Except as noted in paragraph (d) of this section, the data shall be stamped directly on the pressure vessel. The data shall be legibly stamped and shall not be obliterated during the service life of the pressure vessel. Inspector identified data plate on pressure vessel was painted over and not legible. Remove paint and have data plate legible to the satisfaction of the attending Marine Inspector. 46 CFR 54.10-20(c)

Bulkheads or decks of the “A” Class shall be composed of steel or equivalent metal construction, suitably stiffened and made intact with the main structure of the vessel; such as shell, structural bulkheads, and decks. They shall be so constructed, that if subjected to the standard fire test, they would be capable of preventing the passage of flame and smoke for one hour. Marine Inspector observed two bulkhead penetrations in the aft machinery space. Fill penetrations with approved material meeting the requirements for "A" Class boundary to the satisfaction of the attending Marine Inspector. 46 CFR 92.07-5(b)

The instructions supplied by the manufacturer must contain directions for each of the following: Safe operation and servicing of the device so that any discharge meets the applicable requirements of 159.53. Vessel stated they use bleach 4 to 1 with water to service Type II MSD onboard. Provide manufacturers instructions with correct servicing procedures to the satisfaction of the Marine Inspector. 33 CFR 159.57(a)(2)

When the vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the vessel sinks. Inspector identified that EPIRB hydrostatic release expiration date was not indicated. Provide documentation proving the hydrostatic release purchase date to the satisfaction of the Marine Inspector. 46 CFR 199.510(b)

Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. Inspector identified conduate for nav lights going to the life raft deck was corroded throughout. Replace conduate to ensure proper protection of electrical hazard to the satisfaction of the attending Marine Inspector. 46 CFR 111.01-1(a)(2)

On vessels required by 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: Enough lights throughout machinery spaces to allow essential operations and observations under emergency conditions and to allow restoration of service. Marine Inspector identified emergency lights in fwd machinery space on the aft stbd bulkhead were not energizing. Show proper operation of emergency lights to the satisfaction of the attending Marine Inspector. 46 CFR 112.15-1(b)

No manned ship subject to this part may operate unless it carries on board a shipboard oil pollution emergency plan approved by the Coast Guard. Inspector identified vessel SOPEP had expired on September 18, 2022. Submit and receive approved plan by the Coast Guard. 46 CFR 151.27(a) 46 CFR 151.26 46 CRR 151.09

Lighting, including low location lighting if installed, for passageways, stairways, and escape trunks in passenger quarters, crew quarters, public spaces, machinery spaces, damage control lockers, emergency equipment lockers and work spaces sufficient to allow passengers and crew to find their way to open decks and to survival craft, muster stations, and embarkation stations with all watertight doors and fire doors closed. Inspector identified that emergency exit sign in oil pump room would not energize. Fix exit sign to energize with emergency lighting to the satisfaction of the attending Marine Inspector. 46 CFR 112.15-1(c)

The boundary bulkheads and decks separating the accommodations and control stations from cargo, and machinery spaces and from galleys, main pantries and staterooms other than small service lockers shall be of "A" Class construction... The doors shall be of a self-closing type. Inspector identified the door between the galley and accommodation space would not self-close. Ensure door between galley and accommodation space will self close to the satisfaction of the attending Marine Inspector. 46 CFR 32.57-10(c) 46 CFR 32.57-10(d)(4)

Each prime mover must shut down automatically upon loss of lubricating pressure to the generator bearings if the generator is directly coupled to the engine. If the generator is operating from a power take-off, such as a shaft driven generator on a main propulsion engine, the generator must automatically declutch (disconnect) from the prime mover upon loss of lubricating pressure to generator bearings. Inspector was unable to witness satisfactory low lube oil pressure alarm with engine shutdown on all three ships service generators. Provide third party report of satisfactory tests of ship service generator alarms for all three generators to the satisfaction of the attending Marine Inspector. 46 CFR 111.12-1(c)

The prime mover of a generator set must have: all accessories necessary for operation and protection of the prime mover. Inspector was unable to witness any tests of alarms on emergency generator and identified battery charger malfunction alarm activated on emergency generator alarm panel. Provide third party report of satisfactory tests of all alarms on emergency generator to the satisfaction of the attending Marine Inspector. 46 CFR 112.50-1(a)(1)

