Each Captain of the Port may order a vessel to operate in the manner directed when the Captain of the Port has determined that such order is justified in the interest of safety by reason of weather, visibility, sea conditions, temporary port congestions, other temporary hazards circumstances, or the condition of the vessel. Due to your inoperable stbd main egine, while transiting through the Sector Virginia regulated navigation area (RNA), as defined by Title 33, Code of Federal Regulation (CFR), 165.501 (a)(2): your vessel shall be escorted by either: - One tug of not less than 3,000 horsepower and one tug of not less than 2,000 horsepower, or; - Two tugs of not less than 50 Ton Aggregate Bollard Pull, or; - One tug of not less than 50 Ton Bollard Pull.

The administration shall satisfy itself that the general structural strength of the hull is sufficient for the draught corresponding to the free board assigned. Ships built and maintained in conformity with the requirements of a classification society recognized by the administration my be considered to possess adequate strength. The integral bilge water retention tank, IOPP 3.3-Bilge Oily Water Tank frames 204-208, is compromised (either fractured or holed) below the hull bottom plate and permitting the passage of sea water into the tank. The crew reported that the tank began to overflow into the engine room on April 28th on the midnight to 0400 watch. The crew isolated all drain lines to the tank, capped the tank, and removed associated OWS transfer piping from the tank. Provide a class report verifying compliance with the tank's integrity. ICLL Ch 1 Reg 1

Except as specified in paragraph 3 of this regulation, any ship of 10,000 GT an above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. The crew removed the piping to transfer from the Bilge Oily Water Tank to the oily water separator. The IOPP 3.3-Bilge Oily Water Tank located at frames 204-208 is compromised below the bottom plate and the tank is full of sea water, rendering it useless. The crew are unable to use the oily water separator under the current conditions on-board. MARPOL 73/78 (2022 Cons.) Anx 1 Reg 14.2

A main electric lighting system which shall provide illumination throughout those parts of the ship normally accessible to and used by crew. Found numerous inoperable lights in all decks of cargo hold #2. 74 SOLAS(81) II-1/41.2.1

Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls and shall use symbols in accordance with the recommendations of the Organization. The life boat's embarking instruction placards posted are not the correct type of life boats. 74 SOLAS(20) III/9.2.3

The cargo pump bulkhead seal gland is leaking and not gas tight. 74 SOLAS(20) I/11

The purpose of this regulation is to contain a fire in the space of origin. For this purpose, the following functional requirements shall be met: the fire integrity of the divisions shall be maintained at openings and penetrations. The following items to be repaired to as original to prevent the passage of fire and smoke: 1) Unapproved modifications made to KBR bulkhead in way of Auto Tool Rm./Preservation door with a drilled hole and missing insulation. 2) Holed 8” ducting for air supply ventilation in way of overhead penetration. 74 SOLAS (81) Chapter II-2, Part C, Regulation 44

Unless expressly provided otherwise in this regulation, at least two widely separated and ready means of escape shall be provided from all spaces or group of spaces. KBR door marked Auto Tool Rm/Preservation Packaging and Packing Equip/USMC Tool Room on 63 Level with dried paint between door and door jamb preventing the door from being open from the inside or outside of the space. Remove excessive dried paint from between door jam/door and prove operation. 74 SOLAS (20) Chapter II-2, Part D, Reg 13.2.1

Conduct permanent repairs to boiler flat #1. Currently there are temporary repairs made.

Before the ship leaves port and at all times during the voyage, all life-saving applicances shall be in working order and ready for immediate use. Due to the current condition of the lifeboats, the six lifeboats are condemned until a causal analysis is conducted by OEM and permanent repairs are completed to the satisfaction of ABS/USCG. Follwing shall be provided to USCG/ABS; 1) Causal Analysis to determine why the hull materials are waterlogged and why the water intrusion occured; 2) Corrective Action Plan to establish how permanent repairs will be complete; 3) Date final repairs will be complete and ready for inspection. Repair plan must be submitted, reviewed and approved by USCG/ABS prior to commencing repairs.

Load line certificate shall certify that the vessel is in compliance with applicable requirements. The current condition of wastage identified to the bottom shell longitudinal and bottom of the transverse bulkheads show that the vessel is not in compliance with applicable requirements during the time a full term certificate was issued.46CFR8.320(b)(1; 46 CFR 42.07-45 (b)

Each plate and longitudinal member on the transverse section is required to be gauged when the bottom plating is considered to have substantial corrosion (When the percent diminution was greater than 25%). The longitudinal members on the transverse section from Frame. No. 218-237 were not gauged during the dry dock. ABS Rule Part 7 Appendix Section 4

Organization shall implement production and service provision under controlled conditions. The implementation of monitoring and measurement activities at appropriate stages to verify that criteria for control of processes or outputs and acceptance criteria have been met. Attending ABS surveyor issued a load line certificate without verifying wastage allowance on the longitudinal and transverse bulkheads at connection to bottom shell plating from Frame. No. 218-237. ISO:9001 8.5.1(c), 46CFR8.420

Horizontal Shaft Anchor Windlass foundations for the Port and Starboard anchors noted with wastage throughout several areas of their foundation. Anchors windlass including foundation are to be examined, as far as possible. A function test may be required if deemed necessary by Surveyor.ABS Rules Part 7 CH 3 Section 2.1.1.7

Found multiple issues with vessels open lifeboats specifically fiberglass damage in #2 and #5, releasing gear upright pipes corroded on #2, 3 & 4. OEM, Palfinger, called in to effect repairs.

Received 2692 concerning the Aux fire pump motor had failed during routine test.

Repair / replace and prove proper operation of forward safety on No. 1 auxiliary boiler. To be cleared by class (ABS).

Conduct lifeboat weight test (full load condition, 100% of "B" weight) for all lifeboats. To be cleared by class (ABS).

Update all Fire Control Plans to show location of fireman's outfits.

Post life jacket donning placards for all types of life jackets used onboard vessel.

Paint load line marks on port and stbd sides of vessel.

Properly label general alarm in bow thruster space.

Install blank in distribution panel #8P145 located in CO2 room.

Properly label fwd repair locker door located in focsle and update all FireControl Plans to reflect the same.

Properly seal multiple cable penetration in electronic equipment room on bridge deck.

Clean out Gaylord hood wash-down drain in galley.

Properly install non-conductive mat for deck machinery switchboard in focsle.

Post operating instructions for #1 and #2 lifeboat engines.

Install overfill devices in each cargo tank used for carrying oil in accordance with the provisions of 33 CFR 155.480.

Conduct annual test of transfer piping system for oil cargo in accordance with 33 CFR 156.170.

Conduct weight test of lifeboats in accordance with 46 CFR 199.

Reexamine and replace as necessary PFD water-activated lights manufactured by ACR Elect. Inc.

Reinstall missing vent cover to #3A salt water ballast tank vent.

Test the operation of all ammunition hold ventilation dampers and take appropriate corrective action to free any frozen dampers.

Renew forward main deck bow scuttle securing dogs to original condition.

Conduct annual test of oil cargo system pressure gauges in accordance with 33 CFR 156.170 prior to next transfer of oil cargo.

Renew port and starboard DFM service tank vent bell housings.

Provide updated ship distress signals for ship and all lifeboats.
