Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. USCG boarding team confirmed loss of propulsion aboard vessel. Repair engine and demonstrate operation to the satisfaction of the attending USCG marine inspector.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release is expired. Install valid hydrostatic release to satisfaction of attending marine inspector.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry: Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that:(1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113. UL 1113 24.1.1 requires automatic pumps are to be automatically cycled. Automatic float switch for #2 electric bilge pump does not activate pump. Repair or replace float switch to satisfaction of attending marine inspector.

Portable hand bilge pump must be capable of pumping water from all watertight compartments. Portable hand bilge pump does not take suction. Repair or replace portable hand bilge pump to satisfaction of attending marine inspector.

Station documents must be readily available to the licensed operator(s) on duty during the hours of service of the station and to authorized Commission employees upon request. FCC Station License, Safety Radiotelephony Certificate, and Marine Radio Operator permit not available to marine inspector during inspection. Provide required documents to attending marine inspector. 47 CFR 80.403

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. EPRIB registration not available to marine inspector during inspection. Provide current EPIRB registration to attending marine inspector. 47 CFR 80.1061(f)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Analgesic medication onboard expired. Provide 48 doses of analgesic medication to satisfaction of attending marine inspector.

The master of the vessel shall ensure that the EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Monthly EPIRB testing not being logged. Test EPIRB and establish monthly log to satisfaction of attending marine inspector.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Starboard engine raw water strainer not properly secured. Secured strainer to satisfaction of attending marine inspector.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator... Vessel did not have an operators permit on board. 47 CFR 80.159 (e)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Newly installed battery charger missing AC side circuit breaker. Install a UL 489 circuit breaker described by 46 CFR 183.380(m) of the appropriate size for the model of battery charger installed as per the manufacturers installation instructions to the satisfaction of the attending MI. 46 CFR 183.380

Newly installed battery charger missing fuse(s) for each positive conducter from the charger to the battery/ battery bank. Install the required cartridge type fuse(s) as described by 46 CFR 183.380(k)(1)of the appropriate size for the model of battery charger installed as per the manufacturers installation instructions to the satisfaction of the attending MI. 46 CFR 183.380(k)(1)

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2.5 gallon buckets, with an attached lanyard satisfactory to the cognizant OMCI, placed so as to be easily available during an emergency. The words "FIRE BUCKET" must be stenciled in a contrasting color on each bucket. Vessel has no fire buckets of the appropriate size on board. Put three fire buckets on board of the appropriate size with lanyards and the proper labeling. 46 CFR 181.610

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all the specific inspections and drills required. Unable to test if generator is operable and if it shuts down with activation of Fireboy. Demonstrate to attending marine inspector that generator is fully operable and will shut down with activation of Fireboy or completely abandon generator. Generator should not be operated while vessel is underway until deficiency is cleared. 46 CFR 176.404(a)

Provisions must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Fitting used to close starboard engine supply does not sufficiently close the supply duct. Install fitting that properly closes supply duct for starboard engine. 46 CFR 182.465(h)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Bow compartment high water level alarm did not provide visual or audible alarm. Aft compartment alarm did not provide visual alarm. Demonstrate to the attending marine inspector operation of visual and audible alarms for high water levels for all required spaces. 46 CFR 182.530(a)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel's alcohol test strips were all expired. Demonstrate to attending marine inspector that vessel has sufficient alcohol testing devices that are non-expired. Vessel route is restricted to no more than 3nm from shore. 46 CFR 4.06-15(a)(1)

Each life float and buoyant apparatus must be stowed as required under this section. The float-free link required by 180.130(a)(1) must be certified to meet 160.073 in subchapter Q. 2 of 3 life float float-free links are missing certification tags. Install 2 new float-free links with appropriate certification tags. Until the life float arrangements meet the regulations the vessel is restricted to 3 nm from shore. 46 CFR 180.137(b)(1)

