Every vessel subject to the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician once every 5 years. The current inspection report is expired. Vessel is restricted to no more than 1000 ft from shore until inspection is conducted. 47CFR803903

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. The current endorsement is expired. Vessel is restricted to no more than 1000 ft from shore until proper endorsement is verified. 47CFR80.1005

Marine employees shall establish programs for chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who are required by the vessel's COI. Proof of all crew members being enrolled in the company's drug consortium was not made available during the inspection.

Alcohol testing must be conducted on each individual engaged or employed on board a vessel who is directly involved in a serious marine incident. The alcohol testing strips on board were expired.

The master shall ensure that each EPIRB is tested monthly and logged or otherwise documented. No documentation of EPIRB testing was found.

Placards containing instructions for donning life jackets must be posted in conspicuous places regularly accessible and visible to crew and passengers. A donning placard should be placed on the upper deck near the life jacket storage area.

When flexible section is a nonconductor of electricity is used on filling pipes, the metallic sections of the filling pipe separated thereby shall be joined by a conductor for protection against static spark when filling. One of the diesel fill pipes lacks a grounding wire.

Where a flexibility fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration may be used. The flexible tubing on the middle fuel fill has a crack and is no longer serviceable.
