On a nonmetallic vessel, the enclosures and frames of electrical equipment must be bonded together to a common ground by a normally non-current carrying conductor. Inverter in bunk space was not grounded. 46 CFR 183.372

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical socket in fwd bunk room had broken faceplate. 46 CFR 183.200(b)

All vesseks except non-self-propelled vessels shall be provided with suitable steering apparatus. Cotter pin on steering ram is undersized and causing the welling out of pin space. 46 CFR 182.30-1

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Missing burn gel. 46 CFR 184.710

At each initial and subsequent inspection for certification...the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected Port and STBD sliding windows in Pilot House are cracked. 46 CFR 176.25-50

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Smoke alarms not interconnected. Master reprogrammed detectors to be interconnected. 46 CFR 181.405(c)

At each subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems. Discovered fuel leak on inboard side of port fuel tank. Identify source of leak and repair to satisfaction of MI. 46 CFR 176.804

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times of the bilge pipe. The bilge suctions in the lazarette and sewage pump compartment did not have strainers. 46 CFR 182.25-5 (OLD T)

Discovered wet exhaust leaking at penetration on port side forward bulkhead of fuel tank compartment. Repair to satisfaction of the Marine Inspector. 46 CFR 176.804

At each subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems. Starboard engine wet exhaust spool piece found to be heavily corroded. Repair to satisfaction of the Marine Inspector. 46 CFR 176.804

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. Fuel tank vent was missing flame screen on port side. 46 CFR 182.20-35(OLD T)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Discovered electrical 12V DC bus bar unsupported beneath pilothouse operating station. Properly mount bus bar to the satisfaction of the Marine Inspector. 46 CFR 183.220(a)

All cable and wire must, be installed with metal supports space not more than 24 inches apart, and in such a manner as to avoid damage. Several new electrical installations found under pilothouse operating station improperly supported. Support electric wires and cable. 46 CFR 183.340(b)(4)

At each subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the inspection of all railings. Courses on upper deck were found broken, and/or not properly secured to the rail posts. 46 CFR 176.802

In general, compliance with the standards established by a recognized class society will be considered satisfactory evidence of the structural adequacy of a vessel. Transverse support beam for the main deck found with broken fiberglass tabbing at the port deck edge in the Engine Room. Repair to the satisfaction of the Marine Inspector. 46 CFR 177.10-1 (OLD T)

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Hydraulic hoses for steering ram found to be chafed into the wire reinforced construction. Replace hoses with SAE J-1942 standard. 46 CFR 182.30-1 (OLD T)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection and operation of all weather tight closures above the weather deck and the provisions for drainage of sea water from the exposed decks. Evidence of moisture and water intrusion found at the port main deck edge supports in the front bunk room. Restore watertight integrity to the satisfaction of the Marine Inspector. 46 CFR 176.802(a)(6)

At each subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the inspection of weather decks. Identified the following suspect areas: 1) 24" x 24" soft spot identified on port side upper deck. 2) 12" x 12" soft spot identified on stbd side upper deck. 3) Port and Stbd main deck flexing along the outer exposed passageways at existing repair seams. Repair prior to credit drydock. 46 CFR 176.802

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Ring life buoy on port bow did not have retroreflective material. Vessel marked ring buoy with retroreflective material in presence of attending MI. 46 CFR 185.604

Each marine employer shall ensure that random drug tests conducted under this part are unannounced and that the dates for administering random tests are spread reasonably throughout the calendar year. Company has designated a vessel Master to serve as the Company Designated Employer Representative (DER). This arrangement does not allow for a random selection of crewmembers as intended by this regulation. Company to assign responsibilities of DER to an employee that is not one of the members that shall be randomly drug tested as required by this regulation.

The movement of vessel shall be under the direction and control of the master or a licensed mate at all times. The master shall operate the vessel keeping the safety of the passengers and crew foremost in mind by directing the vessel in order to prevent a casualty. Vessel has been operating outside the conditions of the stability letter and has allowed passengers on the upper deck. In order to mitigate this requirement, vessel must provide the following addition: Create a chain at the upper and lower ends of the ladder to the second deck. On lower chain, place a sign at midpoint that reads "Maximum of 2 crew allowed on upper deck. No passengers allowed on upper deck while vessel is away from the dock." Chains will have a quick release device to allow crew to transit safely. 46 CFR 185.304(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: structural members. Operator reported a collision with another vessel that resulted in damage to the aft starboard quarter of the upper deck/house top. Submit repair proposal for approval and, once approved,make repairs to the satisfaction of the attending Marine Inspector prior to credit drydock.