The equipment required for life floats and buoyant apparatus must meet the following specifications; The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The batteries in the floating water lights are not of the approved type and do not work. Install approved batteries. Until the life float arrangements meet the regulations the vessel is restricted to 3 nm from shore. 46 CFR 180.175(f)(4)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. All three life floats have superficial cracking on exterior. Additionally, three paddles have significant cracking throughout the structure of the paddle. Install three new paddles and make repairs on life floats to the satisfaction of the attending marine inspector. Until the life float arrangements meet the regulations the vessel is restricted to 3 nm from shore. 46 180.175 46 180.175

A submersible electric bilge pump may be used as a power bilge pump, provided that the pump is listed by an independent laboratory as meeting the requirement in UL 1113 and the pump is permanently mounted. Bilge pumps in bow compartment, amidships compartment, and aft compartment do not meet UL 1113. Engine Room compartment bilge pump is not permanently mounted. Replace unapproved bilge pumps with UL 1113 approved bilge pumps and permanently mount engine room bilge pump. 46 CFR 182.520

Battery chargers must have an ammeter connected in the charging circuit. Battery charger does not have an ammeter connected in the charging circuit. Demonstrate to attending marine inspector that ammeter is wired into battery charging circuit. 46 CFR 183.350(f)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Sea strainer for starboard engine raw water intake is not properly mounted. Properly mount starboard engine sea strainer. 46 CFR 176.830

An EAP training program must be conducted for the employer's crew members and supervisory personnel.Supervisory personnel must receive at least 60 minutes of training. Vessel Captain unable to provide proof of 60-minute supervisory training. Provide proof of supervisory training to attending marine inspector. 46 CFR 16.401(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List, tide tables, and current tables. Vessel did not have Coast Guard Light List and U.S. Coast Pilot on board. Demonstrate to attending marine inspector that all required nautical publications are on board. 46 CFR 184.420

Conductors must be protected in accordance with their current carrying capacities. Fuses installed in bilge pump circuit were observed as 20 amp fuses, when manufacturer calls for 15 amp overcurrent protection. Install properly sized fuses in bilge pump circuit in accordance with manufacturer's instructions. 46 CFR 183.380(d)

A vessel must comply with the applicable requirements for any radio and EPIRP installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. No in-line fuse protection was installed for the radio installation. Install proper in-line fuse protection for the radio to the satisfaction of the attending MI. 46 CFR 184.502

A vessel must carry one floating water light, unless it is limited to daytime operation, in which case no floating water light is required. (1) Each floating water light must be approved in accordance with § 161.010 in Subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110.The battery in the floating water light is not of the approved type. Install approved battery to the satisfaction of the attending MI. 46 CFR 180.70(d)(1)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Newly installed battery charger is not grounded. Ground battery charger as appropriate for the installation as described in the manufacturers installation instructions. 46 CFR 183.200(b)

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Aspirin and inhalant medicine expired. Replace. 46 CFR 184.710

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. #3 aft audible bilge alarm is not operational. Demonstrate operation to attending marine inspector. 46 CFR 182.530(a)

Nonmetallic piping penetrating required watertight decks must be accomplishing using an acceptable metallic through deck fitting that is welded or otherwise attached to the deck by an accepted method. The vessel is equipped with a nonmetallic vent pipe that penetrates the weather deck. The penetration is sealed only with a marine putty. Provide adequate fitting for nonmetallic piping penetration. The vessel being FRP, a material approved by the attending marine inspector, of equal or greater strength than the hull material may be used in lieu of metal. 46 CFR 182.720

A vessel must comply with one of the structural standards listed. The deck longitudinal beam on the port side of the second compartment aft in way of the nonmetallic vent is rotten. Crop and renew in accordance with the vessels approved plans and accepted repair practices. Repair by completion of the vessel's next scheduled drydock examination. 46 CFR 177.300

Waterlights should be serviceable and batteries replaced annually if not marked with an expiration date. Waterlight was inoperable. Provide serviceable waterlight with battery in regulation. Vessel limited to daylight operations only until rectified. 46 CFR 185.726 (e)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. 47 CFR 80.903

