Vessels must have sufficient starting air compressor capacity to ensure the main engines and emergency equipment can be started as required. The air starting system must be capable of at least 6 starts without recharging the air receivers. ABS Rules Part 4 9.5 ISM Code Part A Reg 10.1

Means must be provided to prevent over pressure in any part of the compressed air system. The number 1 air compressor was found in condition that it would not shut-off automatically and required constant manning to operate. ABS Rules Part 4 9.1.3 ISM Code Part A Reg 10.1

Ships are required to have at least two independent source of electrical power and an emergency source of power. Number 2 generator could not be started using ships air during inspection.

Each bus-tie between a main switchboard and an emergency switchboard must be arranged to prevent parallel operation of the emergency power source (Generator 1) with any other source of electric power, except for interlock systems for momentary transfer of loads.

Each rescue boat launching appliance shall be fitted with a powered winch motor capable of raising the rescue boat from the water with its full compliment of persons and equipment at a rate of not less than .3 m/s. Davit was only capable of lifting unloaded rescue boat intermittently, requiring long pauses before raising could be achieved again. SOLAS III LSA 6.1.1.9 ISM Code Part A Reg 10.

Every launching appliance shall be fitted with brakes capable of stopping the descent of the survival craft or rescue boat and holding it securely when loaded with its full complement of persons and equipment. Between intermittent raising of unloaded rescue boat, the brake was not holding securely and the boat would lower several feet.

The Administration, shall periodically verify the functioning of the ship’s safety management system. In accordance with vessel’s SMS casualty reporting; procedure 7.1-002-ALL, CASREPS shall be submitted not later than 24 hours after it is determined a casualty cannot be corrected within 48 hours. Vessel drafted CASREP, but due to 5th fleet requirements of editing CASREP before release, CASREP was not released within 24 hours. SOLAS IX, Reg 6.1 ISM Code Part A Reg 10.1

The Coast Guard issues the following endorsements for towing vessels: (1) Master of towing vessels. (2) Master of towing vessels, limited. (3) Mate (pilot) of towing vessels. (4) Apprentice mate (steersman). (5) Apprentice mate (steersman), limited Vessel master and only 1 mate are endorsed for towing. Vessel shall not perform towing evolutions unless properly manned. 46 CFR 11.463

The public address system must be reliable and capable of broadcasting emergency announcements throughout the ship from the bridge. System currently not functioning from the bridge. ABS Part 4 11.5.4

A powered watertight door must emit a visual and audible alarm when it begins closing. Watertight door in engine room had no visual alarm. SOLAS II-1, Reg 13

Remote fuel shutoff valves must be installed to stop the flow of fuel to equipment in event of a fire. The remote fuel shutoff valve for the #6 tank was found disconnected. SOLAS II-2, Reg 4

Electrical installations must be installed to prevent fire and electrical shock hazards. Electrical connection found on forward port side bulkhead in main engine space outside of an approved junction box. SOLAS II-1, Reg 45

SMS external audit required prior to departure from port. ISM Code Part A Reg 10.1

Vessel's no.3 fuel tank in engeineers workshop has pinhole leak at a rate of approximately 0.5gal/min.

A60 integrity of the bulkhead at the no. 3 fuel tank was compromised to locate source of fuel leak.

Smoke detectors located above the starboard main engine and between diesel pumps in the pump room were found covered in plastic. This will require an internal ISM audit.

(02) ABs do not meet STCW requirements for underway watch. The vessel is currently moored with no orders for getting underway, but will require sailing short notification if activated before requirements are met.

General condition of hospital space was not satisfactory.

Fuel piping flange was improperly installed, creating leak of approximately 0.5 gal/min into the engine room.

