The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master must provide medical certificate as per MMC requirements. 46 CFR 10.301

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident... shall comply with the requirements of 4.06 in subchapter A of this chapter. S and L. A power-driven fire pump shall be installed on each mechanically propelled "S" vessel which is a ferry vessel or which is permitted to carry more than 49 passengers. Each ring life buoy must be of an approved type, constructed in accordance with subpart 160.050 of this chapter; except a ring life buoy that was approved under former subpart 160.009 of this chapter may be used if it is in good and serviceable condition. All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Address excess and dead end wiring in the pilot house and engine room. 46 CFR 183.01-15 OLD T

A vessel must comply with the applicable design, equipment, personnel, procedures, and record requirements of 33 CFR parts 151, 155, and 156. Vessel must submit waste/garbage management plan. 46 CFR 184.702

Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft non-ferrous metal straps with no sharp edges. Where passing through bulkheads, fuel lines shall be protected by close fitting ferrules or stuffing boxes. Center tank fuel fill hose not mounted or secured properly. Repair within 14 days. 182.20-40 (2) OLD T

A pre-engineered fixed gas fire extinguishing system must: Be approved by the commandant. Vessel must submit documentation regarding CO2 shutdowns to the satisfaction of OCMI. 46 CFR 181.420(a)(1)

Have a breaking strength of at least 6.7 kN (1500 lb.), except that if the capacity of the life float or buoyant apparatus is 50 persons or more, the breaking strength must be at least 13.4 kN (3000 lb.). Vessel must submit documentation regarding the particulars of the painter to the satisfaction of the OCMI. 46 CFR 180.15 (c)(2) OLD T

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. MI observed over accumulation of water in the forepreak and fuel tank compartments. 46 CFR 182.25-1 OLD T

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. MI observed unsealed penetrations in fwd and aft bulkheads in fuel tank space and lazarette space.

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment: Vessel must add bilge alarms to all spaces aft of the collision bulkhead. Complete within 30 days. 46 CFR 182.25-5(a) OLD T

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system. Vessel does not have an interconnected fire detection system. Vessel is restricted to a limited coastwise route until rectified. 46 CFR 181.405(c)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Centerline aft bilge pumps in the E/R has no visual indicator at the operating station. 46 CFR 182.530(c)

An existing vessel with a hull, or a machinery space boundary bulkhead or deck, composed of wood or fiber reinforced plastic, or sheathed on the interior in fiber reinforced plastic, must comply with the requirements of § 181.400 of this part on or before March 11, 1999. Vessel must submit a 7-day oeprational safety plan for an actual engine room fire emergency. After 7 days, Prior to Passenger deficiency will be issued until vessel can conform to approved system designed. Provide service report and CG must witness satisfactory operational test. 46 CFR 181.400

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Vessel must add bilge high level alarms. Coast Guard will coordinate additional inspection as this requirement is being appealed by the operator.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. Marine inspector observed water of unknown origin flowing in engine room. Generator water pump had failed and was replaced prior to the end of the inspection.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible. Vessel is employing electronic charts with no mechanism to provide updates.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. All pages of COI were not posted as required.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests for fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. Cylinders must be retested at least every 12 years. Flexible connections between cylinders and distribution piping of semiportable or fixed carbon dioxide fire extinguishing systems and discharge hoses in semi-portable carbon dioxide fire extinguishing systems must be replaced or tested at a pressure of 6.9 MPa (1,000 psig). At test pressure, the pressure must not drop at a rate greater than 1.03 MPa (150 psi) per minute for a 2-minute period. The test must be performed when the cylinders are retested. Fixed carbon dioxide system is overdue for testing.

Exhaust gas must not leak from the piping or any connections. Main engine exhaust found with leak that was leaving soot deposits on lagging. Repair exhaust and ensure that all exhaust between engine and point of cooling water injection has exhaust insulation.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission. Vessel does not have a Safety Radiotelephony Certificate.

Vessel did not have remote fuel shut off signs posted.

Wiring joints and splices shall be mechanically secure and made in junction boxes. Wiring along upper deck rails was tied with wire nuts. Electrical connection points must be enclosed in junction boxes for replaced with continuous wire runs. Ref: (OLD T) 46 CFR 183.05-50 LIGHTING AND POWER WIRING INSTALLATIONS (LESS THAN 50 VOLTS).

(2) Lifefloats lacked reflective SOLAS tape on several areas. Operator renewed missing tape areas on the spot.

COI expired

MI discovered that the NORTHERN STAR's COI expired on 03MAY2022. 46CFR176.100(a): A vessel to which this subchapter applies (T) may not be operated without having on board a valid US Coast Guard Certificate of Inspection.

A voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation. For alternating-current generators a means for measuring frequency shall also be provided. NORTHERN STAR currently is not fitted with means to measure these operating parameters. Install required meters. 46 CFR 183.10-5(g)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) provided that a positive shutoff valve is installed at the hull penetration. On NORTHERN STAR, several submersible electric bilge pump discharge lines are installed with no positive shutoff valve at the hull penetration. CORRECT DURING NEXT DRY DOCK.

Center engine that had been rebuilt developed an oil leak at the supply side of the stbd turbo. Vessel authorized to operate only on the port and stbd engines until satisfactory repairs and power trials could be witnessed.