Electrical equipment on a vessel must be installed to protect the vessel from electrical hazards, including fire. Both aft batteries require renewal of cables at terminals and switches due to corrosion. Remove or cap dead-end speaker wires. 46 CFR 183.200(b)

Every vessel must be fitted with a suitable magnetic compass designed for marine use, compass must be illuminated or restricted to daytime operation. Compass light not observable due to daylight. Provide recent photograph of vessel's operable compass light, taken at night. 46 CFR 184.402

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Vessel rep claims crew enrollment in consortium, but could not provide required associated forms. Provide satisfactory evidence of a chemical testing program and proof that all crew members have passed pre-employment testing. 46CFR16.230(a)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in 4.03-2 of subchapter A of this chapter shall comply with the requirements of 4.06 in subchapter A of this chapter. No test strips located onboard vessel. Provide sufficient means to test for presence of alcohol. 46CFR185.212

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Vessel's red hand flare distress signals are expired. Replace. 46CFR180.68

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Provide evidence of monthly checks. 46CFR176.810(b)(1)

Vessel's certificate of inspection must be posted under glass or other suitable transparent material such that all pages are visible and readily available for use by the crew and display to passengers and others on request. Vessel had original COI onboard, but not posted. Post COI so that all pages are visible. 46CFR176.302

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and scenario and training topics. Vessel unable to provide drill/training logs. 46CFR185.520(f)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel unable to provide logs for maintenance of survival craft. 46CFR185.702(d)

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and general description of the drill scenario and training topics. Vessel unable to provide logs for fire fighting drills and training. 46CFR185.524(d)

The EPIRB test required by paragraph (a) shall be logged or otherwise documented. Vessel unable to provide log for EPIRB tests. 46CFR185.728(c)

An adult life jacket must be provided for each person carried on board a vessel. Retro-reflective tape on six life jackets in disrepair. Vessel is restricted to 47 persons total due to lack of properly outfitted life jackets. 46CFR180.71

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master unable to provide medical certificate. 46CFR10.301(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for EPIRB expired. Install new hydrostatic release. Battery expired. Install new battery. Vessel restricted to within three miles of shore. 46CFR176.808(a)(5)

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Replace expired first aid kit items to the satisfaction of the attending USCG MI. 46CFR184.710

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Repair broken waterlight bracket. 46CFR176.808(b)

Electrical equipment on a vessel must be installed to protect the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Cable laying in lazarette bilge covered in water. Clean up bilge and secure wire to bulkhead using metal supports. 46CFR183.200(b)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Properly mount port M/E raw water strainer. Properly mount salt water deck wash pump. Properly secure pully protector, which fell forward when vessel got underway. 46CFR182.100

A hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Provide hatch cover attachment device. 46CFR179.360

Life floats must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers. Life floats were not kept apart by spacers. 46CFR180.137(f)

Each paddle required for life floats must be at least four feet in length, lashed to the life float to which it belongs and buoyant. Life float paddles not lashed to the life floats. 46CFR180.175

All vessels must have operable navigation lights in accordance with the Navigation Rules. Masthead light inoperable. Vessel has operable combination anchor/masthead light right above that could be used. 46 CFR 183.420

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI & LAT/LONG for DSC operation. 47 CFR 80.905

Except as provided in 80.13(c), stations must have an authorization granted by the Federal Communications Commission. Vessel did not have current FCC station license. 47 CFR 80.405 (a)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart. Wires and control box for security panel unsupported. 46 CFR 183.340 (b)(4)

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of 111.60-13 in Subchapter J. Vessel has extension cord running to a refrigerator, extension cord not approved use under Subchapter J.

An exposed hazard such as gears or rotating machinery must be properly protected by a cover, guard, or rail. Add guard cover or rail to exposed portions of both MDEs. 46 CFR 177.960

A vessel must carry one floating waterlight unless it is limited to daytime operation, in which case no floating waterlight is required; provide floating waterlight for ring life buoy. 46 CFR 180.70 (d)