Unless otherwise specified, vessel owners must pay the fees required by this subpart before inspection or examination services are provided. Vessel is overdue and in collection status for 2021 User Fees. To resolve, contact FINCEN [phone withheld] OR EMAIL [email withheld]. Resolve prior to next inspection.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation. The vessel currently does not have a valid Certificate of Documentation. 46 CFR 67.313

At each inspection for certification and at every other vessel inspection all observed unsafe practices and fire hazards must be corrected. The hose for the drip-less shaft seal was chafed from continuous contact with the shaft. The vessel is to provide a new hose for the drip-less shaft seal and to arrange the hose in such a way that the hose doesn't come in direct contact with the shaft. 46 CFR 176.830(a)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. The vessel had dead end wires in the engine room that needed to be addressed/removed. 46 CFR 183.200(b)

A vessel's hull must not carry current as a conductor. The vessel's grounding wires were not properly wired and needs to be addressed. 46 CFR 183.370 (a)

On a vessel on an exposed or partially protected route, a watertight coaming with a height of at least 6 inches must be provided under each watertight door. The vessel currently has coaming for the weathertight door that is 4 inches high. 46 CFR 179.360(d)(1)

The station transmitting radiotelegraph emissions must be identified by its call sign. The call sign must be transmitted with the telegraphy emission normally used by the station. The call signs on two FCC certifications were mismatched. Vessel operator to obtain a uniform call sign that aligns with both FCC certifications. 47 CFR 80.99 (a)

Each vessel certificated to operate on an oceans route in cold water must be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. Vessel currently has IBA with capacity of 50 persons. Vessel is restricted to carrying no more than 48 passengers. For each additional crew member, passenger capacity decreases so that no more than 50 persons are onboard.

Materials and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The Port Side AFT deck beam in the Lazarette Compartment is constructed of a wooden laminate. The bottom 1/3 of the wood is delaminated. Provide a repair proposal to the satisfaction of the Marine Inspector. 46 CFR 176.800

Each survival craft must be secured to the vessel by a painter with a float-free link. The hydrostatic release for the 02 IBA's were incorrectly installed, and would not have properly operated. The operator corrected the installation on the spot. 46 CFR 180.137(a)(1)

Vapor barriers must be provided where flammable combustible liquids or vapors are present, such as machinery spaces. The aft bulkhead of the engine room had an unsealed penetration. The penetration was sealed to the satisfaction of the attending Marine Inspector. 46 CFR 177.405(e)

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. Excessive dirt and grime was discovered on the bilge strainer in the Forward Lazarrete space immediately aft of the Engine Room. Clean the strainer to allow proper suction IAW 46CFR182.25-5 to the satisfaction of the MI. 46CFR182.25-5 (OLD T)

All engines must be water cooled and meet the requirements of this paragraph. Starboard side MDE sea chest intake ball value is damaged and needs to be replaced.

The construction and arrangement of a vessel must allow the safe operation of the vessel. Damaged FRP on the deck support strut in the forward void space, port side. needs to be repaired, increasing structural strength for the forward passenger deck.

A vessel must be provided with a main steering gear this is of adequate strength and capable of steering the vessel at all service speeds. Sterning gear transverse support beam weld is cracked, needs to be re-welded.

Each hatch exposed to the weather must be watertight and have a securing device that is attached to the hatch frame or coaming by hinges, captive chains, or another device to prevent its loss. MSD holding tank space hatch did not have a securing device, needs to have a securing device attached.

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Through bulk head penetration, port side of steering gear room does not have vapor tight integrity. Opening must be filling with vapor tight material.

A vessel must be provided with a main steering gear that is of adequate strength. Both starboard and port rudders have excessive play, no record of maintenance every being conducted on the rudder system, and unable to visual inspect bearings. These factors have lead me to require the steering gear system to be dissembled and inspected, and repaired accordingly.

Cross member and deck supports in the forward void space are cracking and separating. Provide a repair proposal and limit operations to 7 NM offshore. 46 CFR 177.01-5

Wiring throughout vessel is improperly installed with frequent loops. Adjust wiring to straighten out and shorten lengths.

In the space forward of the lazarette, hydraulic fittings at the bulkhead for the steering gear are corroded. Replace metallic fitting. 46CFR182.40-1(a)(1)(i)

Crew bunk room inaccessible due to excess stored items filling the space. Clean up space for safer access.

Electrical wiring in the crew bunk room was found running along the deck with no supports. Install supports for wiring.

02 Engine Room bulkhead penetrations were found. Refill with appropriate material to ensure bulkhead is vapor tight. 46 CFR 182.35-1(a)

Generator had exposed moving parts posing a hazard to crew. Install guard to cover moving parts. 46 CFR 177.35-15

Passenger area found with 02 plastic trash cans. Replace with metallic receptacles. 46 CFR 177.30-7

Excessive mold found on the overheads of the crew bunk room space. Remove mold from overhead. 46 CFR 176.25-45

Generator exhaust piping missing hose clamps and tied off using a t-shirt. Install appropriate hose clamps. 46 CFR 182.20-20

Replace bolts on deck beam in forpeakat both forward and stbd shelf. Reinforce connection by sandwiching with wood of sufficient strength, 46 CFR 177.10(a)