Each launching station must be positioned to ensure safe launching with clearance from the propeller and from the steeply overhanging portions of the hull. Vessels inflatable life raft was not positioned to be able be launched straight down, without interference from the railing. 46 CFR 199.120(a) 50 C

The crew accommodations on all tankships of less than 100 gross tons and all manned tank barges must have sufficient size and equipment, and be adequately constructed to provide for the protection of the crew in manner practicable for the size, facilities, and service of the tank vessel. MI found Refrigerators and microwave not secured in Galley. 46 CFR 32.40-60

During preparation, launching, and recovery, each survival craft and rescue boat, its launching appliance, and the area of water into which it is to be launched or recovered must be adequately illuminated by lighting supplied from the emergency power source. 46 CFR 111.75-16 (a)

When alteration or repair of boilers, pressure vessels, machinery, safety valves or piping systems becomes necessary, the work shall be done under the cognizance of the Officer in Charge Marine Inspection. It shall be done in accordance with the regulations in effect at the time such vessel or installation was contracted for or built (whichever is latest), or in accordance with the regulations in effect for new construction. 46cfr50.05-10(a) ***AND*** No extensive alterations involving the safety of a tank vessel either in regard to hull or machinery shall be made without the approval of the Commandant. Before such alterations are carried out, copies of plans and specifications in triplicate for the work involved shall be forwarded to the Officer in Charge, Marine Inspection, in whose zone the repairs will be made, for submission to Headquarters for approval. Vessel removed air compressor and pressure vessel without notifying OCMI. Vessel installed an air compressor skid on the 02 deck. 46 CFR 31.10-25

Approved fire extinguishing systems must be installed on all tankships in the following locations. Previously approved installations may be retained as long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspections. Paint locker had 2 solid shelves (top and bottom) and sealed. Extinguishing agent wouldnot be able to reach the bottom. 46 CFR 34.05-5(a)

Permit assembly and disassembly under dim ambient conditions without damaging gaskets and seals and without joints. Vessel had (2) floating electric water lights (FWD port and Stbd sides of main deck) with failed gaskets effecting the watertight integrity of the light. 46 CFR 31.36-1 --> 46 CFR 199.70(a)(3)(ii) --> 46 CFR 161.010-2 --> ANSI/UL 1196.4.6(a)

Each vessel must have a means of communication among the following: (1) Navigating bridge. (2) Steering gear room, if outside the engine room. (3) Alternative steering station if outside of the steering gear room. (4) Engine control room, if the vessel has an engine control room. One in aft machinery space. Vessels phone in FWD engine and aft machinery room was inop. 46 CFR 31.35-5 --> 46 CFR 113.30-5

It shall be the duty of the master and chief engineer of every tankship to see that such vessel and crew's quarters are kept in a sanitary condition. 46 CFR 35.01-5 ***AND*** Waste water drains from ice boxes, refrigerating equipment, and refrigerated spaces shall be so installed as to prevent backflow of contaminating liquids. Vessels ice maker did not have a backflow preventer on its drain line. 21 CFR 1250.34

At each inspection for certification and periodic inspection the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation. The number (1) ship service generator'S low fuel psi alarm did not work, the low lube oil level alarm did not work. The over speed was tested and tested sat. 46 CFR 61.20-3

Casing doors in category A machinery spaces and all elevator doors must be self-closing. The Vessel’s door between the crews mess and accommodation space is an A-60 boundry and the door did not have a closing device. 46CFR32.56-35 (a)

Barges with sleeping accommodations for more than six persons and all self-propelled vessels shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans: Vessel did not have fire plan by the brow. 46 CFR 35.10-3

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. Vessel had an expansion joint in the Fire main system that they admitted was over 10 years old. 46 CFR 61.15-12(b)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include - (1) Closing each valve leading to an overboard discharge and removing the handle; (2) Padlocking each valve leading to an overboard discharge in the closed position; or (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. MSD over board valve was not locked. Vessel is replacing MSD with new system. 33 CFR 1593.7(c)

The alcohol testing devices need not be carried on board each vessel if obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Vessel is enrolled in a consortium and do not have means of testing if off shore. 46 CFR 4.06-15 (a)(3)

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Engine room ventilation fans had excessive dirt build up on fan motor and blades and could possibly ignite. 46cfr92.05-1 (a)