Protection Against Shock. General examination of safety precautions against shock, fire and other hazards of electrical origin. • Port side bulkhead forward of GMDSS, multiple electrical hazards: splices, missing covers, daisy chains power cord/power strips • Daisy chain power strips for coffee mess, and inboard bulkhead, to include dead-end wires for helo announcing system. • Daisy chain power strip in galley for food prep electrics. • Daisy chain power strips in stores office below galley. • Port side power strip of the non-marine type (orange) routed from port aft of bridge to outside deck. • Dead-end wire for GPS cable at chart table. • Extension cord through port side bridge vent. • Extension cord plugged into light in head (forward of galley), connected to another extension cord, through hole drilled in bulkhead into DC locker. • Exposed wiring on overhead just aft of house. • Light in galley never secured when power was secured for fire drill.• Two dead-end wires protruding from deck on inboard side of rescue boat. • Dead-end wire on port side light for rescue boat. Appears to not be connected to light fixture. • Portable extension cord from starboard side outlet on main permanently routed through overhead to starboard side tool shop, hooked up to a power strip. • Broken light fixture just aft of house. Light is hanging from wire. • Electrical wiring in DC closet overhead not properly secured. • Electrical wiring in overhead of Port workshop secured by plastic zip ties. • Multiple non-marine grade power strips identified on board (DC Locker). • Fans in engine room run with extension cords. • Light in EOS above engine control station hanging by rope • Electrical cable splice through deck outboard of gangway (stbd side) • Steering flat multiple cables with insulation failure • Steering flat junction boxes covers missing • Broken lens/diffusers for lights in steering gear • Grounding cable near port/stbd steering disconnected The SMS indicated that only one shipboard certified power strip was authorized per outlet. In and from all …crew spaces and in spaces in which the crew is normally employed, other than machinery spaces, stairways and ladder shall be arranged to provide ready means of escape to the lifeboat and liferaft embarkations deck. Laundry room egress blocked by mop and gear. Rectified then blocked again. Cite: 74 SOLAS II-2/Reg 21(a)

Fire doors…in stairway enclosures…shall be of the self-closing type. The release mechanism functioned properly when operated, but the fire doors did not close properly. Cite: 74 SOLAS II-2/Reg 37(f)

The training manual…shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided in the ship and on the best methods of survival. SOLAS training manual was missing lifejacket donning instructions for all types on board (3 types). Cite: SOLAS (2009) III/Reg 35.3

In all classes of watertight doors indicators shall be fitted which show, at all operations stations from which the doors are not visible, whether the doors are open or closed. The watertight door position indicator for the hand-operated location on the main deck is faded. Cite: 74 SOLAS II-1/Reg 13 (f)(5)

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Danger tag on valve at stbd bunker station is not filled out and was not in Lockout Tagout (LOTO) logbook. Breaker CKT 6P-415 (forward of EOS), tagged out not in LOTO logbook. Verification, as far as practicable, that the remote controls for stopping fans and machinery and shutting off fuel supplies in machinery spaces are in working order. Emergency fuel shut offs not marked, and key not identifiable. Fuel tank reach rods for #1 and #2 fuel tanks were disconnected. Ships shall be fitted with two means of communicating orders from the bridge to the engineroom. The 1JV is not connected to control station, the wire is not permanently mounted, and the wire is separating from the gland. Cite: 74 SOLAS II-1/Reg 32

The AIS Pilot Plug on any vessel subject to pilotage by other than the vessel Master or crew must be readily available and easily accessible from the primary conning position of the vessel and permanently affixed (not an extension cord) and adjacent (within 3 feet) to a 120-volt 50/60 Hz AC power receptacle (NEMA 5-15). AIS pilot plug requires installation. 33 CFR 164.46 (g)

After any survey of the ship under Regulations 7, 8, 9 or 10 of this Chapter has been completed, no change shall be made in the structural arrangements, machinery, equipment, etc. covered by the survey, without the sanction of the Administration. • Leaking valve from CHT with effluent on deck. • Missing glass cover on port main “raw water” gauge. Stbd bunkering station missing gauge. • Exposed fan blade in port side main deck workshop. • Active leak from CHT valve with effluent on deck. • Missing position indicator for rudder angle indicator on stbd steering gear. • Compressed air line on main deck routed in overhead not properly secured. • General housekeeping in machinery spaces. Identified 2 5-gal buckets of grease and misc. wires in steering flat, garbage, bags etc in engineroom bilge. • Main engine RPM gauges differ from Engine Room. Noted stbd gauge was sticking on bridge. Noted several degree pitch discrepancy btwn bridge and EOS stations.

The means of control of any such fixed gas fire-extinguishing system shall be readily accessible and simple to operate… The operating instructions for the remote and local bow thruster CO2 system were not consistent. Cite: 74 SOLAS II-2/Reg 8(h)

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire station #12 missing spanner wrench. Fire station #12, hoses not attached for immediate use. SOLAS (2009) II-2/Reg 14.2.1.2

Every ship of 400 gross tonnage and above other than an oil tanker shall be provided with an Oil Record Book Part I (Machinery space operations). The Oil Record Book, whether as part of the ship’s official logbook or otherwise, shall be in the form specified in appendix III to this Annex. Missing the master’s signature, tank details in accordance with IOPP, and entry details. Cite: MARPOL Annex I/Reg 17.1

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. Garbage Management Plan/Record Book had 7 entries incorrectly documented. Cite: 33 CFR 151.57(a)

On all ships, within the machinery spaces, emergency escape breathing devices shall be situated ready for use at easy visible places. An EEBD was found expired in the port shaft tunnel. Cite: SOLAS (2009) II-2 / Reg 13.4.3.1

Where the emergency source of electrical power is a generator, it shall be, started automatically upon failure of the main source of electrical power supply. The emergency generator could not take electrical load automatically upon loss of power from SSDG.

