Life floats, when stowed in tiers, the separate units must be kept apart by spacers. Aft life float tier did not have spacers separating life floats. 46 CFR 180.137f

Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Aft life float tier has broken plastic paddle that needs replacement. 46 CFR 180.175f2

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. The forward tier of life floats contained two paddles without the vessel name marked, and three paddles on the aft life float tier were not marked with the vessels name. 46 CFR 182.604g

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel had no portable light at the engine room. 46 CFR 183.430

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Port generator coils are covered in oil posing a fire risk. 46 CFR 182.100

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Vessel master did not have a medical certificate. 705(c) Valid med certificate to be provided prior to crewmember acting as licensed mariner on vessels. 16 CFR 10.301

The owner of a vessel subject to inspection under Section 3301 shall record in a log book information on all access and use of the vessel's master key, including A)dates and times of access; B)the room or location accessed; and C)the name and rank of the crew member that used the master key. Vessel had no record log for master key control. Provide master key control as required. 46 USC 3106

Fire at sea, recommended emergency instructions. Cut off air supply to fire-close ventilators. Starboard aft engine room ventilation did not have damper to secure ventilation. 46 CFR 185.25-5(c)(1) OLD T

The pump outlet shall be fitted with a pressure gauge. Vessel did not have a pressure gauge fitted at the fire pump outlet. 46 CFR 181.10-1c

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. MI observed a hole in the port side overhead that penetrated from the galley to the boiler room. Submit Repair proposal to address the hole identified during inspection to the satisfaction of the MI. 46CFR177.100

Machinery spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads. The machinery space, containing a boiler and furnace, aft of and communicating with the overnight passenger accommodations were separated by a non-tight partial bulkhead that did not extend to the deck or overhead. 46 CFR 182.35-1 50c

An existing vessel with a ...a machinery space ...deck, composed of wood must comply with the requirements of § 181.400 of this part on or before March 11, 1999. The machinery space aft of the overnight passenger accommodations had a wooden deck and did not have fixed fire fighting or fire detection. 46 CFR 181.115 50c

...heating equipment shall be suitable for marine use. Vessel had a residential style oil fired furnace in the machinery space aft of the overnight passenger accommodations. Vessel plans were not available at this unit. 46 CFR 184.05-1 50c

..heating boilers and their associated piping and fittings will be given separate consideration and must meet the applicable requirements of subchapter F...of this chapter as determined necessary by the cognizant OCMI, except that heating boilers must be tested or examined every three years... Periodic inspection and testing requirements for boilers are contained in § 61.05 in subchapter F of this chapter. Evidence of the required testing was not available at the time. 46 CFR 182.310 46 CFR 176.182(b) 50c

all general areas accessible to passengers to have no less than 2 independent avenues of escape that are- (I) constructed and arranged to allow for free and unobstructed egress from such areas; (II) located so that if one avenue of escape is not available, another avenue of escape is available; and (III) not located directly above, or dependent on, a berth;PLAN REQUIRED.—Not later than April 1, 2024, an operator of an overnight fishing charter not in compliance with such section 3306(n)(3)(A)(v) shall submit to the Commandant a plan for complying with such requirements. Plan provided 3/19/24 was inadequate.EXTENSION.—On and after April 1, 2024, with respect to an operator of an overnight fishing charter which has submitted a plan for compliance in accordance with subparagraph (B), a captain of the port may extend essels described by 46 CFR 175.110(d) must comply with the regulations in § 177.500 not later than January 1, 2026.NOTICE TO PASSENGERS.—Beginning on the date on which the requirements under section 3306(n)(3)(A)(v) of title 46, United States Code, take effect, the owner or operator of a vessel for which an extension is granted under paragraph (1)(C) shall provide on the website of such owner or operator of the vessel, the vessel, and each ticket for a passenger a prominently displayed notice that the vessel is exempt from meeting the Coast Guard safety compliance standards concerning egress as described in such section. Notice not on website.

