Each collision bulkhead must not be fitted with any type of penetration or opening except penetrations may be made if they are located as high and as far inboard as practicable and they have a means to make them watertight. The collision bulkhead had a penetration that was not watertight. Make the penetration watertight.

An Employment Assistance Program (EAP) training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Provide evidence of training for owner/captain (Supervisory), and crewmembers (non-supervisory), and provide evidence of training to the USCG.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. The life buoy did not have retroreflective tape.

Generalized pitting and corrosion found and marked in several areas of the aluminum topside railing, near the operating station. Repair or replace. Code 705C Prior dry dock credit.

Wood rot and wood deterioration was found in two deck support frames in the toilet space, above the storage racks. Crop, renew and apply a wood preserver. Code 705C prior dry dock credit.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to protection against fire. The bulkhead between the engine room and steering gear compartment had 03 penetrations with visible gaps allowing for the passage of CO2 if pre-engineered fire extinguished required deployment

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI. The portable fire extinguisher located in the accommodation space was not filled to the appropriate capacity as indicated on the gauge, leaving vessel with 2/3 fire extinguishers required on board.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The retroreflective tape on 06 child sized lifejackets was visible worn and had no retroreflective intensity.

A hatch exposed to the weather must be watertight. The hatch into the center compartment was not watertight due to worn out gasket.

A vessel not required to have a power driven fire pump must have at least three 2.5 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel was found to have in place fire buckets of more than 2.5 gallons. 46CFR181.610 16C

At each inspection for certification and at every other vessel inspection all observed unsafe practices, and other hazardous situations must be corrected. A possible down-flooding point was identified in the portside top railing stern quarter due to a missing cap/vent scoop. 46 CFR 176.830, 16C

Electrical connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. Multiple dead-end wires were found underneath the operating station and not servicing any electrical equipment, and with expose electrical wiring without not being secured. 46 CFR 111.60-17, 16C.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits, survival craft embarkation areas, and ring life buoys; (2) The stowage location(s) of life jackets; (3) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, (4) The location of the instruction placards for life jackets and other lifesaving devices; (5) That all passengers will be required to don life jackets and other lifesaving devices. (6) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master. Vessel crew failed to cover all information required to announce to the passengers in the safety briefing. 46 CFR 185.506, 16C

Each portable fire extinguisher on a vessel must be of an approved type. Portable fire extinguishers brackets were found too small for the extinguisher not allowing easy stowage and retrieval; also not to be the approved bracket for the extinguisher model, according with extinguisher manufacturer. 46 CFR 181.500, 16C

FCC Radio Telephony Safety Certificate was found to have the Call Sign to be incorrect. Contact a certified FCC Technician to re-issue the certificate and provide copy to an attending USCG Marine Inspector. 46 CFR Part 80.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, and other hazardous situations must be corrected. Forward starboard side railing stanchion was found lose, with a penetration in the fiber glass at its base where is secured to the deck. 46 CFR 176.830, 16C.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure. A fracture at the top of the starboard gunwale, allowing the possibility of water intrusion. 46 CFR 176.404, 16C.

EAP education program must include: display and distribution of a community service hot-line telephone number for crew member assistance. EAP Placard was found expired, dated 2023. 46 CFR 16.401, 16C.

CG003: A Certificate of Documentation is required for the operation of a vessel in certain trades, serves as evidence of vessel nationality and permits. Any vessel of at least 5 net tons which engages in coastwise trade , must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which is engaged. Vessel's current and valid original Certificate of Documentation was not available on board. 46 CFR67.1 & 67.7. 17C.

07106: A Pre-Engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. The owner or managing operator shall provide satisfactory evidence of the required servicing to the USCG Marine Inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must be required to performed the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached may be accepted as evidence that the necessary maintenance procedures have been conducted. Current Pre-engineered clean agent CO2 cylinder was installed with pressure gauge and manufacture decal towards the bulkhead, and unable to read. The servicing certificate does not provide any information of what type of service was conducted, nor information on what was the last time cylinder was hydro tested. Cylinder also was recently painted and still shows areas of corrosion. 46CFR181.420(a)(4)/ 46CFR176.810(b)(1)/ 46 CFR176.830. 17C.

11116: A vessel in a limited coastwise route must carry six hand red flare distress signals, six orange smoke distress signals with a USCG approval number under series 160.021; or each orange smoke distress signals can be substituted by either a rocket parachute or a hand red flare distress signal. Vessel was found without any orange smoke distress signals and missing 04 red hand flare distress signals. 46CFR180.68/46CFR176.830 17C.

13199: Fuel shut-off valves shall be installed so as to close against the fuel flow, must be fitted in the fuel supply lines. starboard side fuel shut-off valve was found not to be closing or opening all the way upon actuating, and cause the engine to shut-off in open position during man over board drill and sea trials. 46 CFR182.455(b)(4)/ 46CFR176.830 17C.

07116: Provisions must be made for closing all supply ducts openings for a space protected by a fixed gas extinguishing system. Portside closing device was found not closing completely due to a corroded lower support. 46CFR182.465(h)/ 46CFR176.830. 17C.