In the case of loss of the generator in operation, adequate provision shall be made for automatic starting and connecting to the main switchboard of a stand-by generator of sufficient capacity to permit propulsion and steering and to ensure the safety of the ship with automatic restarting of the essential auxiliaries including, where necessary, sequential operations. The automation function in all the breakers on the main switchboard are inoperable, reducing the automation capabilities of the ship's power system.

Oil filtering equipment referred to in paragraph 1 of this regulation shall be of a design approved by the Administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. The Oily Water Separator on board is inoperable.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. The quick closing valve for the entire fuel supply was found to be inoperable.

Every ship of 100 gross tonnage and above, and every ship which is certified to carry 15 of more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. A plastic garbage can is being used to collect and store garbage.

Doors provided to ensure the watertight integrity of internal openings which are used while at sea are to be sliding watertight doors capable of being remotely closed from the bridge and are also to be operable locally from each side of the bulkhead. The position indicator for the watertight door was not working.

The design, materials, and constuction of all watertight doors shall be to the satisfaction of the Administration. The Type III watertight door had a temporary modification so that it would indicate colsure on the bridge. Provide permanent repairs to the satisfaction of the Recognized Organization.

Fire-fighter's outfits shall comply with the Fire Safety Systems Code. Personel equipment shall consist of an axe with a handle provided with high-voltage insulation. The fire-fighter's outfits did not have axes.

Each panelboard must meet section 17.1 of IEEE 45-2002. Blank off exposed section of the fire alarm panelboard on the bridge.

The foundation for the gyro repeater on the starboard bridge wing is badly deteriorated and needs to be cropped out and the foundation renewed. Provide gauging report within 30 days to the satisfaction of the RO.

The port side steel of the tow booth is wasted. Gauge thickness of steel around the tow booth and provide gauging report within 30 days to the satisfaction of the RO.

Posters and signs shall be provided on or in the vicinity of the survival craft and their launching controls and shall give relevant instructions. BF Goodrich launching instructions were posted when the vessel used Elliot liferafts. Provide corresponding instructions.

The checks and tests of the steering gear shall include the operation of the means of communication between the navigation bridge and steering gear compartment. During test of the emergency steering gear, it was discovered that the means of communication was inoperable.

A power-driven vessel underway shall exhibit a masthead light forward. Forward masthead navigation light installed on rescue boat is currently inoperable. Repair or replace light and demonstrate proper operation.

Each item of lifesaving equipment required to be carried onboard must be approved. Lifebuoys installed for quick release from navigation bridge do not bear USCG approval. Demonstrate flag state apporval or replace with approved buoys.

Liferafts shall be readily available in case of emergency. Hydrostatic release for starboard aft inflatable liferaft found to be installed incoreectly and not ready for automatic release. Demonstrate proper installation of automatic release mechanism.

Machinery driving forced-draft fans, FO transfer pumps, and similar FO pumps must be fitted with remote controls from an accessible position outside the space so the fans/ pumps may be stopped in case of fire. The Diesl Oil transfer pump, LO transfer pump, Dirty Oil pump, and P/S MDE Turbo Ventilation Circuit Breakers do not have an under-voltage trip device. Maintain circuit breakers in LOTO status when not in use. When in use, the ER must be manned IAW amended transfer procedures.ABS Finding Number: DU3556506-I OSR 987. Please see Documents for Class Report. Class issued short term SLE certificate until circuit breakers can be replaced.

Replace worn fuel oil and lube lines on all generators with hoses complying with SAE J-1942

Repair fuel oil leak at #1 SSDG Racor outlet line check valve.

Ensure valves are labeled on bow thruster CO2 local release station as per discharge instructions.

Provide receipt of ABS endorsement on all international certificates

Upon operational testing of Vessel's semergency source of power, numerous lights were found to be inoperative & unable to adequately illuminate the muster stations, launching stations, & stowage positions of survival craft, automatically upon loss of ship's service power, 74SOLAS(Un-Amended) II-1/26(b). Proper operation of e-lightls to be demonstrated to the sat. of the ACS prior to departure.

Records pertaining to, as well as evidence of, correcting the Vessel's onboard navigational publications up to the most recent Notice to Mariners not found readily available on board. Publs to be updated as required by the Vessel's SMS Procedures Manual 7.2.004 section 3.1 & by 33 CFR 164.33, for the intended voyage, to the satisfaction of the ACS (ABS) prior to departure from port.