Vessels described by 46 CFR 175.110(d) must comply with the regulations in § 177.500. A ladder leading to a deck scuttle may not be used as a means of escape. The two required means of escape must be widely separated and, if possible, at opposite ends or sides of the space to minimize the possibility of one incident blocking both escapes. The secondary means of escape is a vertical ladder. Both escapes lead to the same enclosed space. 46 CFR 177.115 46 CFR 177.500(j) 46 CFR 177.500(b) 50c

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB hydrostatic release is due to expire at the end of the month. Install new hydrostatic release to the satisfaction of the attending inspector. 46 CFR 185.740(b)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jacket stowage on starboard side top deck was not segregated from adult life jackets. Segregate child and adult life jackets to the satisfaction of the attending marine inspector. 46 CFR 180.78

Electrical equipment on a vessel must be installed and maintained to prevent electrical ignition of flammable vapors. Generator windings on both generators are saturated with oil. Clean windings to the satisfaction of attending marine inspector. 46 CFR 183.200(d)

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Battery covers were not secured. Secure battery covers to the satisfaction of the attending inspector. 46 CFR 183.350(b)

The monitoring device(s) must: (1) Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive. Watchman device does not provide means to wake crew if unacknowledged. Submit plans for watch device that meets 46 CFR 185.410.

Wiring joints and splices. (1)Wiring joints and splices shall be mechanically secure and made in junction boxes. Wiring in bridge overhead has open junction box. Affix a junction box cover to the satisfaction of the attending inspector. 46 CFR 183.05-50(b)

Vessel missed required due date for plan submittal of monitoring device required by 3/28/2022. Granted 30 day extension for plan submittal. Failure to meet deadline will result in another 835 issued for no passenger overnight accommodations. 46 CFR 185.410(b)

The engine room emergency hatch enters a closet in the main deck accommodation area. Remove locks from closet door. Ensure gear stored in closet does not block this escape route. Mark the escape hatch and the closet door accessing the hatch with “Emergency Exit, Keep Clear” using at least 2-inch letters.

The 6-man life float was not connected to the vessel. Attach the 6-man life float to its own float free link with a 100 ft painter OR attach the 6-man life float painter around the body of the last 25-man life float in the adjacent stack. 46 CFR 180.130(a)(1)

Several minor issues were found on the vessel life floats: -Life float paddles are cracked -Life float grab lines are coiled using electrical tape -The painter on the forward life float stack is bundled using a zip tie -Retroreflective tape is faded in areas. 46 CFR 176.808(b)

Replace expired EPIRB hydrostatic release. 46 CFR 180.64

Two openings were found in the forward engine room watertight bulkhead above the switchboard. Make openings watertight. 46 CFR “Old S” 171.114(a)(2)

The remote shutoff valve for the starboard outboard propulsion engine could not be operated from the weather deck. Repair or replace valve. 46 CFR “Old T” 182.20-40(b)(3)

Repair holes and wasted area in port lazarette trunk.

Re-route fuel sounding pipe to terminate at the same height as the fill.

Stow Ballast in a way that prevents it from shifting. Remove ballast from lazarette and stow in accordance with stability letter. 46 CFR "Old S" 170.235. 50c

Repair/replace ammeter on starboard generator. 46 CFR "Old T" 183.10-5(g). 50c

:ine connecting water light to life ring shall be 3' to 6' long. It shall be connected to life ring body using a metallic clip. 46 CFR 180.79(d). 16c.

Draft a Station Bill for vessel detailing all requirements in 46 CFR 185.514. 46 CFR 185.514. 16c.

Designate all Senior Deckhands in writing in accordance with NVIC 01-91. 46 CFR 176.103. 50c.

Remove dead end wires from under the operating station as discussed. - 46 CFR 183.200(c)

Provide metal supports (no more than 24 inches apart) for the wiring underneath the operating station, so that it is no longer on the deck. - 46 CFR 183.340(b)(4)

Provide simple line diagram, over current protection used, and length and type of wire used in the installation of emergency lighting in the lower accommodation space. - 46 CFR 176.700(a)

AM01: Replace dry-rotted grablines on two liferings located on second deck.

AM02: Remove and replace expired flares.

46CFR176.830: Port side generator has oil leak and windings are oil soaked. Repair and clean to new condition. Do not use until cleared by Coast Guard Marine Inspector.

Port inboard engine exhaust leaking water from weld joint. Must repair.