01104: At least one person is required to have a valid FCC VHF Radio Operator Permit on board a vessels carrying more than six passengers for hire in the open sea or any tidewater area of the United States. Current vessel licensed master is not holding, nor in possession of a FCC VHF Radio Operator's Permit, and in command of a vessel carrying passengers for hire since March 2023 according with the logbook. 47CFR Part 13/46CFR176.830. 17C.

02108: Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions, protect passengers, crew and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. All ground cables in middle room, engine room and steering room have been found covered in sediments, lose, and wasted connectors. 46 CFR 183.200 (a)&(b)/ 46 CFR 176.830. 17C

02108: Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions, protect passengers, crew and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Middle room, engine room steering room & VHF electric bus bars were found corroded, deteriorated with lose cable connections. 46 CFR 183.200 (a)&(b)/ 46 CFR 176.830. 17C

10111: As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Nautical chart, (2) U.S. Coast Pilot, (3) Coast Guard Light List, (4) Tide Tables, and (5) Current Tables. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Vessel did not had on board the following: (1) updated nautical chart, (2) Current tables and (3) Coast Pilot, (4) Coast Guard Light list. 46 CFR 184.420 (a)&(b). 16C.

10111: The operator of each self-propelled vessel 12 meters (39FT) or more in length shall carry, on board and maintain for ready reference, a copy of The Rules for Navigation (COLREGS). Vessel does not have on board a copy of the Rules for Navigation. 33 CFR 83.01 (g). 16C.

04108: The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Posted emergency instructions were found faded and not posted for crew and passengers view. 46 CFR 185.510 (a). 16C

10127: Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits, survival craft embarkation areas, and ring life buoys; (2) The stowage location(s) of life jackets; (3) Either: (i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or (ii) That passengers may contact a crew member for a demonstration as appropriate, prior to beginning a voyage; (4) The location of the instruction placards for life jackets and other lifesaving devices; (5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master. The verbal emergency instructions performed by the master did not covered all the above required information that shall be provided to the master. 46 CFR 185.506, 16C.

10109: A vessel of 12 meters (39FT) or more in length shall be provided with a whistle and a bell, all in accordance with the specifications in Annex III in the Navigation Rules. Diameter of the bell mouth shall be not less than 30mm. Whistle must have audibility range of 1 nautical mile. Vessel has no bell or whistle on board. Navigation Rules Part D, Rule 33, & annex III, 16C.

10109: All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel stern light was found inoperable. 46 CFR 183.420/ 33 CFR 83.20, 16C.

11108: Each life float must be fitted with two paddles of at least 4 feet in length, lashed to the life float and buoyant. Largest life float two paddles were wasted and fracture due to UV light exposure. 46 CFR 180.175 (d)(2), 16C.

11118: Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Only one donning placard was found, and lifejackets being carried on board are different model from the one displayed in the donning placard, which are [name withheld] brand. 46 CFR 185.516, 16C

03107: A hatch exposed to the weather must be watertight, must be attached to the hatch frame or coaming by hinges, captive chains or other devices of substantial strength to prevent its loss. Forwards hatch was found leaking water when tested, and none of the hatches(forward, mid or stern were found secured to the hatch frame to prevent its loss. 46CFR 179.360(a)&(b)(2)/ 46 CFR 176.830, 16C.

13104:a visual and audible alarm must be provided at the operating station to indicate a high water level. Bilge high water audio alarm was inoperable in the middle compartment. 46 CFR 182.530/ 46 CFR 176.830 / 16C.

99101: Glass and other glazing materials used in windows accessible to passengers and crew must be of material that will not break into dangerous fragments if fractured. Windows material in the passenger accommodation area were found fractured, broken and missing pieces. 46 CFR 177.1010 46 CFR 176.830, 16C.

13101: Repairs and alterations to the machinery or equipment that affects the safety of the vessel must not me made without the approval of the USCG Officer in Charge of Marine Inspections. A coolant fluid reservoir bottle was removed from the vessel for the portside engine after it broke, and the other reservoir coolant fluid was found broken, without no any maintenance to repair and replace back into service as intended by vessel engine manufacturer's as designed. 46 CFR 176.700, 46 CFR 176.830, 16C

13199: Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure. Both port and starboard diesel fuel filters were found to have an excessive amount of sediment-debris inside. 46 CFR 182.100/ 46 CFR 176.830 / 16C.

02108: Alterations and modifications, such as replacing extensive amounts of cabling, must comply with the regulations in 46 CFR 183.200 and 46 CFR 110. A large amount of dead-end wires, that have been cut or are not connected servicing any electrical component was found underneath the operating station, in the VHF console, in the engine room, middle room and steering room. 46 CFR 183/ 46 CFR 110/ 16C

07126: At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems. Upon removing the inspection port underneath the ladder that leads to the toilet, an accumulation of garbage (debris) was found. 46 CFR 176.830 (b)/ 16C.