The containments for the Stbd. side bunker station (iwo frame 60) and F.O. vent (Stbd. side aft, iwo frame 90) were found to be holed and wasted. Temporary repairs to be immediately affected. Proper permanent repairs to be affected to restore integrity of containment, to the satisfaction of the ACS (ABS), within 30 days. 23 October 2010.

The markings relating to the incremental position of the rudder angle to Stbd., Stbd. rudder stock, are illegible and no longer visible. Markings are to be made clear and visible from the emergency steering position, to the satisfaction of the ACS (ABS), within 30 days. 23 October 2010.

Provide on board a USCG approved first aid kit (under approval series 160.041) for the rescue boat as required by 46CFR199.175(b)(10) within 30 days. 23 October 2010.

The Vessel's SMS is to be upgraded to include provisions for recording corrections to electronic nautical publications and charts to the satisfaction of the ACS (ABS), within 90 days. 22 December 2010.

The Vessel's records pertaining to the on-board maint & testing of the e-lighting system inadequately reflecst the materiel condition of the system as found on board. Records pertaining to these required tasks to be reviewed for completeness & the Vessel's SMS upgraded to incorporate the proper procedure for testing & servicing of the e-lighting arrangement.

Vessel's on-board SMS fails to address & identify the periodic servicing of the launching appliance for the rescue boat. Vessel's SMS to be upgraded to incorporate the requirements of 74SOLAS2009 Cons. (All Ships) III/20.11.1 & Regulation 36 pertaining to the instructions for on-board maintenance to the satisfaction of the ACS (ABS), within 90 days. 22 December 2010.

The provision for the periodic testing (via test buttons/switches) of the equipment powered by the complete emergency installation unable to be properly demonstrated as required by 74SOLAS(Un-Amended) II-1/26(b)(iii) and 26(a)(v). Proper operation to be demonstrated to the satisfaction of the ACS (ABS) within 30 days. 23 October 2010.

The vessel had a large amount of older 2.5 hoses onboard. The Chief Mate was unsure of their use. Advised him to find out if there is a use or remove from over pack safety gear locker.

The rescue boat cable had been replaced and no weight test had been conducted. Received email from Master 9-18-08 advising test conducted as per NVIC 04-07, confirm at next attendance.

Prove proper operation of the Port and Starboard engine room remote bilge pump valves.

Prove proper operation of #1, #2, and #3 ship service diesel generator mechanical overspeed trips.

Air receiver relief valves 1, 2 7 4 do not lift properly. Service valves and recalibrate lifting pressure.

Rescue boat could not be run at full speed. Provide proof of full throttle rescue boat operation.

Vessel could not provide proof of fall renewal for rescue boat davit. Provide record of renewal or replace. If new fall is installed, insure testing is completed as required by SOLAS LSA code.

No annual 125% hydrostatic test for fuel transfer hoses. Complete pressure test to verify integrity of the hoses iaw 33 CFR requirement.

Galley serving windows self closing mechanism did not function properly. Window would not shut completely when fusible link activated. Service galley serving windows and prove proper operation.

Provide copy of final report for tailshafts wear down readings.

Provide copy of anchor gauging report. Received copy of the report prior to completion of the inspection.

Non-destructively test the tailshaft flange, including the fillet and coupling bolts and provide copy of the report.

Provide copy of the final report for all sea valves and engine room emergency bilge suction valve.

Repair and prove the proper operation of the following watertight door/hatch indicators: Main Deck Passage Door, Transient Workers Access Door, Engine Room Escape Hatch, Starboard Steering Gear Room Escape Hatch.

Provide a new First Aid Kit for the vessel's Rescue Boat.

Repair and prove the proper operation of the General Alarm Bells located inthe Bowthruster Space, Lower Pump Room (#12), and forward Engine Room (#11)._x001C_ Item to be completed prior to operation of vessel to satisfaction of ABS.

HOSE TEST ALL MAIN DECK WATER TIGHT DOORS, HATCHES AND SCUTTLES AND ENGINE ROOM SOFT PATCH. MINIMUM WATER PRESSURE IS 30PSI.

REPAIR THE START AIR RELIEF VALVE IN WAY OF THE #1 AIR TANK.

REPAIR THE #1 STEERING MOTOR FLOAT SWITCH. IF THIS PUMP IS OPERATED UNDERWAY, THE LEVEL MUST BE VISUALLY CHECKED BY ENGINE ROOM PERSONNEL EVERYTWO (2) HOURS.
