than 114 persons until additional required life jacket are provided. 46 CFR 180.71(a)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station. Vessel had audible but no visial indication for high bilge level alarm in starboard lazarette. 46 CFR 182.530(a)

Starbord forward hatch had one rubber T handle that was broken making the hatch not watertight. Make hatch watertight. 46 CFR 179.360

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Vessel's Certificate of Documentation is expired. Provide valid Certificate of Documentation. 46 CFR 67.313(a)

Fuses shall be standard National Electric Code types for the load to be carried and shall be of types listed by, or types equal to those listed by Underwriters' Laboratories, Inc., or other recognized testing laboratory. The overcurrent protection for the port and starboard motor are protected by unapproved blade type fuses. Install appropriate fuses to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 183.05-35(a) Old T

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Make repairs to the port and starboard MDEs as discussed. Conduct operational test to the satisfaction of attending Coast Guard Marine Inspector. 46 CFR 176.700(a)

The construction of lights and shapes and the installation of lights on board the vessel must satisfy the Commandant, U.S. Coast Guard. The sternlight of the vessel is obscured from view by the trampoline and paddleboards and will need to be moved to meet the requirements. Submit new proposed sternlight installation meeting 33 CFR 83.21(c) to USCG Commercial Vessel compliance for approval. 33 CFR 84.20

When operating a vessel on a body of water where the discharge of sewage is prohibited the operator must secure each Type III device in a manner which prevents discharge of sewage. The provided isolation valve was not positively secured. Provide an acceptable means to secure the discharge of both Type III devices to prevent discharge. 33 CFR 159.7

A vessel must comply with applicable requirements for any radio installation. The provided/required VHF radio was not properly wired and operated intermittently. Prove proper operation of the VHF radio. 46 CFR 184.502

All vessels must have lights that are in compliance with the applicable section of the NAV RULES. The vessel was not equipped with an anchor light. Provide an anchor light. 46 CFR 183.420

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard. Excessive fuel oil in the bilge in the STBD tank compartment. 46 CFR 176.830(b)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Two live, heavy gauge, dead-end electrical cables are in the port engine room vestibule. Remove or terminate in a suitable junction box. 46 CFR 183.200

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments. The vessel's fixed bilge system for the port hull was unable to satisfactorily dewater the port shaft compartment. Prove proper operation of vessel's fixed bilge system. 46 CFR 182.115

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The vessel's stern light is not visible in a 135 degree arc as required due to a large inflatable trampoline blocking its view. Make vessel's navigation lights visible as required. 46 CFR 183.420

At each inspection for certification and at every other vessel inspection, toilet and washing spaces, serving pantries, lockers, and similar spaces may be examined to to determine they are in a sanitary condition. Upon examination the are lockers containing the life jackets, the drink serving area, and the heads were infested with cockroaches. Provide a fumigation report from a reputable exterminator. 46 CFR 176.818

Portable fire extinguishers are required to maintained and inspected as required by Chapter 4 of NFPA 10. Portable extinguishers have not been inspected monthly and inspections have not been properly logged. Inspect portable extinguishers as required by Chapter 4 of NFPA 10. 46 CFR 176.810(b)(1)

The master of the vessel shall ensure that each EPIRB is tested monthly and that the required test is logged or otherwise documented, as applicable. There is no indication that the EPIRB has been tested and there is no log of such tests. Conduct monthly test of EPRIB and log as appropriate. 46 CFR 185.728

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of valve and direction of operation. The provided remote fuel shutoffs at both the port and starboard stations were not labeled. Properly label each remote shut off as required. 46 CFR 185.608

Accessories such as switches, fuses, and sockets, shall be standard National electric Code types for the loads to be carried and shall be of types listed by Underwriter's Laboratories, Inc, or other recognized testing laboratory. Discovered non-marine grade pump for attached slide. Remove pump configuration. 46 CFR 183.05-35(a)

Identified multiple exention cords chained together of non-marine grade. Remove extension cords. 46 CFR 183.05-35(a)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have a splice that is soldered, brazed, or welded to a flexible lead or conductor. Discovered improper splicing of wire under starboard side bridge console. Properly splice wire. 46 CFR 183.340(g)(4)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Passenger reportedly received an electrical shock upon holding onto the railing. Vessel is restricted from operating until deemed safe by the attending Marine Inspector, to include: determine, isolate, and/or repair the source of stray electric current to the satisfaction of the attending Marine Inspector. 46 CFR 183.01-5(a)

The movement of vessel shall be under the direction and control of the master at all times. The master shall operate the vessel keeping the safety of the passengers and crew foremost in mind by direction the vessel in order to prevent a casualty. The vessel's inflatable water slide restricts the visibility of the operating station. The inflatable water slide must be fully deflated while the vessel is in transit. 46CFR185.304(a)

For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. The aft mounting bracket for the forward trampolines are not properly secured to the vessel's hull. Properly secure the mounting brackets to the vessel to prevent failure. Passengers are prohibited from using the forward trampolines until repaired to the satisfaction of the attending Marine Inspector. "Old T" 46CFR176.25-1

At each subsequent inspection, the marine inspector shall require all observed hazardous situations be corrected. The mounting bracket for the forward fly bridge bench on the port side is not properly secured. Secure the bench to prevent passenger injury. "Old T" 46CFR176.25-50

At each subsequent inspection for certification, the marine inspector shall examine and testy all watertight closures in the hull, decks and bulkheads. The port and starboard hulls have open penetrations in the main deck for the anchor windlass. Install watertight covers over the penetrations to prevent water intrusion. "Old T" 46CFR176.25-5(a)(2)

