The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: the ignitability of combustible materials shall be restricted. Several oil soaked absorbent pads across were found on the bottom two decks of the bow thruster compartment spanning 30 sq/ft posing a fire hazard. Remove absorbent pads.

For periodically unattended machinery spaces, the Administration shall give special consideration to maintaining the fire integrity of the machinery spaces. The Class A fire door from the engine room into the fan space improperly closed and was unable to latch from self-closing configuration.

Electrical installations shall be such that: electrical services essential for safety will be ensured under various emergency conditions. Various emergency lighting bulbs were found extinguished during the ship’s walk through round including in the bow thruster space, laundry, and the ships office.

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. The vessel had a container arranged aft of the house utilized as a workbench with 4 daisy chained portable electrical extension cords powering multiple lighting installations from a shared power strip into a shared outlet junction positioned beneath the container posing a fire hazard. Replace portable electrical installation with a fixed configuration appropriate for the electrical draw of the lighting installations.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The #3 cargo hatch cover on the starboard side at bay #10 had air hissing from the hatch gasket and requires investigation if seal is fit for purpose or requires servicing/replacement.

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. Attending MI’s noted that plastic garbage cans were utilized for the storage of flammable or combustible rags in the Paint Locker.

Adequate arrangements shall be in place to ensure performance of equipment is maintained. MIs noted bridge panel for light controls with multiple bulbs burned out and no ability to distinguish which deck lights were energized. Satisfactory repairs w/in 30 days to RO.

Inspections of the fire safety systems and appliances shall include review of fire control plans and stowage of gear as approved. MIs noted discrepancies with the classification of spaces (slop stores listed on plan but was the fire locker (FE - FSD 616 main deck). Fire control plan shall be revised and equipment stowage verified by RO.

Conditions of ship shall be maintained to ensure safety and fitness to proceed to sea. Main engine room overhead hatch (main deck) found with wasted corner and unauthorized temporary repair. RO to verify condition and acceptance of temporary repair while awaiting crop/renew (concerns w/ hot work on deck due to cargo load).

Fireman's outfits shall consist of boots of rubber or nonconducting material. Boots for FF outfits found wasted and not in working condition. Damaged boots shall be identified for all suits and replaced with adequate sizes for the assigned fire teams.

Machinery, associated piping systems and fittings shall be adequate for the service for which they are intended. MIs noted Ballast/FM control panel in CCR with numerous burnt bulbs, valves in open & closed condition error, handwritten notes for missing valves in diagram, and faulty remote controls. System repairs & service tech support is required for system grooming and restore remote controls.

At least two independent means shall be provided for communicating orders from the nav bridge to the position in the machinery space where the engines are normally controlled; one of which shall be an engine-room telegraph (EOT). MIs noted alignment issues with the EOT controls during testing between the bridge and ECR. Satisfactory repairs due to correct EOT w/in 30 days.

A self-contained emergency source of electrical power shall be provided in cargo ship. MIs noted emergency generator issues during operation requiring replacement of fuel supply hose.

Fixed fire detection required and shall provide clear information to each indicating unit about the spaces covered and location of the sections in alarm. Ship self-reported fire detection panel in ECR w/ nuisance summary alarm impacting ACCU alarms and notification of fire zone impacted. Technician support require to clear ECR fire summary alarm.

Ventilation shall be maintained in working order for machinery spaces. Ventilation fan in the main deck (aft) battery room was found inop and requires replacement w/ adequate size motor.

Condition of ship shall be maintained. Ballast vent piping (specifically Cargo holds 6/7) found w/ pin holes & deteriorated condition. Vent pipes shall be repaired.

Condition of ship shall be maintained. WTD 806 (Hold 6/7 starboard) found with separated frame and spray foam applied to fix. Door frame adjustments required or new frame for installation.

Electrical equipment and wiring shall not be fitted in enclosed cargo spaces unless it is essential for operational purposes. If electrical equipment is installed in dangerous environments then it shall be of a certified safe type (intrinsically safe/explosion proof) unless capable of completely isolating energy source. Cargo holds are used for the storage of IMDG class I cargoes (explosives) - and holds are classified as Class I/Division I space. IAW DOT SP 9211, modifications to 49 CFR requirements for carriage of dangerous goods under military contracts, allowances are made for electrical equipment in hazardous locations (no mention of certified safe type) but stipulates that equipment must be totally isolated while cargo is being loaded/onboard/discharge. MIs noted cargo hold electrical equipment with multiple issues: light enclosures damaged, missing latches for explosion proof enclosures, dead end wiring, sealing glands exposed, wiring for temperature/humidity sensors exposed. Thorough inspection of electrical equipment in cargo hold spaces required with satisfactory repairs to restore condition of equipment/wiring. Cargo hold lighting to be isolated (breakers locked) until repaired

Safety management system onboard shall be maintained iaw ISM code. MIs noted discrepancies with ship's lock out/tag out procedures for equipment onboard. RO to verify implementation of lock out/tag out SMS procedures onboard and effectiveness of program.