18299: Immediately after the addressing of resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a marine casualty shall notify the nearest Sector Office, Marine Inspection Office, or Coast Guard Group Office whenever a vessel is involved in a marine casualty. Master failed in answering the basic questions about a marine casualty reporting procedures, required hours-days for alcohol and drug testing following a marine casualty, the use of the USCG Form CG2692B, and how many days to submit report to the USCG. Vessel Master must undergo training in such requirements. 46 CFR 185.202/ 16C.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Port and Starboard main diesel engines are leaking diesel fuel onto the engine compartment bilge. Sea trial and drills need to be completed before clearing code 17.

An adult life jacket must be provided for each person carried on board a vessel. At time of inspection, vessel had 39 serviceable adult life jackets of the 52 required per the COI. Passenger count reduced to 39.

Each survival craft must be in good working order and ready for immediate use at all times. Metal tube in the foot netting part of the 6 man life float was found corroded and broken. Requires replacement or repair. Passenger count shall be reduced to 40 on vessel until deficiency rectified.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts and publications. At time of inspection charts and pubs were not on board.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Engine compartment drainage pipe partially obstructs Port fuel shutoff valve from operation intended and is not in good marine practice.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. Dead end wires identified overhead in the toilet space. Requires removal.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual involved in the SMI. Alcohol swab test on board were expired.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. COD on board vessel was past expiration date. COD renewal documentation required.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Ring life buoy was found with a wasted drain plug permitting water intrusion into the body of the ring. Requires replacement.

Each vessel that operates beyond three miles from the coastline must have a category 1 EPIRB installed to automatically float free and activate. Hydrostatic release unit designed to allow the EPIRB to float free was found with an expired date. Requires replacement and cannot operate beyond 3 nautical miles until deficiency cleared.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Solenoids zip tied on both Port and STBD engines due to broken or missing screws need to be repaired/replaced.

Each battery must be free from exposure to water splash or spray. Batteries in engine space require a cover.

Wet woods and angel hair on all beams in the engine compartment. Starboard side suffering severly.

Navigation lights and navigation system non-operable.

Insulation on the engine compartment hatches missing.

Water intrusion in the engine and forward compartments.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Two (02) crew member have worked onboard without pre employment testing, being enrolled in consortium with company.

Each EPIRB must be tested monthly using integrated test circuit and output indicator. Logs showed EPIRB was not tested monthly, master confirmed.

Each item of survival craft equipment must be good quality and efficient for the purpose it is installed to service. Potwarp lines deteriorated, replace on survival craft.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station. An audible alarm that sounds testing equipment, no audible alarm was observed/heard.

On a nonmetallic vessel, the neutral must be connected to the common ground except that aluminum grounding conductor must not be used. Grounding in stern steering space was broken in places. Check for grounding in steering and engine spaces/clean corrosion.

Exhaust piping must be properly supported by noncombustible hangers or blocks. Starboard side piping has broken hanger.

Flexible nonmetallic hose must be complete with factory assembled end fittings requiring no further adjustment of the fitting on the hose. Ensure meets SAE J-1475. Fuel hoses on both engines between engine body and filter were observe to be clamped on in place. Ensure hoses meet SAE J-1942.

A hatch cover must have securing devices. Forward hatch cover (bow) has wasted securing devices. Replace wasted securing arms.

Vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The contents of the first aid kit were expired.

Internal framing, plating or planking of the vessel is concealed, section of the lining, ceiling or insulation may be removed or parts otherwise probed or exposed so that the inspector may be satisfied as to the condition of the hull structure. Observed angel hair rot along all four (04) corner of the hatch covers to the engine space.

Light must be a floating waterlight approve under approval series 161.010. The floating water must be attached around the body of the lifefloat by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultra-violet light5.5 meters (18 feet) in length. The lanyard was wasted and needs replacing.

All machinery equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Port exhaust muffler is missing one metal mounting clamp.

A visual alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. The aft high bilge level visual alarm was inoperable.

A vessel on a limited coastwise route must carry six red distress signals and six orange smoke distress signals. The vessel was not carrying sufficient distress signals.

All GMDSS equipment capable of transmitting an automatic distress alert which includes the position of the ship must have either an integral navigation receiver or capability of being connected to an external navigation receiver. VHF radio does not display GPS coordinates. 47 CFR 80.1085(c)

COLREGS Rule 27. The vessel did not have the international dive Code Flag "A".

The official number of the vessel must be marked on some clearly visible interior structural part of the vessel. Vessel does not have official number permanently displayed.

Man overboard entries shall include a general description of the drill scenario and training topics. Drill logs are lacking details of the drill scenario.

Fuel line must have a remotely activated valve at the tank. The activation cable was seized on the port fuel tank and must be repaired to demonstrate proper operation to the satisfaction of attending Marine Inspector.

Each life float must be equipped with an approved light. The 6 person life float was not equipped with an approved light and passenger count has been reduced to 48 until the light is provided.

Each engine must have a gauge at the operating station to indicate oil pressure. The port gauge was not working and must be repaired.

Each submersible bilge pump must be UL rated. The pump in the engine room is not and must be replaced with UL rated pump.

wire connections must be made in an enclosure. There are multiple dead-ended wires in the void space forward of the engine room and the aft void.