For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. The vessel has an inflatable water slide installed potentially restricting the view of the operator. To mitigate this risk, the vessel must post a lookout when transiting with the water slide inflated. This deficiency will stand until the COI endorsement is finalized. "Old T" 46CFR176.25-1

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Engine belts and shafts missing covers. 46 CFR 177.960

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire. Bus bar in port engine room is missing cover. Protect electronics at operating station by making repairs to console so that it is watertight. 46 CFR 183.200 (b)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart. Many wires unsecured in port engine room and through-space between hulls connecting the compartments forward of engine room. 46 CFR 183.340 (b)(4)

Equipment installed on each vessel must be suitable for the purpose intended. Fuel filter pressure gauge in port engine room is inoperable. 46 CFR 182.100

Machinery and equipment not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The power conductor for the electric submersible bilge pump in the Port Shaft Alley compartment was wasted. Repair the conductor.

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. The hydraulic steering cross connect valve handle in the starboard steering compartment is nonfunctional due to corrosion.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. The generator's main positive, negative, and ground terminals are not properly covered to protect from accidental shock.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. The port aft high water light did not illuminate and the starboard lazarette audible alarm did not sound.

A hinged watertight bulkhead door must be fitted with a quick action closing devise operable from both sides of the door and indicator lights at the operating station showing whether the door is open or closed. The starboard engine room watertight door indicator is inoperable.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The port bilge vacuum pump is not properly connected to its mount.

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. The inboard overhead wire runs in the port and starboard engine compartment have wires that are not being supported by the installed metal supports.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. The starboard rudder grounding wire is broken.

A vessel of more than 19.8 meters (64.96 feet) in length must have at least one fire axe located in or adjacent to the primary operating station.

Each life jacket must be marked wuth Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified as IMO Resolution A.658(16); 85 adult and 7 child lifejackets had retroreflective material that was faded and no longer reflective. Add new retroreflective material. Passenger count reduced temporarily to 54 adults and 13 children. 46 CFR 185.604

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited; Vessel's batteries have many wingnut connectors. Replace wingnut connectors with permanent type connectors.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level; The number 2 (from fwd to aft) compartment on the stbd side's high water visual indicator did not operate. Prove proper operation of visual indicator.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: (1) A light to indicate discharge; (2) An Audible alarm that sounds upon discharge.;Port side Fireboy alarm did not operate. Stbd side Fireboy light and alarm did not operate. Prove proper operation of fireboy lights and alarms for port and starboard sides.

Deck rails on a vessel engaged in excursion trips, including but not limited to sightseeing trips, dinner and party cruises, must have the following installed between the top rail and deck: chain link fence or wire mesh that has openings not more the four inches in diameter, or bars, slats, rail courses or equivalent spaced at intervals not more than four inches. Vessel has rail courses spaced at 12 inches. Install chain link fence, mesh or courses to the satisfaction of the attending Marine Inspector. Submit plans to OCMI for approval prior to modifying vessel.

The name and hailing port of the vessel must be marked on some clearly visible exterior part of the stern. The hailing port of the vessel is not marked on the stern.

Vessel must be provided with 1 fixed bilge power pump and 1 portable hand pump in accordance with 46 CFR 182.520(a). Vessels starboard bilge pump is not in working order

On a vessel of at least 26 feet in length, a visual and audible indicator must be provided at the operating station to indicate high water level in each normally unmanned space with a through-hull fitting, machinery space, shaft alley, space with non-watertight closure and other spaces subject to flooding. Install and demonstrate operation of audible and visual indicators for each space required.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Install visual indicators for automatic bilge pumps.

Crew training shall be logged for review by the USCG upon request. Provide log of safety drills.

The master of the vessel shall ensure the EPIRB is tested monthly and each test is logged or documented. Provide log of monthly EPIRB tests.

Each battery used in lifesaving equipment must be replaced during the annual inspection if the expiration of the battery has passed or the battery is without an expiration date. Replace the battery in the life-ring floating light.

Employers must ensure that they and their crew members meet the requirements of part 16. Provide proof that vessel and crew members are in compliance with 46CFR16.

Provide valid registration of EPIRB.

Except when individual pumps are provided for separate spaces, individual suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve located at some accessible point in the bilge line. Install check valve in the bilge suction line for the port shaft alley space.

A vessel must comply with applicable requirements for any radio installations as set forth in 47CFR80. VHF-DSC shall be programmed with MMSI and integrated with GPS.

Provide FCC Safety Radio Certificate IAW 47 CFR 13

Portside engine room fireboy audible alarm inoperable at operating station. Repair audible alarm.

Install guards covering moving machinery in both engine rooms.

Provide drug testing program for each crewmember.

Provide properly equipped lifefloats, or, amend COI to limit vessel within 3 miles of shore

Provide 3 properly equipped life rings in servicable condition

Conduct a satisfactory fire drill including operation of both fire pumps.

Provide a properly operating stern light.

Provide an emergency broadcast placard at radio operating station.

Properly mount all fire extinguishers.

Cover start batteries

Provide a means to prevent PAX from accessing areas of the deck without lifelines.

Proove proper operation of rudders underway. Provide support bracket amidhsip for tiller tie rod.

Various handrails need to be repaired or replaced. (STBD AFT quarter, Aft ladder rail bolts)

Both garden hoses must be made of proper commercial grade.

Stbd engine room fire extinguisher must be accessible at all times.

Provide proper lifejacket storage number for every storage locker.

Both aft compartment bilge ovbd fittings need to have valves and be made of material of equitant strength of the hull.

Replace nonworking retro reflective tape on lifefloats.
