No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the officer in charge, Marine Inspection. Drawings of alterations shall approved before work is started unless deemed unnecessary by the Officer in Charge, Marine Inspection. The barge has started and still has planed extensive steel repair work, replacement of nearly all electrical wiring, structural fire protection replacement, and all finishing work. The amount of steel repair may be considered to be substantially prolonging the life of the vessel. Submit plans detailing all completed and planned repairs to be conducted on board the vessel in accordance with MSC Plan Review Guide GEN-06 as provided to the vessel representative via email on this date. 46 CFR 91.45-1 46 U.S.C. 2101(18)

The electrical wiring in the accommodation space was removed and replaced prior to informing the OCMI. The vessel rep was unable to provide the latest approved plans for the accomodation space to verify repairs were made in kind. If the latest approved plans are unable to be submitted to the OCMI, new plans must be submitted to Marine Safety Center.

For industrial vessels of 300 gross tons and over but less than 4,000 gross tons, contracted for prior to July 1, 1968, which carry in excess of 12 industrial personnel, existing structure arrangements and materials previously approved will be considered satisfactory so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. Minor repairs and alterations may be made to the same standard as the original construction. Major alterations and conversions shall be in compliance with this subpart to the satisfaction of the Officer in Charge, Marine Inspection. Multiple modifications have been made to the vsl w/o oversite by USCG. Rep was unable to provide the latest approved plans to verify the structural fire protection needed.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. The entire vessel shall be made available to marine inspectors for inspections. Confined spaces must be gas-freed by a Marine Chemist within 24 hours of inspection until the shipyard has a Shipyard Competent Person program that can be verified by MSU Morgan City's office. DHD representatives have been given a check list of items covered under the Marine Safety Manual Vol. I labeled "SWP # 100" that will need to be verified prior to maintaining a Marine Chemist Certificate more than 24 hours.

There shall be at least two means of escape accessible where the crew may normally be employed. Escape hatch leading from machinery space is inaccessable due to the unsatisfactory materiel condition.

Barge did not have full crew complement. Conduct fire, man overboard, and abandon ship drills with full crew complement.

All life rings throughout the barge were not serviceable. Service or replace all life rings.

Liferaft launching placard is missing from aft liferaft installation and must be mounted.

Portable fire extinguisher in control tower and semi-portable fire extinguishers in machinery spaces and at helideck need to be properly stowed in their brackets, free of obstructions.

Six portable fire extinguishers in and around the crane are out of service and need to be serviced.

Fire hose at helideck exit needs to be properly stowed.

Two fire hose nozzles are inoperable and need to be repaired or replaced.

Entire fire and safety plan needs to be re-walked, updated, and re-approved.

Distribution panel in control tower is not dead-front type and needs blanks installed.

Electrical installation in control tower and at helideck is not acceptable and needs to be cleaned up.

Reach rods for fuel shutoffs need to be made operable.

Three emergency lights (two at helideck and one in machinery space) are inoperable and need repaired or replaced.

Lighting lens at helideck exit is broken and needs to be replaced.

The general alarm does not operate under emergency power and must be made operable.

All general alarm contact makers require servicing to ensure proper functionality (i.e. capable of being exercised and automatically held in place when pushed to hold back).

An outlet in the laundry room is uncovered, exposing dead end wires. Dead end wires need to be properly capped, and the outlet needs to be covered.

Light cover outside weathertight door exiting accommodation space is broken and needs to be replaced.

All extension cables and surge protectors throughout accommodation spaces need to be removed.

Ventilation shutdown switch in galley needs to be replaced with a proper switch and properly labeled.

Barge service generators are inoperable. Generators need to be inspected and tested under load.

Handrails outside weathertight doors in accommodation and machinery spaces are wasted and need to be replaced.

Distribution panel missing screws & unsecured in galley. Replace screws & secure panel.

Missing vent screen on stbd main deck. Replace & install new vent screen.

Insulation in refer deck deteriorated. Replace insulation.

Unsecured sink in lavatory space. Secure sink to the bulkhead.

Liferafts were unavailable during inspection. Conduct liferaft inspection when they return from being serviced.