All ships of 300 gross tonnage and upwards be fitted with an echo-sounding device, or other electronic means, to measure and display the available depth of water. MI observed the display was not readable. IAW 33 CFR164.53 a Letter of Deviation is required by the OCMI of the vessel's next U.S. Port. 74 SOLAS V/19.2.3 16A

Maintenance, testing, and inspection of life-saving appliances shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring reliability of such appliances. MI observed fiberglass damage to the Port Life Boat. The damage was on the fwd Starboard side. MI observed a quarter size hole on the cabin top on the Starboard side Life Boat. MI observed an engine exhaust leak on the Port side Life Boat. 74 SOLAS III/ 20.3.1 16A

Machinery shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. MI observed an exhaust leak on the Port side Life Boat. 74 SOLAS II-1/26.1 16A

Inspected vessels are issued a Certificate of Inspection (COI) which indicates the minimum complement of officers and crew (including lifeboatmen) considered necessary for safe operation. The vessel's COI serves as the minimum safe manning document under SOLAS Chapter V, Regulation 14. A person may not employ or engage an individual, and an individual may not serve in a position in which it is required by law or regulation that the individual hold an MMC endorsed with a national endorsement, as well as a corresponding STCW endorsement for service outside the boundary line per 46 CFR 15.401. Each person serving as an able seaman on a seagoing vessel subject to the STCW Convention must also hold an STCW endorsement as able seafarer-deck. Each person serving as an RFPNW on a seagoing vessel of 500 GT or more, subject to the STCW Convention (incorporated by reference, see § 15.103 of this part), must hold an STCW endorsement attesting to his or her qualifications to perform the navigational function at the support level. The vessel’s COI requires a minimum of (3) Able Seaman and (3) additional Able Seaman are required under the “Conditions of Operation” tab as maintenance personnel. (02) of the maintenance Able Seaman are permitted to be Ordinary Seaman so long as they are endorsed for STCW II/4, rating forming part of a navigation watch, and also have the minimum training identified in NVIC 3-83. The vessel is currently manned with (03) Able Seaman and (02) Ordinary Seaman. The (02) Ordinary Seaman do not have the STCW II/4 endorsement for RFPNW. The vessel is sailing short of the minimum manning complement required by the COI. (03) additional Able Seaman personnel or (01) Able Seaman and (02) Ordinary Seaman with appropriate STCW endorsements shall be added to the manning complement in accordance with the COI prior to sailing on a foreign voyage. It is the responsibility of the owner, charterer, managing operator, master, or person in charge or in command of the vessel to ensure that appropriate personnel are carried to meet the requirements of the applicable navigation and shipping laws and regulations per 46 CFR 15.105. 74 SOLAS (20) V/14 702 C

After any survey of the ship under Regs 7,8,9, or 10 of this Chapter has been completed, no change shall be made in the structural arrangements, machinery, equipment, etc. covered by the survey, without the sanction of the Administration. Identified a corroded and holed cargo tank longitudinal stiffner on the starboard side of the #7 cargo tank top. Corrosion and three holes measured approximately 1" in diameter. 74 SOLAS (20) I/11 (b) 16 A

All exposed metal parts of electrical machines or equipment which are not intended to be live but are liable to become live under fault conditions shall be earthed and all electrical apparatus shall be so constructed and so installed that danger of injury in ordinary handling shall not exist. Identified missing or incorrectly installed front covers to the distribution panels located in the port tunnel next to cargo tanks 7 and 8. Electrical conductors were found exposed which may cause injury if live and contacted by personnel. 74 SOLAS (81) II-1/ 27 16A

The machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended... Identified main diesel engine jacket water pump mechanical seal leaking jacket water. 74 SOLAS (81) II-1/26.1 16 A

In all ships, fire-extinguishing appliances shall be kept in good order and available for immediate use at all times during the voyage. Identified missing safety caps on the head units throughout the ships fixed CO2 systems. 74 SOLAS (81) II-2/21 16A

Exposed metal parts of electrical machines or equipment which are not intended to be live but which are liable under fault conditions to become live shall be earthed. MI observed a ground fault on the 24VDC panel on the switchboard located in the Emergency Diesel Generator compartment. 74 SOLAS (81) II-1/45.1.1 16 A

In all ships, fire extinguishing appliances shall be kept in good order and available for immediate use at all times during the voyage. MI observed #1 fire pump check valve located on the outlet side leaking at the flange. Ships crew identified three loose bolts on the flange and tightened correcting the leaking flange on the spot. 74 SOLAS (81) II-2/21 10 C

A main electrical lighting system which shall provide illumination throughout those parts of the ship normally accessible to and used by crew shall be supplied from the main source of electrical power. MI observed an extinguished overhead light located at the emergency fire pump electrical panel. In addition, MI observed protective cover for light fixture hanging and broken located in the overhead behind the Emergency Diesel Generator switchboard. 74 SOLAS (81) II-1/41.2.1 16A

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. MI observed multiple extension cord daisy chains for both battery chargers running on the deck and not properly installed in the Emergency Diesel Generator compartment. 74 SOLAS (81) II-1/45.1.3 16 A

Maintenance of condition after survey. Vessel experienced a loss of propulsion, due to jacket water pressure drop on the #l ME cylinder, while departing the Port of Tampa. Vessel shall seek safe transit to nearest anchorage for proper assessment. Vessel shall remain at anchorage until the proper operation and/or repairs of main engine are made satisfactory to attending surveyor. Vessel must contact Sector St. Petersburg prior to departure from anchorage. SOLAS 81 Amend Ch 1 Reg 11

Every lifeboat shall be arranged that an adequate view forward, aft, and sides is provided. Both port and stbd lifeboats have fwd facing windows that are milky white and visibility is not currently possible.

Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault, precautions against such risks shall be taken to the satisfaction of the administration. Wiring going into lighting appliance no longer electrical rated for haz location. Replace/repair lighting appliance in paint locker IAW OEM.

6-man liferaft forward is mounted on a pedestal across an opening approx 1 meter wide and 2 meters high, making it a danger to safely launch. Raft to be moved to a suitable location for safe handling/launching.

The vertical A/C ducts into cargo holds require a dedicated tool kit in the immediate vicinity, to allow for the quick removal of ducting and closing of the vent.
